5 Purpose As required by Title 19, California Code of Regulations, published emergency procedures information shall be available to all persons entering the building. This emergency plan shall be implemented, maintained and annually reviewed for 100 Mathilda Place. This plan is a legal document. Implementation In the event a fire is detected in the building or a fire alarm activates, or as any other emergency warrants, this emergency plan shall be implemented. 6 Procedure for Reporting an Emergency In the event of an emergency, contact the local authorities by dialing 911 immediately. Next, notify 24-Hour Security for the building. Emergency phone numbers can be found on page 1 of this. Procedure for Notifying Occupants of an Emergency In the event of a fire, fire alarms can be activated manually at a pull station on the first floor in the lobby or will be activated by the smoke detectors, which detect heat and smoke. The alarm sound is a loud pulsating buzzer with flashing lights. The alarm can be heard throughout the building. Upon hearing the alarm, or if occupants are otherwise notified to evacuate the building, immediately go to the nearest stairwell exit, proceed to the street level, exit the building and proceed to the Assembly Area. Procedure for Evacuating Occupants There are three exit stairwells in the building, two on the west side, one on the east side. Occupants will proceed to the nearest stairwell and descend to the ground level. An Evacuation Coordinator will be stationed in the building to help direct occupants toward the building exits and to the designated Assembly Area. In the event of an emergency, DO NOT USE THE ELEVATORS. After exiting the building, evacuees should proceed to the designated Assembly Area at the Plaza de Sol. Evacuees should avoid remaining in front of the building if possible, as the Fire Department and other responding agencies will be occupying that area in an emergency. 2 July 2014

7 Introduction from Building Management was built in is a six story building, with a total square footage of 213,405. The design and construction is steel and concrete, which reduces hazards in the event of fire, earthquake, or explosion. The building was constructed in full compliance with the building and fire codes of the time. The City of Sunnyvale Fire Department supervises all actions in the event of a fire or emergency. 3 July 2014

8 Locations and Functions of Building s Life Safety Features Fire Alarm Information The alarm sound is a loud pulsating buzzer with flashing lights. The alarm will be activated throughout the building and the entire parking garage. The fire alarm system in is monitored by an outside agency, who will then notify the Sunnyvale Fire Department. Smoke detectors are located in the lobby on the first floor, in the elevator lobbies of each floor, ducts and equipment/electrical rooms. is equipped with an automatic sprinkler system and there are tamper alarms on the pressurized water sprinkler system. Building electrical power can be shut down by the Fire Department through a manual shunt trip. Water and gas shut off valves are located at the outside of the building on Mathilda Place. There is a remote fire control panel located in the lobby on the first floor, and the main panel in the Security Office on level P1 of Building 100. These panels function to monitor, record, and silence alarms, and to reset the system. All systems report any troubles or alarms to a remote monitoring company, which immediately notifies the Sunnyvale Fire Department and the Building Management. Public Address System is not equipped with a public address system. In the event of an emergency, Suite Wardens should listen for instructions given by the Incident Commander, Fire Department, or other responding agency. If the fire alarm is activated, proceed to the nearest exit stairwell and evacuate down and out of the building to the Assembly Area. Exits has lighted, battery operated exit signs located throughout the building and at all exits, which identify the locations of the three enclosed exit stairwells. 4 July 2014

9 There are no open stairwells or fire escapes. A safety lighting system is installed throughout the building. In the event of a loss of power, the safety lighting system will be powered by battery back-up for between 2-3 hours. Evacuation signs are posted in lobby areas, at each elevator landing and at each stairwell. Within the stairwells are exit signs at the doors and signs indicating which floor you are on. Fire Doors and Hardware All doors and hardware in are fire-rated at a minimum of 45 minutes. Elevator Recall Elevators are equipped with an automatic recall system, which is activated by the smoke detectors in the elevator lobbies or by a seismic sensor. In the event of an earthquake, the elevators will stop at the next floor in the direction of travel and the doors will remain open. If the automatic recall system is activated by the smoke detectors on Floors 2 through 6, or elevators are manually recalled by the Fire Department, the elevators will return to the first floor lobby and the doors will remain open. If there is a smoke detector activated on Floor 1, the elevators will respond to an alternate floor. Manual Pull Stations and Fire Extinguishers A manual pull station is located in the first floor lobby and fire extinguishers are located throughout. Sequence of Operation When an alarm is activated by a smoke detector or manual fire pull, a signal goes to the fire control panel and illuminates an LED light, indicating alarm(s). The fire alarm panel will sound with a loud beeping and will dial into the remote alarm monitoring agency, which will dispatch fire trucks as well as call the Building Engineers and Building Management. The fire alarm will sound in the entire building and parking garage. 5 July 2014

10 Employee Duties, Assignments and Training Each tenant is responsible for assigning positions as outlined below and provide training for their employees that includes familiarization with duties, evacuation routes and assistance procedures, and exterior assembly areas. Staff should also be familiar with the location and use of fire extinguishers, and with fire alarm signals. Training for staff should occur as part of new employee orientation, and at least annually thereafter. All non-emergency and emergency duties for the positions of Suite Wardens, Assembly Area Manager and Evacuation Coordinator can be found in the checklists beginning on page 35 of this plan. Suite Wardens In an emergency the Suite Warden will organize and manage an emergency situation, ensuring that all necessary emergency duties are performed. Evacuation Coordinator The Evacuation Coordinator assists occupants of in evacuation. All evacuees will be directed to the designated Assembly Area, Plaza de Sol. Assembly Area Manager The Assembly Area Manager accounts for building occupants using the headcount list provided, as they flow through the Assembly Area. 6 July 2014

11 Exiting Procedures When you hear a fire alarm in the building, do not panic. If you are in a closed, internal room, go to the door and lightly touch it. If the door is not hot, gently open it. Be ready to close the door quickly if need be. If the area beyond the door is clear of smoke and fire, go out the door, close it behind you, and proceed to the nearest stairwell exit, knocking on any doors to alert other occupants as you go. When exiting the building, stay against the wall-side of the stairwell, out of the way of ascending fire personnel. Keep calm, exit in an orderly fashion, and follow all directions. Feel doors for heat with the back of your hand before opening, and crawl low in smokeor heat-filled areas. Should a door be hot when you touch it, do not open it. Go to the telephone and call the Fire Department (9-1-1), give them your name, building address, and location within the building, and tell them you cannot get out of the room. Prepare to defend in place. Persons in Need of Evacuation Assistance Disabled occupants should call and tell the dispatcher you are trapped in the building. Relay to dispatcher your exact location in the building. 7 July 2014

12 Fire Before 1. Keep flashlights ready and in a handy place. 2. Be careful not to overload electrical circuits. Short circuits are the cause of many fires. 3. Discard all flammable liquids, excess trash and unnecessary paper storage. 4. Keep flammable materials at least 3 feet away from heaters. 5. Be aware of your environment: a. Be aware of at least two exits from the area you are working in. b. Know the location of manual fire alarm pull stations. When alarm is activated, the alarm sounds throughout the building and the entire parking garage and the Sunnyvale Fire Department is notified. The sound of the alarm is a loud pulsating buzzer with flashing lights. c. Make sure that nothing is blocking, attached to, or otherwise interfering with the proper operation of alarms, emergency lighting or sprinkler systems. Sprinkler systems are located throughout the property, including tenant suites. Report any sign of damage or malfunction to building management. d. Never block fire exits, or block doorways, halls, or stairways. Never prop open fire doors. (Doors with closures are meant to be closed.) Actions of Anyone Who Smells Smoke or Finds Fire 1. Pull or activate fire alarm in the lobby. 2. Extinguish small fires, if safe to do so. 3. Call 911 from a safe location. 4. Evacuate and restrict the spread of fire by closing doors and windows behind you. 5. Alert others in your area. 6. Assist others in exiting, if it is safe to do so. 8 July 2014

13 During 1. Elevators will automatically be recalled when activated by the smoke detectors in the elevator lobbies, or when manually recalled by the Sunnyvale Fire Department. 2. If the alarm is activated, or you are otherwise notified to evacuate the building, follow your building s evacuation plan and go to the designated Assembly Area for headcount, injury assessment and further instruction. 3. Test doors for heat before opening them by placing the back of your hand against them so you do not burn your palm and fingers. Do not open a hot door; instead, find another route. If the door is cool, brace your shoulder against it, open it cautiously, and continue. 4. If you encounter smoke or flames, use your alternate escape route. 5. Close all doors behind you to slow the spread of fire and smoke. 6. Crawl low, under the smoke, to breathe cleaner air. Cover nose and mouth with a wet cloth. 7. Do not use the elevator. 8. If your clothes catch on fire, stop, drop, and roll, or wrap a blanket around you to smother the flames, until the fire is extinguished. Running only makes the fire burn faster. 9. Once outside the building, stay out of the way of firefighters. Tell the Fire Department if anyone is left in the building, and do not re-enter the building until you are instructed to do so. If You Are Trapped 1. If smoke, heat, or flames block your exit route, stay in place with the doors closed. If possible, go to a room with an outside window. 2. Seal doors with duct tape or wet sheets and towels. 3. Seal ventilation ducts and any other openings where smoke may enter. 4. If there is a telephone in the room, call and tell the dispatcher where you are. Do this even if you can see fire trucks in the street below. 5. Stay where rescuers can see you through the window, and wave a brightlycolored cloth to attract attention. 9 July 2014

14 6. Do not break windows unless you are in absolute danger of suffering from smoke inhalation. 7. If smoke is entering the room, breathe through a wet towel and stay low. 8. Be patient as occupants are rescued. 9. If trapped in a smoke-filled corridor, crawl on hands and knees to an enclosed stairway or a smoke-free room and defend in place. After 1. Do not enter a fire-damaged building unless authorized by emergency personnel. Fire Extinguisher Use 1. Ensure that alarm has been sounded. 2. Check fire extinguisher. Is it the proper type and in good condition? 3. Remove the extinguisher from its place and hold it upright. Stand back 8 to 10 feet from the fire. 4. Pull ring pin and aim at base of fire. Crouch down low. Squeeze handle completely. Sweep side-to-side, working your way up. (Remember to keep between the exit and fire; get fresh air immediately.) 10 July 2014

15 Earthquake All elevators in are equipped with seismic devices that respond to vibration. In the event of an earthquake, elevators will proceed to the next floor in the direction of travel and stop. The doors will open and remain as such until the elevators have been safety checked and the seismic switch has been reset by the elevator contractor. Only then will the elevators be allowed to be returned to service. Before 1. Earthquake bracing: a. Secure larger shelves or furniture to wall or floor. b. Lower heavy objects or displays which could fall and injure or block exit ways. c. Keep drawers and cabinets latched. d. Secure TV s, computers, monitors, and other expensive electronics to their bases. 2. In the event of an earthquake, the building management will not provide emergency supplies for you. Basic emergency supplies to have on hand include: a. A portable radio with spare batteries. b. A good flashlight and spare batteries. c. A First Aid Kit and First Aid manual. d. Water and food for at least three (3) days. e. Manual can opener. f. Rescue tools. g. Extra clothing. h. Whistle. i. Heavy-duty shoes and gloves. j. Prescribed medicines and extra eyewear. k. Large and small plastic bags, duct-tape, and scissors. l. Cash. (ATMs and credit cards may not work.) 11 July 2014

16 3. Communications a. Have on hand emergency phone lists with numbers for work, school, play and day care. b. Establish a long-distance message check-in phone number. c. Keep a cell phone for emergency use. 4. Familiarization Exercises a. Know safe, take-cover locations. During 1. Remain calm. 2. Stay inside; take cover in a safe location. 3. Get under the table, desk, or brace yourself in the entrance doorway. 4. Calm others. 5. Stay away from windows to keep from being injured by debris or broken glass. After a Devastating or Damaging Earthquake 1. Be prepared for aftershocks. 2. Check immediate location. (Are you safe?) 3. Check for injuries of others; apply first aid as needed. 4. Wear shoes at all times; watch for broken glass. 5. Extinguish any fires. Do not light matches or smoke. 6. Turn off gas if you: a. Smell gas; b. See a broken pipe; or c. Are ordered to do so. 7. Turn off water or electricity if it is causing a hazard or a pipe is broken. 8. Be prepared to go without emergency services; help yourself and others. 9. Check and make sure that all telephone receivers are on their phone cradles. 10. Use telephone only for dire emergencies. 11. Listen for news or instructions over radio or television. 12 July 2014

17 Anticipate what you will need before an earthquake occurs. Try to be as self-reliant as possible after an earthquake occurs. Should it be necessary to turn off the electricity, natural gas, water, etc., the Fire Department (or building management) will do so. If you hear the fire alarm in your building after an earthquake, assume that the earthquake has caused a fire. LEAVE THE BUILDING IMMEDIATELY BY USING THE STAIRS. DO NOT USE THE ELEVATOR. 13 July 2014

18 Elevator Malfunction / Entrapment The elevators at are equipped with a variety of features designed to ensure the safety of passengers during normal operation, during a malfunction of the equipment, and other emergency that may affect operation. On occasion, elevators do malfunction even though they are properly maintained. This section addresses an elevator malfunction coupled with an entrapment. If passengers get trapped in an elevator, it is considered an emergency. Qualified elevator personnel will respond, as they have the expertise, experience and training to handle these complex situations in which a rescue may be involved. Building Management and/or security WILL NOT provide elevator rescues. In case of an entrapment or emergency in the elevator: 1. Use the Emergency Telephone to establish constant voice communication with the elevator company, Otis Elevator. 2. If the elevator stops between floors, use the Emergency Telephone. DO NOT try to pry open the doors as it may cause additional damage to the equipment that could prolong the emergency. 3. Stay calm and wait for help to arrive. 4. Follow the instructions of Building Management and/or Security. Building Management and/or Security will advise the passenger that: 1. They are safe and should remain calm. 2. Help is on the way to assess the cause and to safely remove them from the elevator. 3. They should stay away from the elevator doors. Do not attempt to pry open the doors. 14 July 2014

19 4. Building Management and/or Security will advise passenger at regular intervals to keep them informed of the status of rescue efforts. The Building Management and/or Security shall ask some/all of the following key questions to gather information and to ease the potential anxiety. The answers to these questions will advise the elevator mechanics of the situation inside the car and provide information to decide the best approach to solving the problem. Questions during initial contact may include: 1. How many passengers are trapped? 2. What are the passengers names and suite locations? 3. Are the passengers calm or experiencing any physical difficulties? 4. Do any of the passengers have medical conditions that might affect their release? 5. Are the lights inside the cab functioning? 6. In what direction was the elevator traveling? 7. Is any call button lit on the panel? If so, which ones? 8. Is there a floor number displayed on the floor position indicator? 9. Is there anyone that the passengers would like us to notify for them? The elevators are equipped with an automatic recall system, which is activated by the smoke detectors in the elevator lobbies or by a seismic sensor. If the automatic recall system is activated by smoke detectors, the following will apply: If a smoke detector is activated on the 1 st Floor, the elevators will recall to the 2 nd Floor. If a smoke detector is activated on Floors 2 through 6, and including the elevator machine room on the roof, the elevators will recall to the 1 st Floor. 15 July 2014

20 All garage elevators will recall to the 1 st Floor if a smoke detector is activated in the garage. All garage elevators will recall to Level P1 if a smoke detector is activate on the 1 st Floor. In the event of an earthquake, the elevators will: 1. Stop at the next floor in the direction of travel and the doors will remain open. 16 July 2014

21 Power Failure A power failure from the utility company can occur for various reasons. Power outages can be building specific, cover a small area (e.g. a few square blocks), city wide or regional. In the event of power failure: 1. DO NOT contact Building Management. 2. Building Management will attempt to advise you regarding the length and cause of the power failure as soon as possible. 3. Do not evacuate unless instructed to do so. is equipped with emergency lights on all floors and in stairwells. These lights, as well as EXIT lights, have a two-hour battery back-up system. 17 July 2014

22 Medical Emergency 1. Upon feeling that you need medical help: a. Immediately inform the closest person to call and summon medical help. b. If no one is around, immediately call and request medical help. c. Give building address, floor number, and the phone number you are calling from. d. If possible, call the Building Management Office and report the arriving medical units. Give your location and your phone number. 2. Upon seeing or being informed of another person in need of medical help: a. Immediately call b. Make the ill person as comfortable as possible. c. Give building address, floor number, and the phone number you are calling from. d. Call the Building Management Office and report the arriving medical units. Give your location and your phone number. e. Get the victim close to an elevator, if possible, or f. Send someone to wait at elevator to guide medical units. 3. Front desk or other building staff upon receiving information that someone in the building is in need of medical help: a. Call to ensure medical response. b. Prop open front doors. c. Recall one elevator for responding medical units. d. Direct medical units to victim. 4. Medical tips: a. Assist the victim to the degree you are trained. b. Wear latex gloves. c. Check breathing; clear airway if necessary. d. Stop bleeding by applying direct pressure. e. Cool a burn with cool running water. 18 July 2014

23 Civil Disturbance 1. Do not go through a violent crowd to leave or enter a building. 2. Call and inform the emergency operator of the situation. 3. Answer any questions asked by the operator. 4. Inform building management or security of the situation. 5. Stay away from any unruly crowd. 6. Remain within the building. Do nothing to antagonize the demonstrators. 7. Close all blinds or drapes in exterior rooms and then avoid window areas. Lock all doors. 8. Report to the core area of the building. 9. Avoid unnecessary phone conversations. 10. Secure valuable materials in a safe place or at least out of sight. 11. If a demonstrator enters the premises, keep calm, be courteous and avoid an incident. Avoid actions, arguments, or verbal responses that may provoke the demonstrator. They have entered the building to propagandize, confront or agitate the building s occupants. Let them make their point. Frustrating them is dangerous. Do not try to reason with them. Call Police to have the individual removed if you can do so without incident. 19 July 2014

24 Terrorism 1. Learn about the nature of terrorism. Terrorists look for visible targets where they can avoid detection, such as international airports, large cities, major international events, resorts and high-profile landmarks. 2. Be alert and aware of the surrounding area. The very nature of terrorism suggests there may be little or no warning. Risk of Attack LOW (Green) Recommended Actions Develop a written emergency plan to address all hazards. Include an emergency communic ation plan to notify employees of activities; designate an offsite report to location in case of evacuation. Develop continuity of operations plan to include designating alternate work facility/location for business. Be alert to suspicious activity and report it to property authorities. GUARDED (Blue) Dialogue with community leaders, emergency management, government agencies, community organizations an utilities about disaster preparedness. Ensure emergency communication plan is updated to include purchase of needed equipment. ELEVATED (Yellow) Be alert to suspicious activity and report it to proper authorities. Contact private security firm for security risk assessment and to determine availability of support/reinforcement. Be alert to suspicious activity and report it to proper authorities. HIGH (Orange) Review emergency plans to include continuity of operations and media materials on hand. Determine need to restrict access to business or provide private security firm support/reinforcement. Contact vendors/suppliers to confirm their emergency response plan procedures. Listen to radio/tv for current information/instructions. Be alert to suspicious activity and report it to proper authorities immediately. SEV ERE (Red) Work with local community leaders, emergency management, government agencies, community organizations, and utilities to meet immediate needs of the community. Determine need to close business based on circumstances and in accordance with written emergency plan. Be prepared to work with a smaller or dispersed work force. Ensure mental health counselors are available for employees. 20 July 2014

25 Bomb Threat Upon Receiving a Bomb Threat Try to retrieve the following information from the caller: 1. What time will the bomb go off? 2. Where is the bomb? 3. What type of bomb is it? 4. Why was the bomb planted? 5. Did the caller plant the bomb? 6. What is the caller s name? Note the Following 1. Time of call. 2. Gender of the caller. 3. Accent, speech patterns. 4. Background noises. 5. Emotion. Upon Hanging up With the Caller 1. Leave the phone off the hook. Do not use it to make any phone calls. You may be able to dial *69 and trace the call back. 2. 27 Explosion 1. If objects are falling around you, get under a sturdy table or desk for cover. 2. Follow your building s evacuation plan and go to the designated Assembly Area for headcount, injury assessment and further instruction. 3. Do not use elevators. 4. Do not stop to retrieve personal possessions or make phone calls. If Trapped in Debris 1. Use a flashlight. Do not light any matches. 2. Cover your mouth with a handkerchief or clothing. 3. Stay in your area so you don t kick up dust. 4. Tap on a pipe or wall so that rescuers can hear where you are. Use a whistle if one is available. 5. Shout only as a last resort; shouting can cause inhalation of dangerous amounts of dust. 6. Be patient. Rescue could take several hours. After 1. Wait for emergency personnel to arrive. 2. Stay out of damaged buildings. 3. Untrained personnel should not attempt to rescue people in a collapsed building. 23 July 2014

28 Biological or Chemical Attack Listen to your radio for instructions from authorities such as whether to remain where you are and seal the premises or to evacuate immediately. In-Place Sheltering 1. Close and lock doors. 2. Seek shelter in an internal room, preferably one without windows. Be sure to take a battery-operated radio with you. 3. Evaluate and implement measures to minimize the transmission of the virus through environmental sources, specifically hard surfaces. For common areas: 1. Limit the number of people in a common area at any given time. 2. Close non-essential common areas. 3. Make hand-sanitizers and other hygienic supplies readily available to employees, if not already done. 4. Suspend discretionary events such as training sessions and large meetings. 5. Implement increased cleaning and sanitation measures. Include the sanitizing of commonly touched surfaces such as arms on chairs, tabletops, doorknobs, countertops, handrails, window sills, light switches, sinks and faucets, copier/printer/fax control buttons, cabinet and file drawer knobs/handles and coffee pots. For shared equipment areas: 1. Educate employees about the means by which the influenza virus is transmitted. 2. Make hand-sanitizers, sanitizing wipes, and other hygienic supplies readily available to employees. 3. Instill hygienic practices of wiping down common surfaces prior to touching them. 4. Limit the contact of shared items. For deliveries and visitors: 1. Limit or prohibit visitor admittance to facilities. 2. Advise employees to meet visitors at an off-site. 3. 45 The following code sections apply to. California Code of Regulations Title ning and Information (a) All office buildings 2 or more stories in height (except highrise buildings as defined by Health and Safety Code Section 13210). (1) Owner(s) or operator(s) shall employ either one of the following methods of providing emergency procedures and information to the building occupants: (A) Emergency procedures information published in the form of a leaflet, brochure, or pamphlet shall be available to all persons entering the building. Emergency procedures information shall be located immediately inside all entrances to the building, as determined by the authority having jurisdiction. Locations shall be clearly marked; or, (B) A floor plan providing emergency procedures information shall be posted at every stairway landing, at every elevator landing, and immediately inside all public entrances to the building. The information shall be posted so that it describes the represented floor level and can be easily seen immediately upon entering the floor level or the building. Emergency procedures information shall be printed with a minimum of 3/16-inch high non-decorative lettering providing a sharp contrast to the background. (2) Emergency procedures information shall provide all ambulatory, nonambulatory, and the physically disabled, instructions to be followed in the event of an emergency. Emergency procedures information shall include, but not be limited to the following: (A) Location of exits and fire alarm initiating stations, if required; (B) what the fire alarm, if required, sounds and looks like (audible and visual warning devices); (C) fire department emergency telephone number 911; and (D) the prohibition of elevator use during emergencies, if any. 41 July 2014
