3 I. REASONS FOR THE PLAN The University of Saskatchewan has a responsibility for protecting life and property from the effects of fire, explosion, bomb threats and other emergencies. II. PURPOSE OF THE PLAN To develop a state of readiness which would facilitate efficient and orderly evacuation of people and possibly items such as cash, records, etc., in case of an emergency. 8 V. GENERAL INSTRUCTIONS 1. Action in Case of Fire (a) The alarm system in place will sense any fire and will sound the alarm automatically. In case of failure, the person discovering the fire will: (i) Sound the fire alarm by pulling the handle on one of the fire alarm boxes located in the hallways of the buildings on each floor. The sounding of the alarm will automatically alert the Fire Department and Campus Safety. (ii) Indicate the location of the fire when the Fire Warden, or his/her Assistant arrives, and, if required, assist in extinguishing the fire. (b) On hearing the fire alarm, all other persons will proceed out of the building using the predetermined evacuation routes. ELEVATORS MUST NOT BE USED. (c) Supervisory personnel must have PREDETERMINED what special attention should be given to cash, personal records or classified material in case of a fire or an emergency. Supervisors should ensure that employees are aware of, and periodically briefed on, procedures to be followed. (d) All instructors or demonstrators who are conducting a lecture, tutorial or lab at the time of an alarm must immediately ensure that all members of their class immediately exit the building by the predetermined exit routes. Only after all class members have been evacuated should they attempt to assume any other fire-related responsibilities. (e) It should be HIGHLY STRESSED that no person is to be allowed to remain in the building on the basis of their own downplay of the dangers involved. 2. Fire Alarm Boxes Fire Alarm boxes (pull stations) are coloured red and are located near every posted exit and stairwell from all floors. 3. Fire Extinguishers These are located on all floors, usually near the fire alarm boxes and are for the use of all employees in an emergency. 4. Fire Alarm System Inspections On the first Monday of every month, the fire alarm system is inspected by Facilities Management. The checking of the alarm system usually occurs in the morning and includes several soundings of the alarm at regular intervals lasting less than 10 seconds. These tests are necessary to ensure that the system is functioning. 5. Exits All employees should familiarize themselves with the nearest exits from offices and buildings and be fully aware of alternate exits. FIRE PLAN 2011.doc 8

9 6. Evacuation Routes Floor Wardens and Assistant Floor Wardens will direct the complete evacuation of the building following the routes described in Section X. 7. Visitors and Disabled Persons -Visitors in any office should accompany the group from that office. -Floor Wardens will ensure that disabled persons are assisted from the building. -Disabled persons in a wheelchair, which are not at the time of the alarm on the main floor, should wait by the proper staircase to be lifted and taken out of the building by the firemen. The Floor Warden in their area should stay with them during the alarm and relay the information of their location to the Chief Fire Warden through any of their Assistant Floor Wardens. The disabled person may request assistance with evacuation in which case the floor warden should find volunteers to lift the person in the wheelchair up or down stairs and outside. REMEMBER: ** If a fire is discovered, sound the fire alarm. ** ** On hearing the fire alarm, evacuate the building. ** ** Walk, do not run. ** ** Keep to the right on staircases and in corridors. ** ** If requested, assist your fire warden or floor warden. ** ** Know the evacuation routes from your building. ** FIRE PLAN 2011.doc 9

10 VI. GENERAL JOB DESCRIPTIONS FOR WARDENS A. CHIEF FIRE WARDEN Responsibilities: To provide leadership, assistance and follow up in assuring the establishment and continuity of the plan. To ensure the safety of all employees, students and visitors in the building in case of a fire. Duties: Regular: (ongoing) Emergency: - To recruit all wardens. - To establish fire emergency procedures and coordinate training. - To act with regard to fire hazard reports from floor wardens. - To act as a liaison with Facilities Management personnel and Security concerning any emergency. - In case of a fire alarm he/she will immediately meet with the Deputy Fire Warden at the annunciator panel outside Theatre 107. The Deputy Fire Warden will remain there to give information to the City Fire Department officials upon their arrival and direct them to the area of emergency. - In the meantime, the Chief Fire Warden will locate the fire and, if controllable, take action to extinguish it. If not, he/she will leave the room closing all windows and doors and inform the firemen of any special hazards. - He/she will then check with the Fire Wardens in the area of the Report Center to verify the accountability of all individuals from each floor. - When appropriate, upon authorization from the City Fire Department official, he/she and/or his/her Deputy Fire Warden will communicate instructions to allow reoccupation of the building, etc. B. DEPUTY FIRE WARDEN Responsibilities and Duties: Regular: - To assist the Chief Fire Warden in his/her duties and responsibilities. (ongoing) - To recruit someone else to perform his/her duties in the case of his/her absence. Emergency: - If the Chief Fire Warden is present at the time of the alarm, his/her duties will be as described in Section VI A. - In the case of the absence of the Chief Fire Warden from the building, the Deputy Fire Warden will act in his/her stead and designate a suitable person to take over his/her own duties. FLOOR WARDENS Responsibilities and Duties: Regular: - To be familiar with building fire emergency procedures and the Fire (ongoing) Prevention Code. - To regularly check their area for fire hazards and take action to alleviate them or report to the Chief Fire Warden if the hazard is serious. - To be prepared for the safe evacuation of all individuals including the disabled and visitors in their area if an alarm sounds. - TO ARRANGE FOR THE CONTINUANCE OF THEIR DUTIES WHEN THEY WILL BE ABSENT FROM THE BUILDING DURING WORKING HOURS. Specific conditions to watch for are: - inoperative, missing or tampered-with fire fighting equipment. - careless use or storage of volatile liquids or other materials. - unnecessary leaving open of elevator or fire doors. - exit light not working, etc. Emergency: - Direct all persons out via this chosen route(s). - Close any hallway doors that have been propped open. - Upon evacuation, ensure that door guards are posted at each exit to keep persons from re-entering the building. If the assigned door guards are absent, request any person standing nearby to assume that role. (See map 7) - Receive the accountability report for all individuals from the Assistant Floor Wardens and relay this information to the Chief Fire Warden at the report centre. - If there is any person in a wheelchair in their area and they are not on the main floor, the Floor warden should stay with the wheelchair person by the appropriate staircase and wait for the firemen. An Assistant Floor Warden should also be instructed to relay the wheelchair person s location to the Chief Fire Warden at the report centre. - Act on any possible orders from the Chief Fire Warden. - Inform his/her assistants and all door guards upon receiving word from the Chief Fire Warden when the building may be re-occupied, etc. - If the emergency has occurred in the warden s area, then he/she should assess the situation and take appropriate action to ensure the safety of personnel and notify the Chief Fire Warden or Fire Department of any special hazards in the immediate area of the emergency. - After all clear is given by Chief Fire Warden, Fire Department or WSEP/Campus Safety personnel, advise all of your door guards and tunnel guards to permit persons to re-enter building. - Ensure that the evacuation duties of any absent assistant floor warden are completed. ASSISTANT FLOOR WARDENS Responsibilities and duties: Regular: - They must be familiar with the building fire emergency procedures and the (ongoing) Fire Prevention Code. - THEY MUST ARRANGE FOR THE CONTINUANCE OF THEIR DUTIES IN CASES WHEN THEY WILL BE ABSENT FROM THE BUILDING DURING WORKING HOURS. - They must regularly check their respective area of the building in accordance with details outlined in Section VI - C. Emergency: - Immediately and systematically check all rooms in their area of responsibility ordering all occupants out and closing all doors and hallway doors. - Relay this information to the Floor Warden at the respective exit accounting for everyone in their area. - If a door guard has not yet reached any exit, act as door guards at the exit until word is received from the Fire Warden with regard to re-occupying the building, etc. - Replace the Floor Warden if he/she is absent at the time of the alarm and ask another person to assume your assistant floor warden duties. E. DOOR AND LINK GUARDS Responsibilities and duties: Emergency: - Immediately proceed to your assigned door and prevent any unauthorised person from prematurely entering the building until the emergency is over. You may choose to proceed outside the building to your appointed station if that is not your closest exit. Insist that building occupants stand clear of the entrance so that emergency response personnel may easily enter. Note that floor wardens may need to reenter building at SE new wing doors D49, 50 to report to Chief Fire Warden. You may allow all persons to re-enter the building once advised by the floor warden for your area after he/she receives authorisation from the Chief Fire Warden, the Fire Department or WSEP/Campus Safety personnel. If the link area is safe, Link guards are to stand at the end of the ramp from Geology and direct any persons coming from Geology to exit the building via the Geology/Physics rear exit. You should prevent unauthorised persons from entering physics building via this link during an emergency. You may allow all persons to re-enter the building once advised by the basement floor warden after he/she receives authorisation from the Chief Fire Warden, the Fire Department or WSEP/Campus Safety personnel. 13 F. TUNNEL GUARDS Responsibilities and duties: Emergency: Immediately walk through tunnel to their station at the College building tunnel entrance. You should prevent unauthorised persons from entering physics building via the tunnel during an emergency. In the unlikely event that there is a simultaneous emergency in both physics and college buildings, you should be met in the tunnel by the College building tunnel guard. Should this occur, you should return to the physics tunnel entrance and prevent persons from exiting physics via the tunnel. You would then exit the building once advised by the Floor Warden that the physics basement has been evacuated. You may allow all persons to re-enter the building once advised by the basement floor warden after he/she receives authorisation from the Chief Fire Warden, the Fire Department or WSEP/Campus Safety personnel. G. ALL FACULTY AND STAFF MEMBERS: - ARE ASSISTANT WARDENS - EACH INSTRUCTOR IS RESPONSIBLE FOR HIS/HER CLASSROOM FIRST DURING AN EMERGENCY AND MUST SEE TO THE IMMEDIATE AND COMPLETE REMOVAL OF HIS/HER STUDENTS THROUGH THE PREDETERMINED EXIT ROUTE BEFORE ASSUMING ANY OTHER FIRE- RELATED DUTIES. 14 VII. REPORT CENTRE Location: In front of the annunciator panel, outside Theatre 107 (Map 7, page 32.) VIII. MARSHALING AREAS If the weather is: pleasant: outside, in vicinity of respective exits. harsh: in either of 3 adjoining buildings: - OLD/NEW ADMINISTRATION BUILDING - JOHN MITCHELL BUILDING - KIRK HALL (Map 7, page 32) DO NOT USE THE BIOLOGY AND GEOLOGY BUILDINGS. IX. ALARM SYSTEM ZONES The buildings of Physics and Geology share a 3-zone alarm system as depicted in the diagram. - When the alarm originates in G then it also sounds in P0 but not in P. - When the alarm originates in P then it also sounds in P0 but not in G. - When the alarm originates in P0 then it sounds in both G and P. Map 1: 3-zone alarm system The rooms belonging to P0 are: 59, 60, 159, 160, 261, 263 and 312. To avoid confusion when the alarm is sounding in G and P0 but not in P, the occupants of offices in P that are nearby P0, should play it safe and evacuate. FIRE PLAN 2011.doc 14

16 A) BASEMENT OLD WING WARDENS AND GUARDS Floor Warden: Heather Friesen Responsible for all of basement floor in old part of Physics Building including Betatron wing. Review duties listed in Part VI section C. During emergency, any wheelchair-bound persons should be brought near door D39,40 Physics rear door. (If they request, seek assistance to lift them up the 5 steps to the outside.) After evacuation of area is complete, receive reports from assistant floor wardens at each door, assign a guard if nobody is present and then walk outside to Report Centre to advise Chief Fire Warden. After all clear is given, advise your door guards to permit persons to re-enter building. Assistant Floor Wardens: Lori Weitzel Responsible for evacuation of Rooms 56 to 63. After evacuation of assigned rooms is complete, go to Geology rear doors 37,38. Review duties listed in Part VI sections C and D Assume Floor Warden duties if Heather Friesen is absent. Rooms 56 to 63 EXIT - 1st choice: North of hallway via D39, 40 toward Kirk Hall. - 2nd choice: North of room 60, via D37, 38 toward Kirk Hall. - 3rd choice: Up the stairs and out via D5, 6 toward Admin. bldg. (Maps 2 & 3) Jennifer Fasthuber Responsible for evacuation of Rooms 51 to 55 and 63.1 to 72 and 76. After evacuation of assigned rooms is complete, go to Physics rear doors 39,40. Review duties listed in Part VI sections C and D Assume Floor Warden duties if Heather Friesen and Lori Weitzel are absent. Rooms 51 to 55 and 63.1 to 72 and 76 EXIT - 1st choice: North of hallway via D39, 40 toward Kirk Hall. - 2nd choice: Up the stairs and out via D5, 6 toward Admin. bldg. (Maps 2 & 3) Dr. Hirose and Dr. Ghezelbash Responsible for evacuation of Rooms 74 to 74.6 (Betatron wing). After evacuation of assigned rooms is complete, go to doors D41,42 (Betatron rear). Review duties listed in Part VI sections C and D Assume Floor Warden duties if Heather Friesen, Lori Weitzel and Jennifer Fasthuber are all absent. Room 74 EXIT - 1st choice: Via the north Betatron exit D-41, 42 toward Kirk Hall. - 2nd choice: To the main hallway, up the stairs and out via D5, 6 toward Admin. bldg. 17 B) BASEMENT NEW WING WARDENS AND GUARDS Floor Warden: Dave McColl Responsible for all of basement floor in new part of Physics Building. Review duties listed in Part VI section C. Ensure that all hallway doors have been closed. If safe, any wheelchair-bound persons should be brought outside via Loading Dock Alternate Exit D43. If that area is not safe, bring them to basement of stairs opposite women s washroom 17, and request another person to report their presence to the Chief Fire Warden at the Report Centre while you remain with them. (The stairwells are built to be relatively safe during a fire while waiting for a fireman. If they request, seek assistance to lift them up the steps to the main floor.) After evacuation of area is complete, receive reports from all assistant floor wardens. If unsure whether tunnel guard is at station, send one assistant warden to act as extra tunnel guard at College building end of tunnel. You should then go to Report Centre to advise Chief Fire Warden. After all clear is given by Chief Fire Warden, Fire Department or WSEP/Campus Safety personnel, advise all of your door guards and tunnel guards to permit persons to re-enter building. Assistant Floor Wardens: Dr. Dazhi Liu Responsible for evacuation of Rooms 20 to 32 (Plasma lab corridor). After evacuation of assigned rooms is complete, report to Floor Warden and then exit building by nearest exit unless otherwise instructed by Floor Warden. Review duties listed in Part VI sections C and D Assume Floor Warden duties if Dave McColl is absent. Rooms 23 to 32. EXIT - 1st choice: If safe, out through the Loading Door alternate exit D43. (Map 2) - 2nd choice: Up the North stairwell one floor and out through the N.W. door, D45, 46 to the Kirk Hall bldg. (Maps 2 & 3) - 3rd choice: Up the stairwell opposite women s washroom and out through the SE main door, D49, D50 towards Administration/College building. (Maps 2 & 3) Rooms 20 to 22. EXIT - 1st choice: Up the stairwell opposite women s washroom and out through SE main door, D49, D50 towards Administration/College building. After evacuation of assigned rooms is complete, report to Floor Warden and then exit building by nearest exit. Review duties listed in Part VI sections C and D Also assume Dr. Liu s Assistant Floor Warden duties if Dave McColl is absent. Assume Dave McColl s Floor Warden duties if both Dave McColl and Dr. Liu are absent. Rooms 13 to 17. EXIT - 1st choice: Up stairwell opposite women s washroom and out through the SE main door, D49, D50 towards Administration/College building. (Maps 2 & 3) - 2nd choice: If safe, out through the Loading Door alternate exit D43. (Map 2) - 3rd choice: Up the North stairwell one floor and out through the N.W. door, D45, 46 to the Kirk Hall bldg. (Maps 2 & 3) Blair Chomyshen Responsible for evacuation of Rooms 1 (study corridor), 4 (student lounge), 6 (computer lab), 8, 10 and 11 (graduate student offices). Check that door to old part of basement is closed and that all hallway doors are closed. After evacuation of assigned rooms is complete, report to Floor Warden and then exit building by nearest exit. Review duties listed in Part VI sections C and D Rooms 1, 4, 6, 8, 10, 11. EXIT - 1st choice: Up the stairwell near student lounge and out through the South main entrance, D1-D4 towards bowl. (Maps 2 & 3) - 2nd choice: Up the stairwell opposite women s washroom and out through the SE main door, D49, D50 towards Administration/College building. (Maps 2 & 3) - 3rd choice: Out through the Loading Door alternate exit D43. (Map 2) Ted Toporowski Responsible for evacuation of Rooms 22 and 33 to 46 and closing of hallway doors. Note that Room 22 is accessed through Room 42 but the students should exit into Plasma corridor and then turn right to exit via stairwell opposite women s washroom. After evacuation of assigned rooms is complete, report to Floor Warden and then exit building by nearest exit and check that door guard is stationed at loading dock. Review duties listed in Part VI sections C and D Rooms 33 to 46. EXIT - 1st choice: Out through the Loading Door alternate exit D43. After evacuation of area is complete, then go outside to Report Centre near Room 107 and advise Chief Fire Warden.
