If No, provide Details Type and Location of Panel Is a Remote Annunciator Present? If Yes, note location(s) Is the power light on? Are any trouble or supervisory lights illuminated on panel? If Yes, note device, location and reason Are the batteries tested and in good working condition? Is there an intercom or P/A system as part of the fire alarm system? Are there pull stations located at all exits? Are there heat detectors present in the building? Are there smoke detectors present in the building? Are there any battery operated detectors present in the building? If Yes, note the last date the batteries were replaced Are there horns, strobes or bells in the building? Are there Carbon Monoxide detectors present? Are there Carbon Dioxide detectors present?
2 FIRE ALARM TESTING Is the fire alarm system tested annually as per CAN ULC S524? Is the fire alarm system tested monthly as per CAN ULC S524? Check Status of Primary and Remote Trouble Signals (lights) on Panel Status of Primary Power Signal (light) on Panel Test one Field Device or Pull Station (Rotational Basis) One Alarm Signal Tested (Rotational Basis) Annuciator Tested for Activation of Devices Operation of Audible abd Visual Devices Batteries are clean and Terminal Clamps are secure? Provide the last date of inspection and by whom: Is the inspection certificate present? AUTOMATIC SPRINKLER SYSTEMS Is there a monitored sprinkler system present? If No, describe What percentage of the building has sprinkler protection % What type of sprinkler system in present: WET DRY PREACTION OTHER Is there adequate heat provided in the building? Is there adequate heat provided in the sprinkler valve room? Are all sprinkler valves and devices accessible > 3' cleareance? Are all sprinkler control valves in the open position and supervised?
3 Is the sprinkler system pressurized Note pressure on the gauges: Supply Gauge System Gauge Are Main Drain Tests completed and inspection tags present? Are there water flow switches present and tested? Are there pressure switches present? Is there a spare head cabinet, extra sprinkler heads and a wrench present? Is there a Fire Department connection present? Is the Fire Department connection accessible and visible? Does the Fire Department connection have its caps in place? Is the water motor gong functioning and clear of exteral obstructions? Are there any sprinkler heads in excess of 50 years old? If Yes, has testing of the sprinkler heads been completed? Are there any obstructed sprinkler heads installed? Are there any sprinkler heads less then 18" above storage? Are there sprinkler heads installed under all obstructions greater then 4'? Is there any evidence of sprinkler piping leaks? If Yes, note location Does the piping show signs of corrosion? If Yes, note location Are there any painted sprinkler heads present? If Yes, note location

WATER SUPPLY Is the fire protection water supply provided by the Municipality? If No, provide details Provide the size of incoming water supply line Is there a fire hydrant located within 150' of the fire department connection? Are there any private fire hydrants located on the property Are the private hydrants tested annually? Provide the date of the last inspection: FIRE EXTINGUISHERS Confirm the type of extinguishers: ABC CO2 CLASS K WATER Are all fire extinguishers accessible and properly mounted?
5 Are fire extinguishers present near all exits and hazardous locations? Are up to date inspection tags present? Are the fire extinguishers checked monthly and tags signed? System Pressure Verified at highest cabinet / rack Gauge Pressure Condition of Hoses & Length 75' 100' Are all hoses properly mounted?

KITCHEN SUPPRESSION SYSTEMS Type of Systems: WC DC Other: Inspected semi-annually and links changed? Is the manual release button accessible? Is the cooking hood clean and free of grease build up? Is the grill and fryer clean and free of grease build up? Is a Class K extinguisher present? Is the ductwork clean and inspected?
