8 All rollers in good shape Free of Oil leaks Proper Air pressure to brakes Has swivel packing been changed after each air section of well or every well when NOT drilling on air Wiggle steer working properly PIPE HANDLER Working properly Lined up properly with Top-drive Dies ground down Free of leaks Properly roped off Pins and Bushings are in good shape and properly greased Airline is connected to airbag and airbag is inflated Cylinder bushing/pivot pin being greased (as per HWD policy) at least 4 times in a 24 hour period STAIRS, WALKWAYS, HANDRAILS, GUARDRAILS Adequate stairs provided throughout all equipment on location Stairs level, secure, with no obstructions Stairway platforms are bolted at top where necessary Stair treads are uniform, of non-skid type Guardrails, mid-rails, and toe boards are installed Guardrails, mid-rails, and toe boards are free from damage FUEL STORAGE TANKS Fuel tanks properly located Discharge nozzles, hoses, and valves are in good working order Piping and fuel lines inspected and free of damage Stationary ladders on tanks are inspected and in good working order NO SMOKING signs are posted Tanks properly labeled as to their contents Tanks located inside containments FIRE PROTECTION Adequate fire extinguishers are located around the entire location 8

9 Fire extinguishers are inspected weekly, are operational, and not obstructed Flammables are stored in U.L. safety cans Welding operations are performed safely with F.E. and firewatch present Hotwork permits are filled out appropriately Spark and heat arrestors are operational and present on engines Flare area clear of combustibles Comments: Superintendent: Print Signature Date 9
