1 CONCORDIA UNIVERSITY Office Inspection Checklist Offices/Classrooms/General Office Areas SCOPE The Office Inspection Checklist is a tool to identify workplace hazards and as well as to provide a system to monitor whether these hazards have been corrected. This checklist does not apply to areas such as laboratories, workshops, or service rooms. PROCEDURES Every area in all Faculties and Departments should be inspected at least once per year. The inspection should be carried out by at least a supervisor accompanied by an employee. The inspection report should be posted for 1 month in a conspicuous area. Workplace inspections require a systematic approach so as to identify all hazards. You may choose to split your area into smaller sections and tackle them one at a time. The checklist can be used to inspect: Offices Classrooms Conference rooms Office storage rooms Seminar rooms To identify the hazards: Look at the workplace environment Look at the equipment or substances Observe work practices Talk to individuals involved (faculty/staff/students) Use the checklist TRAINING Any person requiring assistance to complete the checklist can contact the Environmental Health and Safety Office at Revised July 2006 Lina Filacchione 1

Are broken chairs, desks, bookshelves, and other furniture removed from the office? Are the ceiling tiles in place and free of any water leaks, or stains, etc? Are garbage bins routinely emptied? Is sprinkler system operational (sprinkler heads free from damage)? Are radiators/space heaters free of combustible material (ex. paper, cardboard)? Are emergency phones in working order? Is a list of CERT (Concordia Emergency Response Team) members posted? Emergency Exits Are emergency exit doors marked and clearly visible? Can emergency exit doors be opened easily at all times? Are exit corridors clear of obstructions? Is access to emergency exit stairwells, ladders, catwalks and fire escapes clear of obstructions? Are exit fire escapes cleared of snow or built-up ice in a reasonable time frame? Are all emergency and evacuation procedures (floor plans) displayed? Electrical Are there any broken plugs, sockets or switches? Are cords frayed or damaged? Are there any temporary or makeshift power boards? Are electrical circuits or outlets overloaded? Are extension cords only used for temporary purposes? Are coffee machines turned off and/or unplugged every night? If space heaters are used, are they approved (i.e. CSA or by Facilities Management)? Lighting Is the lighting adequate and in good condition? Is glare and reflection adequately controlled? Are there any flickering lights? Is emergency lighting available and in working order? Is mechanical lifting equipment available for lifting heavy loads? Is adjustable seating available when appropriate? Are footrests available for those who need them?
