(Levelton) was retained by the City of Vancouver (the City) to conduct a Hazardous Materials Management Survey (HMMS) for the Fire Hall #17, located at 7070 Knight Street, Vancouver, BC. The scope of work is detailed in the City of Vancouver Real Estate and Facilities Management Hazardous Materials Division Request for Cost Proposal, Hazardous Materials Assessment Project COV , dated November 29, The HMMS for this site was completed on December 18, 2013 by Kelly Konrad, Environmental Scientist of Levelton. 5 City of Vancouver File No.: R Hazardous Materials Management Survey January 24, 2014 Fire Hall # Knight Street, Vancouver, BC Page BACKGROUND AND PURPOSE This Hazardous Materials Management Survey is a part of the City of Vancouver s Hazardous Materials Management Program (HMMP) initiative to maintain compliance with WorkSafeBC requirements. The HMMP is intended to help the City of Vancouver efficiently manage hazardous materials at City properties, protect the health and safety of City staff, contractors and the general public when on district property. The findings from this survey should only be used for the purpose of building management and routine maintenance. The results of this survey are not intended to be used for construction, renovation, demolition or project tendering purposes. 4.0 SCOPE OF WORK AND LIMITATIONS This assessment was performed with the objective of compiling an inventory of hazardous building materials, including asbestos-containing materials (ACM), lead in paint and other lead-containing materials, PCB-containing light ballasts, thermostats or fluorescent light tubes containing mercury, construction materials which may contain silica, equipment containing ODSs, suspected mould/microbial growth, and sharps. Suspect asbestos-materials similar to homogenous materials sampled were visually extrapolated. A risk/condition assessment was prepared for each asbestos-containing material identified in the inventory with due regard for the condition of the material, its friability, accessibility and likelihood of damage, and the potential for fibre release and exposure of workers. The hazardous assessment was conducted in accordance with the City of Vancouver matrix and indicated on the accompanying excel spreadsheet. A management survey only identifies the hazardous building materials that could be encountered or disturbed by building workers or occupants during regular work activities. This is a non-destructive survey. As a result, hidden and below-ground materials are not addressed and any hidden materials which may contain asbestos, lead, PCBs or mercury should be assumed to be hazardous until sampling can demonstrate otherwise. Energised or operational electronic and hydraulic equipment and systems, building contents, roofing materials, materials in hidden areas such as within solid wall and ceiling cavities, areas beneath floor finishes, and materials within pipe chases were not assessed as part of this survey. These materials should not generally present a risk to workers if undisturbed, even if they are asbestoscontaining. Nevertheless, in case of planned renovation work or accidental disturbance we recommend that suspect materials be sampled and analyzed.

6 City of Vancouver File No.: R Hazardous Materials Management Survey January 24, 2014 Fire Hall # Knight Street, Vancouver, BC Page 3 Following the site assessments and receipt of laboratory analysis results, Levelton has prepared this report which includes the findings of this investigation and, where applicable, provision of recommendations for further work. 5.0 METHODOLOGY 5.1 ASBESTOS-CONTAINING MATERIALS Levelton representatives collected fifteen (15) representative bulk samples of suspect ACMs from exposed architectural finishes and structural systems at the site for confirmatory asbestos analysis. The bulk sample collection methodology was consistent with recognized industry standards and principles of good occupational hygiene practice. The number of samples collected was based on experienced professional judgment in consideration of, but not necessarily limited to, the era of building materials construction, uniformity of materials, types of materials reported to be modified during currently planned renovations, and size of area of homogeneous materials, etc. The locations and material descriptions of the collected samples were described on the Chain of Custody forms submitted along with the original samples. Copies of completed Chain of Custody forms are included in Appendix III of this report. The samples were subsequently transported to Asbestos Analytical Services Limited (AASL) in Vancouver, British Columbia for asbestos analyses. AASL participates in the American Industrial Hygiene Association s (AIHA) Bulk Asbestos Proficiency Analytical Testing Program. Samples were analysed in accordance with Standard National Institute for Occupational Safety and Health (NIOSH) Method The locations, material descriptions, and analytical results of bulk samples of suspect ACMs collected from this facility are presented the Laboratory Reports included in Appendix II of this report. Based on Levelton s professional opinion, the following materials were assumed not to contain asbestos during this survey and were classified as non-asbestos materials: Wood, wood panelling Carpet Concrete, Brick Fibreglass Plastics, rubbers, metals Gypsum board without taping compound/mud

7 City of Vancouver File No.: R Hazardous Materials Management Survey January 24, 2014 Fire Hall # Knight Street, Vancouver, BC Page LEAD-CONTAINING MATERIALS During the HMMS, visual review of the painted surfaces was conducted for paints and coatings. Sampling frequency was based on Levelton s Standard Operating Procedures. Levelton personnel collected twenty-six (26) representative bulk samples from the structure. The samples were submitted for lead analysis. The representative bulk sample collection was consistent with recognized industry standards and principles of good occupational hygiene practice. The number of samples collected was based on experienced professional judgment in consideration of, but not necessarily limited to, the era of construction, uniformity of materials and size of area of homogeneous materials. Information from the U.S. Occupational Safety and Health Administration (OSHA) suggests that the improper removal of lead paint containing 600 mg/kg (600ppm) lead results in airborne lead concentrations that exceed half of the exposure limit. This would trigger the requirement for an Exposure Control Plan and safe work procedures. Lead concentrations as low as 90 mg/kg (90ppm) may present a risk to pregnant women and children. Any risk assessment should include for the presence of high risk individuals within the workplace. Lead samples were sent to Maxxam Analytics, in Burnaby British Columbia for lead analyses. Samples were analysed by Inductively Coupled Plasma Atomic Emission Spectrometry (ICP- AES). Maxxam laboratories are accredited by the SCC and certified by the RTI and the MOE. A Chain-of-custody protocol was observed during handling and transportation of the bulk samples. Other lead-containing materials observed during the survey were recorded and are included in this report. 5.3 OTHER HAZARDOUS MATERIALS During the site work this facility was visually assessed for the presence of suspect PCB-containing equipment and electrical fixtures, mercury containing equipment and electrical fixtures, building materials containing silica, ozone-depleting substances, suspected visible mould and sharps.

Laboratory reports in Appendices II. 6.1 ASBESTOS-CONTAINING MATERIALS (ACMS) Based on the construction date of this facility, asbestos is anticipated to be present. Asbestos-containing cement pipe was not observed in this facility. A total of eleven sample results were provided to Levelton prior to the start of the survey by the City of Vancouver. Asbestos had been identified in three samples. Levelton cannot however comment on the accuracy of these sample results, but has included them and a visual observation of asbestos- labelled material at the request of the client. Levelton collected a total of fifteen suspected ACM bulk samples for analysis. Asbestos was found in two samples. The maroon with white streaks 9 x9 vinyl floor tile in Hallway (Location 201) and the black vinyl floor tiles in the Hallway (Location 201) contain 1-5% Chrysotile asbestos. Previous sampling results conducted by others indicate the black and red 9 x9 vinyl floor tile in the Radio Room (Location 107) and Second Floor Janitor Room (Loc. 211) contains 5-10% Chrysotile asbestos. The boiler insulation within the mechanical room (Loc. 003) was marked as asbestos-containing and at the request of the client not sampled therefore should be treated as an asbestos-containing material.

13 City of Vancouver File No.: R Hazardous Materials Management Survey January 24, 2014 Fire Hall # Knight Street, Vancouver, BC Page 10 Asbestos containing materials identified in this survey should be managed in accordance with the Hazard Assessment Form included in the accompanying Excel spreadsheet in Appendix V. Prior to any renovation or demolition activity a detailed pre-demolition asbestos survey should be completed as required by Section of the Occupational Health and Safety Regulations. 7.2 LEAD-CONTAINING MATERIALS Paint containing more than 90 mg/kg lead was found in this facility. Lead concentrations as low as 90 mg/kg (90ppm) may present a risk to pregnant women and children. An exposure control plan and safe work procedures must be put in place if any painted surfaces with a lead content greater than 600ppm are disturbed during renovation activity. Lead containing flashings and roof vents should be recycled when taken out of service. Disturbance of these items while in service should be avoided. 7.3 PCBS PCB-containing transformers and capacitors were not observed in this facility. Florescent light ballasts were observed PCBs may be present. All ballasts taken out of service should be checked for the presence of PCBs. Contact the Hazardous Materials Management Programme Administrator for further information regarding storage locations for PCB containing equipment at the District of Mission. All stored PCB containing equipment must be stored in labelled drums in a secured area. When the drums are full, they must be manifested and transported to an approved destruction and disposal facility in accordance with the Part 5 of the BC Occupational Health and Safety Regulation. 7.4 MERCURY In the event that the compact fluorescent lights (CFLs) and/or florescent light tubes are to be disposed of, the CFLs and light tubes should remain intact to mitigate work exposure to mercury fumes. Intact bulbs and ampules in thermostats can be recycled. The LightRecycle program in British Columbia includes all lamps, ballasts and fixtures from residential, commercial and industrial sectors. The Light Recycle website provides a list of recycling facilities on their website, at

During any refurbishment or demolition activity, follow safe work procedures when cutting or grinding concrete and other items containing crystalline silica. 7.6 ODS Should any ODS containing equipment be impacted during renovation activities and become waste, prior to disposal, the units should be degassed by a certified/licensed contractor as required by British Columbia's Ozone Depleting Substances Regulation. With proper maintenance and disposal practices, refrigerators, freezers, coolers, and HVAC units present a low risk to human health. 8.0 CLOSURE No hazardous materials survey can wholly eliminate uncertainty regarding the potential for recognized hazardous materials conditions at the site. Performance of a standardized hazardous material survey protocol is intended to reduce, but not eliminate uncertainty regarding the potential for recognized hazardous materials at the site, given reasonable limits of time and cost. This report has been prepared by Levelton Consultants Ltd. (Levelton) exclusively for The City of Vancouver, and is intended to provide a survey of the potential for the presence of hazardous materials on the site. No warranty, expressed or implied, is made. Any use which a third party makes of this report, or any reliance on or decisions to be made or actions based on it, are the responsibility of such third parties. Levelton accepts no responsibility for damages, if any, suffered by a third party as a result of decisions made or actions based on this report. Hidden Conditions: The Client acknowledges that subsurface and concealed conditions may vary from those encountered or reviewed. Levelton can only comment on the conditions observed on the date(s) the assessment is performed. The work is limited to those areas of concern identified by the Client and/or outlined in our proposal. Other areas of concern may exist but were not investigated within the scope of this assessment. b. Reliance on information: The evaluation and conclusions contained in the Report have been prepared on the basis of conditions in evidence at the time of site investigation and field review and on the basis of information provided to Levelton. Levelton has relied in good faith upon representations, information and instructions provided by the Client and others concerning the site. Accordingly, Levelton cannot accept responsibility for any deficiency, misstatement or inaccuracy contained in the report as a result of misstatements, omissions, misrepresentations or fraudulent acts of persons providing information. c. Additional Involvement by Levelton: To avoid misunderstandings, Levelton should be retained to assist other professionals to explain relevant hazardous materials and occupational health and safety findings and to review the hazardous materials and occupational health and safety aspects of the plans, drawings and specifications of other professionals relative to the services provided by Levelton. To ensure compliance and consistency with the applicable hazardous materials and occupational health and safety codes, legislation, regulations, guidelines and generally-accepted practices, Levelton should also be retained to provide field review services during the performance of any related work. Where applicable, it is understood that such field review services must meet or exceed the minimum necessary requirements to ascertain that the work being carried out is in general conformity with the recommendations made by Levelton. Any reduction from the level of services recommended by Levelton will result in Levelton providing qualified opinions regarding adequacy of the work. 6. ALTERNATE REPORT FORMAT When Levelton submits both electronic and hard copy versions of the Instruments of Professional Services, the Client agrees that only the signed and sealed hard copy versions shall be considered final and legally binding upon Levelton. The hard copy versions submitted by Levelton shall be the original documents for record and working purposes, and, in the event of a dispute or discrepancy, the hard copy versions shall govern over the electronic versions; furthermore, the Client agrees and waives all future right of dispute that the original hard copy signed and sealed versions of the Instruments of Professional Services maintained or retained, or both, by Levelton shall be deemed to be the overall originals for the Project. The Client agrees that the electronic file and hard copy versions of Instruments of Professional Services shall not, under any circumstances, no matter who owns or uses them, be altered by any party except Levelton. The Client warrants that the Instruments of Professional Services will be used only and exactly as submitted by Levelton. The Client recognizes and agrees that Levelton prepared and submitted electronic files using specific software or hardware systems, or both. Levelton makes no representation about the compatibility of these files with the current or future software and hardware systems of the Client, the Approved Users or any other party. The Client further agrees that Levelton is under no obligation, unless otherwise expressly specified, to provide the Client, the Approved Users and any other party, or any or all of them, with specific software and hardware systems that are compatible with any electronic submitted by Levelton. The Client further agrees that should the Client, an Approved User or a third party require Levelton to provide specific software or hardware systems, or both, compatible with the electronic files prepared and submitted by Levelton, for any reason whatsoever included but not restricted to an order from a court, then the Client will pay Levelton for all reasonable costs related to the provision of the specific software or hardware systems, or both.
