4 Introduction Hospitals have undergone a dramatic transformation in the past 20 years. More and more electrical medical equipment is now used in patient treatment areas of hospitals and medical practices. With the increase in the amount of equipment comes the increase risk of electrocution. As a result, electrocution protection equipment is now a common requirement in almost every area of the hospital and medical practice. The Electronic Safety Product range of electrocution protection equipment has been around since the introduction of electrical safety and the product range still continues today. Over the years the product range has proven its reliability and continues to meet the requirements of hospital electrical installations today.

6 The Proven Performer THE PROVEN PERFORMER Before becoming part of Clipsal Integrated Systems, the company Electronic Safety Products (ESP), was one of Australia s leading designers and manufacturers of equipment which provides a high level of protection against electrocution. Since the 1970s ESP was involved with the design, development and manufacture of Residual Current Devices (RCDs), Line Isolation and Overload Monitors (LIOMs), and Medical Service Panels suitable for use in patient treatment areas. The ESP product range and expertise continues within Clipsal Integrated Systems and now comprises of a range of electrocution protection equipment, various styles of Medical Service Panel Systems, and a large range of accessories specifically developed to meet the requirements of patient treatment areas of hospitals. 7 AS/NZS3003. ELECTRICAL INSTALLATIONS: PATIENT TREATMENT AREAS OF HOSPITALS AND MEDICAL AND DENTAL PRACTICES The ESP Series product range is designed to meet the requirements of AS/NZS3003. AS/NZS3003 sets out the specialist electrical installation requirements often found in the following premises. Hospitals Medical Practices Dental Practices Physiotherapy Practices The areas within these premises that would need to meet AS/NZS3003 are classified as Body or Cardiac Protected Areas. The procedures carried out to classify these areas as Body or Cardiac Protected are as follows: Within Body Protected Areas procedures are carried out where a patient is connected to an item of electro-medical equipment which reduces the impedance of the skin by either electrode paste or by the entrance of conducting fluids, metal needles, saline filled catheters etc. ELECTRICAL I N S T A L L A T I O N S Within Cardiac Protected Areas the same procedures are carried out as in the Body Protected Areas but also procedures where a patient has an electrical conductor placed within the heart or is likely to come into contact with the heart eg. electrical wires such as cardiac pacing electrodes and intracardiac ECG electrodes, or insulated tubes filled with conducting fluids. 8 Medical Service Panel Systems SURFACE MOUNT MEDICAL SERVICE PANEL SYSTEMS Medical Service Panels (MSPs) provide access to power, data, nurse call, hospital gas and environmental controls in patient areas of hospitals and medical practices. The Surface Mount MSP system is typically used in areas of a hospital where an MSP incorporating a limited number of accessories is required to be mounted to a wall that is not easily suited to the installation of a recessed wall box. An example of this would be in a refurbishment of an existing area within a hospital where the walls are of brick construction. The Flush Mount MSP system is typically installed in critical areas of the hospital where a strong and reliable product is required. These areas are operating theatres and other high dependency areas. The Surface Mounted Duct MSP system is primarily used in areas of a hospital where long lengths of medical service panels are required. The advantages of a surface mount system over a flush mount system is that there is less internal wall preparation required because the wall does not need to be modified to accommodate recessed wall boxes or modified to accommodate long runs of wiring and gas piping. It is used to provide audible and visual indication of when the RCD it is monitoring, trips. 18 Line Isolation & Overload Monitor (LIOM) Operation MEDICAL SERVICE PANEL ELECTRICAL ACCESSORIES An isolated supply is a continuous supply. The isolation transformer isolates the power from earth therefore a fault on both lines of secondary transformer winding to earth needs to occur before a fault current flows. A fault on only one line and earth will not cause a fault current to flow. A Line Isolation and Overload Monitor (LIOM) enables the integrity of an isolated power supply system to be continuously monitored. The LIOM is able to detect when one fault occurs therefore is able to warn of a potential hazardous situation before any fault current actually flows. The first fault is know as Prospective Hazard Current (PHC). The LIOM displays the PHC on a LCD bar graph display and when 5mA PHC is reached a mutable alarm sounds along with a flashing LED indicator labelled PHC Warning. If a PHC alarm condition arises, the medical staff can, in the light of the seriousness of the patient s condition, choose to continue with the procedure. In this situation, the surgeon is relying on a second fault not occurring that would turn the PHC into a real flow of current. During this time, measures can be taken to rectify the fault condition by, for example, disconnecting all nonessential equipment. In addition to the LIOM monitoring isolation it also warns of an impending overload, thus averting problems which could result from a loss of supply. When the overload level is reached a mutable alarm sounds along with an Overload Warning flashing LED indicator. LINE ISOLATION & OVERLOAD MONITOR Schematic Diagram 16

19 Installation When installing an isolated supply there are several things to take into consideration. Long runs of cable between the transformer and the LIOM and/or long runs between the LIOM and the outlets should be avoided. Due to capacitive coupling between cables and between cables and conduit, runs longer than 50m can result in high standing PHC (ie;>2ma). Parallel runs of cables from two or more isolated supply systems should also be avoided, The cross talk between parallel cables may cause rhythmic fluctuations on the unit's displays. Where parallel wiring cannot be avoided, wiring should include an earth (eg; TPS). The earth conductor should be earthed only at one end of its run. Whilst running cable in this manner will lessen cross talk problems, it may increase standing PHC. Because the E05 LIOM monitors the load current on line the LIOM must be the first item connected to the secondary winding of the isolation transformer. All items to be powered from the isolated supply must connect back to the terminals of the LIOM labelled Active 1 Out and Active 2 Out. Failure to do this will result in incorrect measurements of the load current. Due to the high inrush current when power is first applied to the isolation transformer a suitable circuit breaker needs to be chosen. 20 Isolation Transformer E14 SERIES The E14 Series Isolation Transformer in conjuction with the E05 Series Line Isolation and Overload Monitor form the basis of an isolated supply system which provides electrocution and overload protection as detailed in AS/NZS3003 Electrical Installations - Patient treatment areas of hospitals and medical and dental practices. This product is available in three different versions. The first being the respected E14 version, which for over a decade has been installed into hospitals throughout Australia and the Asia Pacific Region. The remaining two E14/2 and E14/3 versions are an economical alternative to the E14, while still meeting the requirements of the respective standards. Features Steel enclosure. The E14 & E14/2 versions are fully encapsulated in heat cured epoxy resin to reduce noise and prevent ingress of moisture and dust. CAT NO. 22 Equipotential Junction E34 AND E35 SERIES MEDICAL SERVICE PANEL ELECTRICAL ACCESSORIES The E34 and E35 Series Equipotential Junctions (EPJ) form part of the Equipotential Earthing System which provides micro electrocution protection in cardiac protected areas as detailed in AS/NZS3003 Electrical Installations - Patient treatment areas of hospitals and medical and dental practices. The EP Earthing system requires all earthed points within the cardiac protected area to be connected back to a common EP Junction via low resistance earth wiring (large cross section). The low resistance system reduces the potential difference between two earthed points in the area therefore reducing the risk of micro electrocution from the stray currents flowing in the earthing system. The E34 and E35 Series EPJ is available in the four different styles of ESP Series Medical Service Panels (MSP). 40 Why is there a need for special electrical installations in patient treatment areas? TECHNICAL INFORMATION WHY IS THERE A NEED? Modern medical procedures utilise and increasingly rely on, electrical and electronic equipment. This equipment ranges from lighting to patient monitors and electrosurgical equipment. The human body s main defence against the effects of electrical current passing through it are two-fold. Firstly, the resistance of the skin, which limits the amount of current that can pass. Secondly, the muscular contractions that accompany electric shock usually tend to disconnect the subject from the source of electrical energy. In medical procedures, both of these factors may be radically altered. Firstly, electrodes may enter the body or be attached to the skin, thus bypassing the skins resistance. Fluid may be taken from the body, treated and then returned (eg: kidney dialysis), providing a low resistance path for electrical current into the body. Secondly, the patient may be under anaesthetic. The source of the current (ie: an electrode) may be fastened to the patient and so not easily disconnected. Furthermore, where electrical conductors come in close contact with the heart (eg; in open heart surgery or pacemaker implantation), the imperceptible current that can flow between normally safe earthed conductors may result in micro shock, and possible death. There are three main results from current passing through the human body. Refer to graph on the following page. These are firstly, burning, particularly where the current enters and leaves the body. Secondly, muscular paralysis, which may temporarily halt breathing. Thirdly, electric shock may cause cardiac arrest; this ventricular fibrillation results in the pump chambers and valves of the heart going out of synchronisation, and blood ceases to be pumped around the body. The first two effects, whilst uncomfortable, seldom cause permanent damage. Ventricular fibrillation on the other hand, requires immediate reversal with the defibrillator before brain damage or death occurs. Electrical hazards are not only present for patients, but also for medical staff. The use of a multiplicity of equipment which individually presents no hazard may give rise to current paths within the patient circuit that cause a problem. Equipment that is designed to deliver current to patients (ie; diathermy electro-surgical units or defribillators) also have the potential, if damaged, mis-wired or incorrectly used, to produce an electrocution hazard to the patient of equipment operator. 38

41 TECHNICAL INFORMATION PHYSIOLOGICAL EFFECTS Zone 1 Usually no reaction effects. Zone 2 Usually no harmful physiological effects. Zone 3 Usually no organic damage to be expected. Likelihood of muscular contractions and difficulty in breathing. Reversible disturbances of formation and conduction of impulses in the heart, including atrial fibrillation and transient cardiac increasing with current magnitude and time. Zone 4 In addition to the effects to Zone 3, probability of ventricular fibrillation increasing up to about 5% (Curve C2), up to about 50% (Curve C3) and above 50% beyond Curve C3. Increasing with magnitude and time, pathophysiological effects such as cardiac arrest, breathing arrest and heavy burns may occur. WHY IS THERE A NEED? 39

42 Area Classification & Requirements TECHNICAL INFORMATION Medical treatment areas can be divided into the following three categories, as detailed in AS/NZS3003 Electrical Installations - Patient treatment areas of hospitals and medical and dental practices. UNPROTECTED AREA If procedures do not require the physical attachment of electro-medical equipment to the patient, electrical safety in such areas typically involves nothing more than normal domestic or industrial wiring practices. The area in this case is known as an Unprotected Area. BODY PROTECTED AREA Where electro-medical equipment is attached to external parts of the body so that contact resistance is reduced and ready disconnection is also not possible, Body Area Protection is required. Examples of areas and procedures that may require this type of protection are ECG, EMG, EEG, and Physiotherapy rooms, plus any room used for dialysis. AREA CLASSIFICATION & REQUIREMENTS A Body Protected Area requires installation of Residual Current Devices (RCDs) or Line Isolation and Overload Monitors (LIOMs) plus meeting the additional requirements detailed in AS/NZS3003. CARDIAC PROTECTED AREA Where electro-medical equipment is in close contact with the heart, Cardiac Area Protection is required. Examples of areas and procedures that may require this type of protection include CCU, ICU, and operating theatres where cardiac procedures are performed. A Cardiac Protected Area requires firstly RCD and/or LIOM protection as per Body Protected Areas, and secondly an Equipotential Earthing System plus meeting the additional requirements detailed in AS/NZS

43 RCD Functional Description An RCD provides macro electrocution protection by removing power when a fault current is detected. The RCD uses the magnetic field created around a current carrying conductor to detect the imbalance of current caused when a sufficiently low impedance path is provided to earth. This path may constitute a faulty piece of equipment, the patient, a member of the nursing staff, or a combination of these. The active and neutral conductors are passed through a toroid coil which is a coil containing many turns of wire around a doughnut shaped core. A changing magnetic field will cause a voltage to be induced into the coil. This voltage is detected and used to switch off the supply by opening the breaker contacts. During normal operation the currents flowing in the active and neutral conductors are equal and opposite, hence the voltage in the coil is zero. However, should current pass through a resistance to earth, the resulting imbalance of current flowing through the active and neutral conductors (the residual current) will, if it is of sufficient magnitude, cause the supply to be interrupted. TECHNICAL INFORMATION Any RCD in a Cardiac or Body Protected Area should trip off with a residual current of between five and ten milliamps (ma) and should interrupt the supply within forty milliseconds (ms) of the fault occurring. 44 Line Isolation & Overload Monitor Functional Description TECHNICAL INFORMATION The isolated supply comprises of a Line Isolation and Overload Monitor (LIOM) and isolation transformer, and is used to protect against macro electrocution. It is a continuous supply which differs from an RCD protected supply, which removes power when a fault is detected. An isolation transformer isolates the power from earth therefore a fault on both lines of the secondary transformer winding to earth needs to occur before a fault current flows. A fault on only one line and earth will not cause a fault current to flow. This current that does not flow is known as Prospective Hazard Current (PHC). A LIOM enables the integrity of an isolated power supply system to be continuously monitored. The LIOM is able to detect when one fault occurs therefore is able to warn of a potentially hazardous situation before any fault current actually flows. In order for the LIOM to monitor the isolation of any system with respect to earth, a small test signal must be injected between those same two points. If the test signal is to be applied continuously, it should be in a form which does not effect the integrity of the isolated system. For this reason, voltage sources cannot be used as they would solidly bind one part of the isolated system to earth. Current sources, on the other hand, exhibit a high impedance characteristic and if kept to a low amplitude, will not significantly effect the isolation of a system. The measurement of an impedance with a current source is relatively simple; inject a known current and measure the voltage drop across the test impedance. If, however, there is already a current flowing in the test impedance, the test current must be encoded in some manner so that the corresponding voltage drop may be determined. When measuring the impedance from one line to earth in this manner, the value obtained is equal to the parallel combination of all impedances between the isolated system and earth. The current that would flow in the event of a fault can be calculated by taking into account the voltages between each line and earth. LIOM FUNCTIONAL DESCRIPTION Schematic Diagram 42

45 Equipotential Earthing System Functional Description The Equipotential Earthing System is used to protect against micro electrocution. The EP Earthing system requires all earthed points within the cardiac protected area to be connected back to a common EP Junction via low resistance earth wiring (large cross section). The low resistance system reduces the potential difference between two earthed points in the area therefore reducing the risk of micro electrocution caused by the stray currents flowing the earthing system. 46 Choosing between an RCD or LIOM TECHNICAL INFORMATION All socket outlets and directly wired equipment (eg; X-Ray machines) in Body and Cardiac Protected Areas must be protected by RCDs or an Isolated Supply and LIOM. The selection of whether to use an RCD or an Isolated Supply and LIOM depends on the type of procedures to be undertaken, taking into account current and future requirements. The main difference between the two systems of protection is in continuation of supply. If the interruption of the supply after the detection of a Line-to-Earth fault makes the RCD system unacceptable for medical reasons, the Isolated Supply and LIOM should be chosen. MAJOR POINTS OF COMPARISON If an evaluation determines that an Isolated Supply and LIOM are required for a particular procedure undertaken within one theatre, this requirement does not necessarily extend to all theatres in the suite or to all outlets in that particular theatre. Any one area may have some sockets protected by RCDs and some protected by Isolated Supplies and LIOMs. However, neither RCDs nor LIOMs in one Cardiac or Body Protected Area may supply sockets or equipment in another Cardiac or Body Protected Area, or in an unprotected area. No unprotected socket outlets are allowed within a Body or Cardiac Protected Area.
