3 Soil Sample Submission Sheet Helpful Hints Billing/Group ID: if you are a company or Extension office. Ensure name on sheet and name on boxes are identical. If paid online, write in online order #. Please print legible, results will be delayed if not legible. Sample ID: the name you wish to call the sample(s). Ensure names on sheet and on box match. Crop s: The codes to ID the plants or crops to be grown. Can have 4 per sample. Common crop codes here, more on crop code sheet. Organic Matter: Useful for landscape jobs where soils must meet certain specifications. Fertilizer recommendation do not change with OM values. Soluble Salts: Useful if salt was heavily applied to driveways or sidewalks during winter icing. Texture: Useful for landscape jobs where soils must meet certain specifications. One can estimate texture by the feel method. Lab # : SPPC assigns, leave blank

Take five to ten sub-samples to comprise one composite sample from the management zone. Samples should be taken from the 0 to 6 inch depth. Place sub-samples into a bucket and mix well. After sample is well mixed, fill the soil box. If Texture is wanted, please fill two soil boxes. Label the box with your name and sample ID (example lawn, garden, 1, A, etc ) Example management zones Front yard Back yard Garden Figure 1.
