1 Goal Get the right resource with the right staffing delivered to the right call at the right time. TVF&R consistently analyzes our response data which includes many factors: number of calls, types of calls, response time, location, etc. A consistent message our data has been telling us is the occurrence of medical and public service calls is increasing, and the occurrence of fires is decreasing. We have heard this message and are now making deployment changes to ensure we are meeting the needs of the District and our citizens from both a response and a cost efficiency standpoint. The following table illustrates the message. Dispatch Type refers to the type of call our crews were dispatch to; Situation Found refers to what the crews found after they arrived. The Other row contains many of our public service calls and false alarms. Does it really take four firefighters and a fire truck to help my sick neighbor? In light of recent economic changes, and the public s interest in where their tax dollars go, these are just two simple examples of the questions we are increasingly receiving. This is the core of the right resource with the right staffing component of the goal. There still is, and always will be, a need for fire resources. When the fire does occur, we need to have the units and personnel ready to respond and solve the problem. In 2010, all but three stations will be staffed with four person fire apparatus and a third Truck is being added to the system (illustrated further on page 3). But as we continue to grow as an agency both in size and call volume, we need to continually evolve and ensure we re addressing the problems at hand (increasing medical and public service calls, coupled with a struggling economy). Therefore, in 2010, we will also be adding a third Medic unit and four Cars to our deployment model. The Medics will be staffed with two EMT Paramedics, one of which will be a Company Officer, and the Cars will be staffed with one EMT Paramedic. Outside of having one Company Officer on each Medic, there are no rank requirements for the other positions. To assist us in determining the best locations of these units, we took response data from , extracted the incidents that we ve determined to be Medic and Car Dispatch Types (further explained below), and geo coded them into a map of the District. The result is a map that allowed us to visualize the call density of these specific incidents.

4 Right Call TVF&R staff recently reviewed the Dispatch Types in order to address the delivered to the right call component of the goal. The majority of the proposed changes occur within the medical types. Utilizing data from actual calls from , we selected incidents that were dispatched as Abdominal Pain, Chest Pain, Unknown Injury MVA, and Traumatic Injuries. We then took those incidents and matched them with the report that was written by the crews to determine if these incidents resulted in a Patient Care Report (PCR) and required medical intervention. The following graphs, which are broken down by time of day, illustrate the findings. The purple line represents the Dispatch Type; the green line represents the number of actual patients that were found after we responded (a PCR was written). Page 4 of 16

8 Interestingly, of the few patients that were found during a Trauma response (as noted above), even fewer were actually considered Trauma System Entries (TSE) and entered into the Trauma System. Page 8 of 16

9 After reviewing this type of before and after data, consulting with the EMS Department and Physician Supervisor, and evaluating the number of personnel needed to perform critical tasks per incident type, we are proposing that the Medics and Cars be inserted into the Dispatch Types in the following manner. 12 Given the Dispatch Types noted above, the general equipment list for these units includes: EMS Kits Defibrillator Drug Box Safety Vest Flashlight Flares Cones ( Collapsible) Extinguisher Shovel Broom Absorbent Smoke Detector Kit Scene Tape Lock Out Kit Gas Monitor Air Pack Turnout Gear Wildland Bag Right Time Not every call requires a Code 3 response; this is the basis behind the at the right time, component of the goal. Currently, there are Dispatch Types that are preprogrammed as Code 3 that aren t necessary (e.g., MVU). Also, because of the 30 second call processing requirements applied to dispatch, oftentimes calls may be initiated as a Code 3 response in order for the Call Taker to get the incident dispatched so they can continue to speak with the caller to further evaluate the response and add, subtract, or slow units as needed. These are system issues that are in need of revision. TVF&R staff is working with the Fire Defense Boards and then through FUG/TAC to change the call processing requirements applied to dispatch. We are proposing a 90 second call processing time to allow the Call Taker enough time to fully vet a call in order to assign it the appropriate Dispatch Type from the beginning. Zones In conjunction with revising call processing requirements and Response Modes, we are introducing a new layer (Zones) to the Medic and Car deployment, separating the District into three areas. These three Zones are specific to the Medic and Car deployment only (do not have an effect on other units) and are only applied to Code 1 Dispatch Types. The premise behind the Zone response is to allow any Medic or Car in their assigned Zone to be dispatched to the call first. If a Medic or Car is not available then the closest unit would be dispatched. Example: A call comes into dispatch and is triaged as a Fall. The caller s address is located near Station 68. Medic 61 is already on another call, but Car 62 and Car 67 are available. Based upon location, Car 67 is closer and therefore is dispatched to the Fall Code 1. If the same call occurred, but the Medic and both Cars in that Zone were unavailable, the call would fall to Engine 68, the next closest unit. Our intent is to have this information programmed into CAD. The Zone response model will be based upon CAD s ability to automate this process; it is not our intent to make this a manual process. The days and hours of the week were determined again, based on actual call volume information from the timeframe, with consideration given to shift change management (shift beginning at 0700 hours as opposed to 0900 hours), and the impact on the personnel working on the units (consecutive 10 hour days as opposed to breaking them up). Given this information, the Medics and Cars will be deployed Tuesday through Friday, from 0700 hours to 1700 hours.
