2 Section 1 MOLD/IAQ SCREENING REPORT General: Subject: Mold Screening Findings Date: 11/1/12 Time: 1:00 pm Location: 123 ABC Lane, Any City, LA Client: Home Buyer or Seller Inspector: Chris Savoie, CMA, CIE, CREI Company: Inspection Connection, 156 Egret, Covington, LA Weather: Sunny Temperature: 75 Degrees Section 2 Method: Visual inspection of the entire dwelling based on the Indoor Environmental Standards Organization, Standards of Practice for the Assessment of Indoor Environmental Quality. This inspection is to provide a general characterization of the indoor air quality of the dwelling. A visual inspection of the residence was performed and pertinent data and information was collected. This assessment is not a certificate, assurance, warranty or guarantee of future conditions that may occur, but is an evaluation of the conditions, which were present and detected on the date of the inspection. Three air samples were taken, one at the Front Yard, one at the Living Room, and one at the Front Right Bedroom. (Note: No air testing is performed at crawlspaces, attics, garages or any other area of the house that shares outdoor air). A tape sample was pulled from the far right A/C duct in the living room where visible mold was observed. 2

3 Air sample medium: Air-O-Cell spore trap cassettes. Although air sampling has its limitations, it can be a useful tool in determining the extent of airborne fungal spores and whether or not hidden reservoirs of fungal growth are resulting in high concentrations of spores in the indoor environment. To conduct such air sampling, air samples are collected from the outdoor air and the air from selected interior rooms at a rate of 15 Liters per minute for 10 minutes, or a Total Volume per sample of 150 Liters. Section 3 Findings: Surface mold was detected as follows: On several A/C ducts (Living Room, Kitchen, Hall Bath). Tape sample taken from A/C duct in living room. Elevated indoor mold spore counts detected as follows: Elevated means higher than the outdoor baseline air sample. Elevated mold spore counts were detected/found in the Living Room and Front Rt. Bedroom air samples, as compared to the outdoor air sample. Section 4 Discussion: Visible mold was observed and tested at the A/C duct in the living room (far right duct).this mold sample showed signs of Aureobasidlum, which is a moisture indicator mold type. Some of the A/C ducts have been replaced at the living room and the front right bedroom. 3

4 Consult a licensed HVAC technician for the proper replacement of the affected air ducts and to clean the evaporator coil if needed. Conclusions: Is remediation needed? YES Section 6 Remediation Procedure This report is solely for purposes of disclosing the presence and approximate degree of mold at the subject property. This report in not a remediation protocol. Inspection Connection neither provides mold remediation services, or is affiliated with or endorses any mold remediation professionals. Since remediation is needed, based on the results of this mold/iaq screening, Inspection Connection offers the following: Mold remediation is needed. Clean/remove any visible mold. Have the affected areas treated/cleaned with a mold preventative substance. There are several substances available/used to treat surface mold. Consult a qualified mold remediation contractor for recommendations on the type of substance that should be used in this case to prevent any future mold growth. Bleach/water mixture may work in this case (mold at garage door). The mold affected A/C ducts should be replaced and not just cleaned. We also recommend that the air throughout the home be filtered; using a HEPA-filter equipped air filtration devices (AFDs) to effectively scrub the air and to remove mold residual/particulate spores throughout the home/affected rooms for approx hours after abatement of the affected areas. AFDs are typically provided by or rented from mold/restoration contractors. It is recommended that the homeowner consult with a licensed/qualified mold remediation contractor to create a mold remediation protocol and to properly treat all affected areas. This contractor should properly scrub the air before any other air sampling is performed for a clearance. 4

5 The remediation proposal, activities, specifics, methodology and location (s) to be remediated should be clearly detailed in the written proposal. As part of their proposal, the remediation company should provide a written warranty for successful remediation verified in passing a third party inspection/air sample clearance testing.
