Periodic check that there are no obvious dangers in the grounds. Checked November 2015 by P Bradley. b. Advise parents of risk Children and Youth Workers to advise parents of site risks. C4 Children hurting themselves in the kitchen (burning themselves on the urn etc) Low a. Equipment and appliances are similar to any ordinary kitchen. b. Urn and kettle to be placed out of reach of children on secure worktop. No further action required. C5 Children hurting themselves playing in area of the organ and musical instruments. Low Low Parents are advised of their responsibilities towards children and adults are in the vicinity whenever children are present. No further action required.

2 C6 Children hurting themselves inside the bus. Low The stair case is not excessively steep and the steps are wide. No sharp edges identified inside bus. No trip hazards identified. Children could hurt themselves by falling out of the top deck fire escape window. Children could also hurt themselves by falling over the railings on the top deck surrounding the top of the stairs. Children not allowed to open the emergency exit on the top deck, nor to stand on the seat at the front of the bus, top deck, driver s side. Youth workers to be reminded of these additional risks. Small children using lower deck could attempt to climb stairs Safety gated fitted on lower deck to prevent children from going to unsupervised areas. C7 Children using bus could run away from supervised area Youngest children no longer use the bus on a regular basis. C8 Children hurting/ burning themselves on the Kettle or Toaster in the bus Only used in the evenings for the older youth group Kettle and toaster to be removed when small children are on the bus. Children 12 and over only to use the Kettle or toaster under adult supervision, Fire

Ensure that fire doors are unlocked at each service or event (south side door has a breakable tube with attached hammer). Additional fire wardens needed at side door exits for events of greater than 120 attendees. b. Confirm that routes to fire exits are kept clear at each service or event. Regular check F5 Blocked road access for emergency vehicles / Limited by width of security gate. Potentially limited by cars parking near chapel door Ensure that cars are not parked outside door during services or events. Regular check F6 Fire on bus spreads to chapel Low / Park bus suitably far away from chapel so that risk of a fire spreading would be only very slight. Bus is parked well away from the chapel. F7 Fire on the bus Low / Assurance is given that all fixtures and fittings comply with fire safety regulations by virtue of the bus having been used commercially and no new fixtures or fittings having been added. a. Escape ladder is fastened to end of bus under the top deck fire escape window to enable people to use this fire escape if needed. Bus workers to be briefed on how to set up and use the top deck escape ladder. Instruction sheet explaining how to set up and use the fire escape ladder to be fastened adjacent to the top deck fire escape window.

5 b. Fire extinguisher installed and maintained.. Building B1 Trip hazards, electrical wires. Low Low Electrical wires in rubber cable guard or other suitable cover where they are a trip hazard. checks. B2 Trip hazards, toys Low Low Main concern is with crèche area at front of right aisle. Creche area has been cordoned off with gates. B3 Trip hazard, uneven floor and nails in floor. Low Low Loose floor tiles set as flush as possible. Nails and screws removed from floor. checks. B4 Trip hazard from carpets and rugs. Low Low Carpets fixed down. checks B5 Collapse of lighting bars Low Ensure chains are strong enough. Chains rated at over 80kg each / 240kg total. Total weight of bar and lights is 50kg. B6 People hurt by falling masonry Low Needs periodic check Check all walls and knock off any plaster that looks insecure. checks B7 People hurt by tables toppling Low Low a. trolley has restraining strap.

6 B8 out of trolley People hurt by ladder falling Low Ladder will be removed from church when it is no longer needed. b. trolley kept out of main body of church. Ladder is stored in store room out of easy reach. B9 Electrocution Low Annual safety test of electrical equipment (see test certificates). B10 LPG blow heaters. Positioning, operation or malfunction of heaters leads to injury or affects means of escape. LPG Blowers to be used for pre heating only. Units to be switched off when choir/ congregation starts to assemble. B11 Electrical heaters. Unsupervised use of electrical heaters leads to: Falling hazard, Overloading of circuits or electrical problems. 1. Only members with knowledge of the electrical distribution system to move heaters to a different plug. 2. Space heaters are permanently fitted and cannot be moved. ON/off is by plug or circuit activation.

7 B12 Choir Conductor s rostrum. Conductor standing on makeshift rostrum to gain height leads to a falling hazard. A properly constructed conductor s platform is provided for both rehearsal and performance locations B13 Vehicle and pedestrian entry and egress. The simultaneous use of roadways by both vehicles and pedestrians leads to an accident or forces pedestrians into hazards Access road to chapel is narrow and short therefore speeds are kept low and is clearly a space to be shared by all. B14 Parking. Indiscriminate parking leads to restricted access for emergency vehicles, conflicting manouvres being conducted, and vehicles presenting a hazard for pedestrians. Parking in front of main doors is discouraged. Parking on access road is kept to one side only. checks.

8 B15 Poor ground conditions. Waterlogged or muddy ground leads to a slipping or falling hazard Warnings and reminders given during periods of extended wet or cold weather. Repairs to undertaken to the worst affected areas. checks B16 External Lighting. Limited external lighting leads to a personal safety or falling hazard. External lighting in place. Choir reminded to carry personal torches. Health, Hygiene and Safe Working H1 People suffer cuts and bruises Low Existing First Aid box is a 10-person size. First Aid box is available in kitchen and on bus. Accident Book is available in kitchen. Stock in Box regularly checked. H2 Someone requires medical attention Low It would be good to have 2-3 trained first-aiders in congregation. Realistically they will not always be present. Several church members are qualified first aiders, others have experience in relevant medical specialisms. (eg Sarah Flashman is a trained Nurse, Annette Lawrance-Owen and Annie Bradley have extensive first aid experience).

9 H3 Poor handling of food Low Low There will be no food preparation in Chapel beyond making drinks and opening biscuits, cakes etc. Advise those on coffee rota about appropriate food handling measures. Notices about cleaning and food storage are displayed in the kitchen. Need a Leader/church member to own the kitchen and prompt users to follow the guidance. H4 Kitchen worktops not kept clean, risk of rodents or unhygienic. Low As above. As above H5 People having accidents or trouble with visitors when alone in the Chapel Nobody should be alone in the building for periods of more than 15 minutes. They should ideally have a mobile phone with them and know the phone number of the security guard. Advise church of this, particularly those who help with cleaning or maintaining the building and its equipment. Post phone number of security guard in kitchen. Check that security number is still posted in kitchen H6 Hot water. Use of water boilers or carrying of kettles leads to a personal injury hazard for the user or others. Kettles or urn not to be carried whilst containing hot water. Thermos jugs available to carry hot water from the kitchen to the serving table.
