1 Office of Environmental Health, Safety & Risk Management May 15, 2008 Texas Department of State Health Services Radiation Safety Licensing Branch 1100 West 49 th Street Austin, Texas RE: RML No. L03321 Log No Dear Mr. Kelly: The following are our responses to the additional information that you requested regarding the amendment to our radioactive material license. Item 1: As a graduate student Dr. Lane used approximately 100 µci of 32P-dCTP, 14Cbutyrate, or 14C-glucose at one time. Her graduate advisor was Dr. Barry Jesse, Department of Animal Science, Rutgers University. Average activities are listed here: As a postdoc Dr. Lane used 100 µci of 32P-dCTP or 3H-retinol per reaction. Her postdoctoral supervisor was Dr. Lorraine Gudas, Department of Pharmacology, Weill Medical College of Cornell University. As a faculty member at University of Texas Dr. Lane used 100 µci of 32P-dCTP, or 25 µci of 32P-dATP, 100 µci of 33PdCTP, or 100 µci of 3H-retinol or 3H-thymidine or 14C-chloramphenicol, or 3H phosphatidyl inositol at one time. Dr. Lane also used 15 µci of I125-IGF-1 in one experiment. The maximum amount of radioactivity that Dr Lane has handled in one experiment is 1 mci of 32P-dCTP as a postdoc. Average amounts and maximum amounts per experiment are the same for all other isotopes. Maximum amounts stored in her UT lab are indicated on Dr. Lane s UT Authorization to use Radioactive Materials form. She does not have storage information for her postdoctoral and graduate labs where I was not the license holder. As you can see from Dr. Lane s UT Authorization to use Radioactive Materials form, she was also approved to use S35. She did not use this isotope at UT and does not anticipate using it in the future at Texas State. Also, she will not be using I125 at Texas State. See Attachment 1 and Attachment 2 for documentation of Dr. Lane s training and authorization for radioactive material use at University of Texas. Item 2: See Attachment 3 for diagram of laboratories where radioactive material will be stored when not in use. Item 3: Countertop surfaces will be covered by plastic-backed absorbent paper to control the spread of contamination. Texas State University San Marcos 601 University Drive San Marcos, Texas Telephone: Fax: Texas State University-San Marcos is a Member of the Texas State University System

2 Office of Environmental Health, Safety & Risk Management Item 4: In addition to standard laboratory safety equipment which includes safety glasses, laboratory coats and gloves other safety equipment available includes remote handling equipment such as: pipettemen, aerosol tips to be used on the pipettemen to prevent contamination of the pipettement, and tweezers. Several Plexiglas containers for shielding the waste during storage as well as a large Plexiglas shield to protect the user from 32P. Also have small Plexiglas containers that hold individual eppendorf tubes to further shield the user. Spills will be cleaned with RadiacWash until all contamination is removed. A hand held Geiger counter is also available. Item 5: The door to room 151 is being permanently sealed so it will not open. The door to room 153 locks and will be kept locked to secure the isotopes and prevent access from unauthorized personnel. Item 6: All laboratory fume hoods at Texas State are checked by the Environmental Health, Safety and Risk Management Office on a monthly basis. Any none functioning fume hoods are reported to the Principal Investigator and a Facilities Request is submitted to correct the problem. During the past year the fume hood located in the Family & Consumer Sciences building Room 151 was tested twelve times. All tests were conducted at a sash opening of 12 inches, the maximum reading was 300 fpm and minimum reading was 150 fpm, the average reading was 240 fpm. Most readings were between 200 and 250 fpm. A filtration device will not be used, none of the compounds requested to be used are volatile. Item 7: The use of radioactive gases is not requested nor is it anticipated. If you have any further questions please do not hesitate to contact me. Thank You, Wm. A. Hollingsworth, RSO Environmental Health & Safety Specialist Texas State University San Marcos 601 University Drive San Marcos, Texas Telephone: Fax: Texas State University-San Marcos is a Member of the Texas State University System
