1 This guide is meant to be a starting example that should be customized by your organization. This guide should support your Safety VSP. Covered Topics: Inspection Approach Inspection Considerations Severity Evaluation Counting Guidelines Per Inspection Approach SafetyNet allows for diverse styles and approaches for capturing the level of safety in an area or within an organization. Some best practice approaches are outlined below. 1. Select a single category and inspect for it in a comprehensive manner. a. For instance, you could select ladders and inspect every ladder onsite for all its subcategories. This method is commonly used to satisfy an OSHA requirement such as Fire Protection (extinguishers) or for an Assured Grounding Program. 2. Choose a specific task or operation and inspect every condition and process for that task. a. An example of this could be work in an electrical panel. When working within an electrical panel, inspecting tasks could involve the following categories: PPE, Electrical, Housekeeping, /Public Protection, Hand and Power s, and Medical/Emergency. b. The intent of this inspection would be to measure the level of safety only for that particular task or operation. 3. Walk around an area, taking mental snap shots and recording what you see (The Snap Shot method) a. Select a specific work group or crew and capture all observations related to that work group or crew only. Inspection Consideration SafetyNet s flexibility is a key component of its success. SafetyNet allows for diverse styles and approaches for capturing the level of safety in an area or within an organization. Some best practice approaches are outlined below. 1. Ask the following questions when conducting an inspection: QUESTION YES NO Is the work area set up properly? Are the tools and equipment appropriate? Are the correct materials being used? Are the appropriate procedures being followed? Are employees exposed? Severity Evaluation 1. Determining Severity

2 LIKELIHOOD a. HIGH Severity = injury requires hospitalization or worse b. LOW Severity = injury does not require hospitalization 2. Determining Likelihood a. Four factors help determine the likelihood of an adverse event. 3. Number of employees exposed a. <5, or >5 4. Frequency and duration of exposure a. 10 minutes/month intermittent vs. 8 hours/day all day every day 5. Environmental conditions a. Dry, moderate temperature, well-lit vs. Wet, hot, low light 6. Stress factors a. Speed b. Crowding c. Other hazards in the area that make this hazard more likely to occur 7. Determining Exposure Level a. Use the table below to establish exposure level based after you have determined severity and likelihood. CONSEQUENCE Low High Low LOW HIGH High MEDIUM LIFE-THREAT
