WHEN CALLING 911 OR CSUPD WITH AN EMERGENCY In the event of a fire, smoke, explosion, hazardous spill, toxic leak, releases of flammable or toxic materials, or acts of nature which potentially endanger lives, the following procedures should be followed: PHONE State your name. 2. State the purpose of the phone call (police, fire, ambulance needed, etc.) 3. State the location of the emergency including building name, building address, room # and/or area. EX: Veterinary Teaching Hospital, 300 West Drake Road, Building A, Anesthesia, Room C State what type of emergency has occurred (fire, chemical spill, medical, etc). 07/11/2014 Page 4 of 20

5 5. Indicate if people are injured or trapped. 6. Provide information on special precautions that response personnel should take (see Special Areas ). 7. Let the dispatcher end the phone call thus assuring they have all the information they need or stay on the line as instructed. POLICE RESPONSE 1. Faculty, Staff and Students may witness a response by University Police to a situation requiring police presence within your building. In the event of a building-wide emergency, Police Officers may direct you to either stay in your office or classroom, not enter the building, or to leave the building immediately. 2. If you are contacted by a Police Officer, ALWAYS follow their instructions, as you may not know the reason that an officer is giving you instructions, or what danger(s) may exist. Failure to follow officer instructions could endanger yours as well as the lives of others, and could lead to your arrest for failing to comply with the officers instructions. 3. Do not run from a police officer when contacted or instructed in any way. This could lead to injury to you or others, or enforcement action being initiated against you. 4. Some buildings contain security or life and safety alarm systems that when activated, in most cases, require Police response. Until the alarm is proven to be false, police respond as if there is an actual intrusion, or emergency. Even if you know the alarm to be false, you must STILL follow all instructions from responding police officers as well as any protocols that have been established for an alarming system within your building. 5. In any event where University Police have been dispatched to your building, the Police will follow up by contacting no more than two (2) responsible parties within the affected building to explain the response, even in the event of a false alarm. Those contacted may include department head(s), dean(s), or building proctors who will then be responsible in determining who/when to distribute information regarding the police response to a given situation. It will be the responsibility of each building to provide 24/7 contact information for those responsible parties to the Police Department, as well as update that information as necessary. EMERGENCY PREPAREDNESS EVACUATION PLAN 1. Faculty who are teaching classes at the time of the emergency are responsible for the orderly evacuation of the class. 2. DO NOT take time to turn off computers, printers, or office lights. Close, but DO NOT lock, office door. 3. Exit the building through the closest exit. 4. All personnel should be familiar with the exit paths for their areas. REFER TO THE VTH EVACUATION PLANS and be familiar with the shortest path possible. 5. Proceed in an orderly manner as quickly as possible to the nearest exit and then to the designated reporting area. 6. Stay in the designated reporting area until you are instructed to leave. Faculty and Lab assistants are responsible for the students. 7. Upon arrival of University Police, the proctor will assist them in whatever manner they request or direct. Poudre Fire Authority staff and/or the University Police will clear the building, checking elevators, areas for the use of the physically disabled, and laboratory areas in the building. 8. Emergency alarms being turned off DOES NOT mean the building is clear and safe to re-enter. They are silenced so that emergency response personnel are able to communicate with each other. DO NOT RE-ENTER THE BUILDING FOR ANY REASON until instructed to do so by fire department, EHS, police officials or Hospital Administration. 9. Evacuation of animals from an endangered area in the building or barns should be carried out ONLY IF HUMAN LIFE WILL NOT BE THREATENED. It is important to check with emergency response personnel (fire, police etc.) if there is a question as to whether animals can be evacuated safely. The fire department or police will be the final authority on this matter. 10. A skeleton crew of necessary personnel may remain in the Critical Care Unit, Urgent Care, Large Animal Surgery, Livestock Surgery, Small Animal Surgery, Endoscopy, Dentistry and the Small Animal Anesthesia Prep room (to insure the health and safety of critically ill or anesthetized animals), until they are notified by emergency response personnel to evacuate the building. 6 make this notification using the intercom system in these designated areas, or in person. Because there is only an intercom for the Small Animal Surgery area, those personnel are responsible for notifying Anesthesia prep room personnel of the evacuation signal sent. 11. If you are working with an anesthetized or critically ill animal in another area of the VTH and must continue to provide care, you must take the animal to one of the designated areas above so that changes in evacuation status can be communicated. 12. This evacuation plan includes weekdays, weekends and Holidays. EXERCISES, DRILLS AND REVIEWS 1. Emergency exercises, drills and reviews should take place once each semester or as directed by accreditation processes specific to each lab or area within the facility. At least annually, a building-wide drill or table top exercise will take place. For South Campus facilities, the South Campus Facility and Safety Manager working with building proctors or department designee/s in a coordinated effort is responsible for maintaining a calendar of planned exercises and procedure/plan updates resulting from the exercises. An actual emergency or false alarm can count as an exercise if a debriefing with staff and follow up review of the plan are completed within 10 working days. 2. Emergency exercises or reviews should be discussed during a staff meeting or training to review the emergency plan for the department and each staff member s role or responsibility. Alternately, an orientation exercise may be offered on-line. On-line information should include a mechanism to note an individual has read and understands presented information. 3. A table top exercise involves a scenario that involves one section of the emergency plan and discussion of all department members roles and responsibilities in executing the plan, getting to safe gathering areas, communicating during an emergency, then reviewing the exercise and making changes in the plan as appropriate. 4. A drill involves actually playing through a situation that tests one or more portions of the department s emergency plan. 5. All South Campus fire drills will be initiated by the South Campus Facility and Safety Manager and must be coordinated through the Environmental Health Services Fire Safety Officer who will work with CSU alarm shop, CSUPD and responding authorities such as Poudre Fire Authority. Drills are coordinated with neighboring department and building proctors to prevent panic and confusion. SAFETY, SECURITY INSPECTIONS AND TRAINING PREPARATIONS 1. Building proctors or designees are to know who completes and how often maintenance checks are performed for AEDs, fire extinguishers, exit lighting, sprinkler systems and fire department water connections. Proctors are to assure that inspection tags are current and visible on the equipment devices, or that a written report is available if requested through campus Facilities Management. Building occupants should know localities of emergency equipment (AEDs, fire extinguishers) and locations fire pulls. 2. CSU will notify building proctors prior to emergency generator testing. 3. Annually proctors or assigned department personnel are to review designated gathering areas to ensure that they still accommodate department needs. Also check secondary (weather contingency) locations and update agreements with other buildings or departments as needed. Proctors or assigned department personnel are to review exit/evacuation routes, shelter locations and posted maps to ensure all are accurate and up-to-date. Building occupants are responsible for knowing evacuation policies and procedures. 4. Responses and actions to as-needed, monthly, quarterly, and/or annual orientations, table-top exercises or drills are to be clearly outlined for building occupants. Occupants are responsible for accessing and understanding presented information and responding procedures. TRANSPORTATION OPTIONS Before an emergency occurs, consider how critical staff will be transported to/from work during floods, snowstorms, etc. if they have insufficient transport capability on their own. Consider assigning volunteer drivers with suitable vehicles to safely transport personnel so that operations can continue on a limited emergency operations status. WARNING AND NOTIFICATION 1. Building occupants and personnel are to be periodically advised how warnings and emergency notifications and alerts will be forwarded. People will know via information forwarded by safety officials, administrative representatives, building proctors and/or spokespersons, various announcement services available and how to sign up for warning notifications. 2. All emergency contact information including notification trees and associated processes, are to be current at all times for buildings/departments. After-hours and emergency contact information is to be available to building occupants and personnel through physical postings, on-line availability, and/or through Facilities dispatch. 07/11/2014 Page 6 of 20

7 1. If the shooter is outside your building: EMERGENCY RESPONSE PROTOCOL AND INFORMATION ACTIVE SHOOTER/ARMED PERSON a) Turn off all the lights, close and lock all windows and doors. If you cannot lock the door, try to block the door with desks and chairs. b) If you can do so safely, get all occupants on the floor and out of the line of fire. c) If you can do so safely, move to the core area of the building and remain there until the police tell you it is safe to leave. Do not respond to commands until you are certain they are issued by a police officer. 2. If the shooter is inside your building: a) If it is possible to escape the area safely and avoid danger, do so by the nearest exit or window. Leave in the room books, backpacks, purses, etc. b) As you exit the building, keep your hands above your head and listen for instructions that may be given by police officers. If an officer points a firearm at you, make no movement that may cause the officer to mistake your actions for a threat. Try to stay calm. c) If you get out of the building and do not see a police officer, attempt to call the police by dialing 911. Tell the dispatcher your name and location and follow their instructions. d) If you are unable to escape the building, move out of the hallway and into an office or classroom and try to lock the door. If the door will not lock, try barricading the door with desks and chairs. Lie on the floor and/or under a desk and remain silent. Wait for the police to come and find you. 3. If the shooter enters your office or classroom: a) There is no set procedure in this situation. If possible, call 911 and talk with a police dispatcher. If you cannot speak, leave the phone line open so the police can hear what is going on. b) Use common sense. Attacking the suspect is a last resort but may become necessary. c) Attempting to overcome the suspect with force is a last resort that should only be considered in the most extreme circumstances. Only you can decide if this is something you should do. Remember there may be more than one shooter. d) If the shooter exits your area and you are able to escape, leave the area immediately. Do not touch anything in the area and remember to be alert for responding police officers who may mistake you as the shooter. While escaping, as soon as you see a police officer put your hands over your head and immediately comply with the officers instructions. 1. Remain Calm. BOMB THREAT (RECEIVED VIA PHONE CALL) 2. Record every word spoken by the caller and any background noises. Follow up immediately by completing the Bomb Threat Questionnaire (page 17). 3. Alert a co-worker via note (if possible) while on the line with the bomb threat. Have co-worker call University Police at 911 and have the following information available; where the bomb threat is, who is taking bomb threat call, an estimate of how many people are in the building, and have them stay on the line until the dispatcher tells them to hang up. 4. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch 5. Notify the department head. 6. Notify the building proctor. 7. Evacuate the building immediately. See Emergency Telephone numbers on page 3 to reach department head or building proctor. EARTHQUAKE 1. Take cover under heavy furniture a table, desk, or bench or within a doorway. 2. Keep away from glass. 3. Wait for quake or tremor to subside and all falling objects to come to rest. 07/11/2014 Page 7 of 20

8 4. For small quakes and tremors with NO apparent damage, return to normal activities. Building proctors will survey the entire building for possible damage such as; leaking pipes, fallen books, etc. All proctors will meet in designated areas to report damages to University Police. 5. Remain calm. 6. If damage appears heavy, evacuate ONLY when notified by University Police that it is safe to leave. 7. Proceed immediately to designated area. 8. Stay away from electrical power sources, fallen lines, buildings, or other tall objects. 9. Do NOT smoke. Gas lines may have ruptured. See Emergency Telephone numbers on page 3 to reach department head or building proctor. FIRE/SMOKE 1. Pull the red fire alarm. Refer to the floor plan and be familiar with the nearest alarm location. An alarm will ring at CSU police. University Police will call the Fire Department. 2. If there is immediate danger, remain calm and follow evacuation procedures and then call Police Department at 911 from a nearby building. 3. If there IS NOT an immediate danger, call University Police Department at If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch. 5. Follow evacuation plan. Listen for directions and congregate at the designated area. Faculty and lab assistants will account for students. 6. DO NOT RE-ENTER BUILDING. 7. Call the building proctor. See Emergency Telephone numbers on page 3 to reach department head or building proctor. Designated Evacuation Location: Always remain at least 100 feet away from building. 1. Front parking lot past the circle drive south side of the hospital. (Section A) Primary: Kathie Sexton/Jessica Brand Secondary: Chad Alexander (Needs to speak to fireman) Kathie and Chad will both have a push-to-talk phone. Kathie will also have a walkie-talkie (charging in the director s office). 2. West parking lot past sidewalk. (Section B) Primary: Laura Chubb (ACC Lab Representative) and Sheri Hankins (VTH Representative) will work together. Backup: Laura s back up will be Barb Rose/Sheri s Back up will be Jen Tucker and/or Alysha Ducharme. There will be a walkie-talkie placed in Central for Sheri to use, and when Sheri is absent, it will be moved to Small Animal Surgery for Jen. 3. North of the round pen. (Section C) Primary: Linda Hutson Backup: Sherrie Henderson or Kerry Grohman Linda will use a walkie-talkie, which will be charging in the director s office. If Linda is absent for the day, Sherrie or Kerry will take their place with a walkie-talkie. 4. All after hour and weekend staff should evacuate to the front parking lost past the circle drive south side of the hospital. (Section A) IF YOU ARE TRAPPED IN THE BUILDING 1. If the door to the room you are in is hot to the touch and/or smoke is seeping in around it, DO NOT OPEN IT. 2. Remain calm. Walls, ceilings, floors, and doors are designed to withstand fire for a safe period of time. 3. Pack the crack under the door with clothing or other material to keep the smoke out. 4. Let someone know you are trapped. Call 911 and stay on the line until the dispatcher tells you to hang up. If there is no phone available, yell out the window, wave out the window to gain attention. 5. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch. 07/11/2014 Page 8 of 20

9 6. Stay low to the floor near the window as the smoke will fill higher areas first. FIRE EXTINGUISHERS ARE TO BE USED ONLY TO PUT OUT A PATH OF FIRE TO GET OUT OF BUILDING OR FOR A SMALL CONTAINED FIRE. See map on page for location of fire extinguishers or Fire maps posted throughout the building. 1. Evacuate the affected area. 2. Report to designated area. 3. Call Facilities for assistance in having water shut off. FLOODING INTERIOR FLOODING 4. Call University Police at 911 from a nearby building and have the following information available; where the flooding occurred, if there are any injuries and stay on the line until you are told to hang up. 5. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch. 6. Stay away from all power (electrical) sources. 7. Stay away from utility vaults. 1. Remain calm. EXTERIOR FLOODING 2. Call University Police at 911 and let them know what building you are in, how high the water is, and how many people are with you. 3. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch. 4. If there is water all around the building, proceed to the roof of the building or the highest point accessible. 5. If there is water on only one side of the building, proceed in an orderly fashion out of the building exit that has NO water. 6. Immediately go to the highest area possible. HAZARDOUS MATERIALS SPILL BENCH TOP SPILLS A bench top spill is defined as; a spill that will not contaminate the water supply, sewer, air handling system, or any other area, is small enough to be easily handled by staff, and there are NOT any injuries. 1. Remain calm. 2. Contain the spill with absorbent pillows. 3. Consult the Materials Safety Data Sheet (MSDS). 4. If you are familiar with handling the spilled reagent, obtain the proper spill kit and follow the directions that are with the spill kit. 5. Notify the Principle Investigator for the laboratory. 6. Dispose of all absorbent materials according to the Colorado State University Hazardous Chemical Waste Systems Manual. If you are not familiar with the spilled reagents or you do not feel comfortable cleaning up the spill, follow instructions for large spills. LARGE SPILLS A large spill is defined as; a spill that may contaminate the water supply, sewer, air handling system, or any other area, is too large to be easily handled by staff, and/or there are injuries. 1. If there is immediate danger, follow the evacuation procedures and call University Police at 911 from a nearby building and have the above information available. 2. Remain calm. 07/11/2014 Page 9 of 20

10 3. Only if there is NO immediate danger; call , have the following information available for the dispatcher; where the spill has occurred, what was spilled, how much was spilled, when the spill occurred, and if there are any injuries. Stay on the line until the dispatcher tells you to hang up. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch 4. Call the Principle Investigator for the laboratory that is listed on the lab door. ALL LABORATORIES SHOULD BE EQUIPPED WITH SPILL KITS FOR THE APPROPRIATE MATERIALS BEING USED IN THE LABORATORY (EXAMPLE, ACIDS, BASES, MERCURY, ETC) AND MATERIALS SAFETY DATA SHEETS (MSDS) SHOULD BE UPDATED AND ACCESSIBLE IN ALL AREAS OF THE HOSPITAL. 1. Remain calm. MEDICAL EMERGENCIES 2. Call 911 and stay on the line until the dispatcher tells you to hang up. 3. If calling 911 from a cell phone, the call will go to the Larimer County Dispatch Center. Immediately identify your location as at CSU and you will be transferred to CSUPD Dispatch. 4. Do NOT move the victim or give first aid unless you are trained and certified to do so. 5. Remain with the victim and try to keep the victim warm and alert by talking with them until emergency response teams arrive. NOTE: An Automated External Defibrillator (AED) is located in the following areas: Hallway outside of Medical Records At the top of the southwest stairs between ACC and Clinical Sciences. ODORS / INDOOR AIR QUALITY Unusual odors or smells should be reported to the Building Proctor or Assistant Proctor immediately so that Environmental Health Services can be dispatched to evaluate. In a laboratory setting, odors/smells should be immediately reported to the Lab Coordinator. ODORS / OUTDOOR AIR QUALITY In the event of a hazardous material spill outside that affects air quality, note the wind direction. Remember up! Stay upwind (stay with the direction of the wind), uphill, upstream. Follow instructions of emergency responders. Report any missing person or individual that was observed to have possible exposure to the spill. CALL 911 IF ANYONE IS HAVING MEDICAL OR PHYSICAL REACTIONS TO FUMES OR ODORS AND MOVE OUT OF THE AREA. OTHER EMERGENCIES ASSAULT, HARASSMENT, DESTRUCTION OF PROPERTY, VANDALISM, THEFT AND OTHER CIVIL DISORDER: 1. Call University Police immediately at Observe suspicious persons but DO NOT TRY TO DETAIN THEM. 3. Ask the victim to remain until University Police arrive. 4. Obtain names, addresses, and telephone numbers of witnesses. 5. Notify the building proctor. 6. Any individual directly threatened or in danger is to do whatever is necessary to escape, hide, or defeat the threat and call 911 as soon as possible! Involve supervisors, administrative authorities, police, Human Resources and others as needed during any event or suspected threat. See Emergency Telephone numbers on page 3 to reach department head or building proctor. POWER FAILURE 1. Facilities maintenance will be contacted by designated departmental personnel. 2. University Police will be contacted by designated departmental personnel after normal working hours and on the weekends. 07/11/2014 Page 10 of 20

11 3. Animal care/custodial staff should check room B113C, ACC 139, ACC 239, B225, B221, H101C (these rooms house a number of sub-zero freezers that need to be set). See Emergency Telephone numbers on page 3 to reach department head or building proctor. SNOW AND WINTER WEATHER 1. Administration will inform you of any closures during normal business hours. 2. In a severe storm, Housing Management along with other University operations will coordinate food and shelter as necessary. 3. Each section/service chief and the appropriate supervisors will set up plans that are specific to their sections and service areas which provide direction for staffing their sections/service areas at the appropriate level based on the presenting conditions. 4. Section/Service Area Chiefs and Supervisors will identify back-up personnel to fulfill responsibilities in their absence. 5. Weather-Related Personnel Staffing Plan for Hospital Operations should be reviewed by each section/service area annually for accuracy. These plans should also be reviewed with all staff within each section/service area annually and upon hire to ensure open communication and understanding of expectations. TORNADOS AND WINDS 1. There are weather alert radio spotters in designated areas including the Barns, Director s Office, CCU, ACC, Clinical Sciences and Reception. 2. Personnel will be notified of a Tornado Warning through the VTH overhead paging system and runners. 3. In the event of a tornado warning, please proceed to the designated VTH Tornado Evacuation Locations which are: First Floor a. Hallway in the area of Dermatology rounds room, Urgent Care and CCU b. Radiology c. Hallway by Large Animal Surgery d. Locker Rooms (either next to CCU, across from Central Supply or by Equine Surgery) e. Small Animal Wards Second Floor Move to the First Floor a. Hallway in the area of Dermatology rounds room, Urgent Care and CCU b. Radiology c. Hallway Locker Rooms (either next to CCU or Equine Surgery) d. Locker Rooms (either next to CCU, across from Central Supply or by Equine Surgery) e. Small Animal Wards Barns a.
