2 KDC West Introduction On 30 June 2012, it transpired that an underground fire had started approximately 2855m below surface at the Ya Rona (4) Shaft of KDC At that time, a number of persons were working underground at the shaft and emergency operations immediately commenced to recover such person and bring them to surface All such persons were subsequently recovered and brought to surface by Mines Rescue Services (MRS) proto teams. Unfortunately, five of these persons were fatally injured (also referred to as the deceased persons ) Mlungiselwa Madevana Colekele John Bankane Josiah Musa Driefontein Dlamini -Bambisanani Mziwakhe Seagent Mankunzi Bavuyise Mbola 2
3 KDC West Sequence of Events Ya Rona (4) Shaft Date Time Sequence of events 30 June h45 Central control fire detection heads alarming 30 June h00 Tramming crew proceeded underground to do mud loading on 39 Level on an overtime shift 30 June h36 Reports received that persons collapsed on 39 Level 30 June h43 F Vermaak and J Storm called out 1 July h35 Two proto teams called out 1 July h55 Instructions given regarding beacon heads and critical fans 1 July h08 Proto teams arrived in control room 1 July h38 Reports received from 38/31 - need assistance 1 July h00 Production Supervisors underground to assist 1 July h16 Proto team on 38 Level 1 July h47 Reports to various managerial personnel that beacon heads alarm and observations underground 1 July h12 Report from proto teams on rescue of 15 persons 1 July h20 Proto team on 39 Level established a fresh air base 1 team to proceed in to search for missing persons 1 July h28 Two additional proto teams and MRS personnel called out 1 July h00 Proto team reported that they located the 5 missing personnel and report on their status further the following readings was taken: CO 25000ppm, CO ppm 3
4 KDC West Sequence of Events Date Time Sequence of events 1 July h50 Additional Proto teams arrived and proceeded underground to assist with the recovery of the deceased persons. DMR notified telephonically of fatalities. 1 July h25 07h59 DMR Inspectors arrived and briefed 1 July h38 Bodies of deceased persons retrieved at the bottom of the material raise on 39 level 1 July h01 All bodies arrived on 38 level being transported to the station 1 July h00 Additional proto teams proceeded underground to locate the fire on 39 Level 1 July h05 Proto teams arrived on 38 level station being transported out 1 July h07 Bodies of deceased persons arrived on surface bank 1 July 2012 Ongoing (see update presentation) Proto teams in various aspects of searching for fire, building seals and taking readings 4
5 KDC West Refuge bays and personal protective equipment The deceased persons were found in close proximity to a refuge bay All of the deceased persons were found with self-contained self-rescuers (also referred to as SCSR s ). None of the deceased persons donned (engaged) their SCSR s. Four of the deceased persons had gas detection instruments (also referred to as GDI s ) with them at the time of the accident. The purpose of GDI s is to detect increased levels of, amongst others, Carbon Monoxide and Carbon Dioxide, and provide early warning in this regard 5
6 KDC West Remedial Action All operations at KDC immediately stopped by Management and Gold Fields SHSD Board notified KDC East operations resumed on Tuesday, 03 July 2012 after consultation with DMR KDC West operations resumed on Monday, 09 July 2012 after presentation to DMR Ya Rona (4) Shaft operations remained suspended, excluding the ongoing fire containment and extinguishing work Conducted risk assessments and declared area safe for essential services employees Multi-disciplinary task teams established to conduct audits and cross-audits in respect of the following categories:- Reporting systems Ventilation systems Accumulations of gasses Common central control room Re-entry conditions Personal Protective Equipment Emergency preparedness - underground 6
