Date: Approved: ***-) ö --"_*_ a Date: I Transportation Manager, Sound Transit Light Rail á-7-//?* zott J;^ ll 1.0 PURPOSE The purpose of this Standard Operating Procedure (SOP) is to describe the responsibilities of Rail personnel to respond to and investigate an accidenv incident involving Rail operations on the Rail mainline and the yard. The focus of the investigation is fact finding for purposes of preventing future occurrences. The Transit Safety Section is designated to act on behalf of the General Manager in the investigation of all accidents resulting in injury or damage involving Central Link and all Rail Section employees. This includes incidents or near miss incidents as determined necessary by Transit Safety. All Rail Section employees shall cooperate fully with Transit Safety in these investigations 2.0 DEFINITIONS Accident - An unforeseen event or occurrence which results in injury or property damage. AHJ - Authority Having Jurisdiction

KCM - King County Metro Minor lnjury - An injury treated at the scene. Serious lniurv - An injury requiring immediate medical attention and injured transpofted away from the scene. 3.0 REQUIREMENTS All employees involved in Rail accident/incident investigations shall submit a written report. The LCC or the employee's immediate Supervisor shall report the incident to the appropriate KCM Superintendent(s)/Manager(s) and Safety Officers. The Safety Officer will notify state oversight regulatory authority of a reportable accidenuincident within two (2) hours. 4.O PROCEDURES 4.1 Controller: Upon receiving a call regarding an accidenuincident on the mainline the Controller will utilize the appropriate accidenuincident checklist(s) from the All Hazards Response Plan and make all required notifications. The Controller shall instruct the reporting employee and other employees involved to provide a written report of the incident, on the appropriate KCM Rail accident/incident form(s). The Controller will dispatch a Rail Supervisor to the scene to begin an investigation of the accident/incident.

The Controller will communicate with the incident commander at the incident scene. The Controller will confirm the completion of the on-site investigation The Controller will verify that determination has been made Administrator. a post accident drug test by the Drug and Alcohol 4.2 Rail Supervisor/Chief shall: Repod arrival at the accident/incident scene to the Controller and assumes the role of incident commander Ensure that injured person(s) are attended to immediately Upon arrival, will conduct the investigation and appoint others for on scene management Assess the situation and make request for back up help if necessary lsolate and take statement from LRV Operator Assist in the field investigation and complete the Supervisor's Accidenillncident Report. The report shall be comprehensive and include all notes and photos Take and document witness statements Photograph and document all vehicle(s) involved, including license plate numbers, make, model, color, and year of vehicle(s).

Photograph and document the findings. These findings are to be included in a detailed diagram as part of the Supervisor's Accident/l ncident Report Provide regular updates to LCC Ascertain the identification of all persons involved in the accident/incident Notify LCC when the scene has cleared and the trains may resume normal operations Transport the LRV Operator for post-accident drug testing if necessary. 4.3 KCM Responders shall: Report arrival to the scene to the LCC and the incident commander(s) Notify the LCC when leaving the scene. 4.4 LRV Operator: The LRV Operator shall assist Rail supervision and investigating police agencies in the investigation. The Operator of a Rail vehicle involved in an accidenuincident will provide information including, but not limited to, the following:

Nature of any injuries. Whether the OCS, pantograph, or switches are involved. Provide Rail card to supervision or police upon request. Provide a complete Operator's Report of Accident/lncident before the end of shift. 4.5 Rail Employee lnvolved in non-lrv incident: The employee shall also assist Rail supervision and investigating agencies in the investigation. The Rail employee involved in a non- LRV incident will immediately notify the Controller and provide the following: The exact location of where the incident occurred, including the nearest cross streets, milepost signs, signals and/or switches numbers Nature of any injuries Whether the OCS, pantograph, or switches are involved Provide Rail card, driver's license or other legally acceptable identification to supervision or police upon request Provide a complete detailed Operator Accident/lncident Repoft before the end of shift.

18 t{ King County Department of Transportation Metro Ra l OÞerations Sectic n Standard Operating Procedure tsouruprnarusr RIDE THE WAVE SOP: 4.2 Page 6 of 7 Date of lssue: June 30, 2011 Revision: 2 ON.SCENE RAIL ACCIDENT INVESTIGATION PROCEDURE 4.6 Chain of Custody: All information and findings by Rail staff during the investigation shall be secured and made available to the Safety Officer, Metro Transit police, Sound Transit (ST) police, and local police agencies upon request. 4.7 LRV lmpound: LRVs involved in a collision may be impounded and accompanied by vehicle maintenance personnel until secured at the Operations and Maintenance Facility for further examination and testing. The Vehicle Maintenance Chief or designee will oversee the impound process and ensure the LRV is staged on the appropriate track for further examination and necessary data collection. The LRV will be held until after the conclusion of the post accident investigation and upon completion of any necessary repairs. The LRV impound process could also include the following: Confining the LRV in the body shop by taping off entry doors to the body shop and placing the controls of the impounded vehicle "out of service" by attaching blue tags to the controls of both cabs, prohibiting any unauthorized entry or movement without permission from the Vehicle Maintenance Chief or his designee. 5.0 RESPONSIBILITIES The Rail Training Chiefs are responsible for ensuring that the training for all Rail section employees on rail accidenuincident scene managemenu investigation procedures is completed It is the responsibility of all Rail employees to have full knowledge and understanding of this SOP and to adhere to its provisions.

National Fire Prevention Association (NFPA) - An international organization charged with creating and maintaining minimum. standards and requirements for fiie prevention and suppression aclivities, training, and equipment, as well as other life-safety codes and standards.

The Link Control Center is iesponsible for Metro Transit rail Operations, Communications and DSTT access. Runner - One of two controllers in the LCC who can leave the facility (for short periods of time), while maintaining radio contact, to perform LCC related tasks. Seattle Fire Department (SFD) - The primary fire and medical first responder to the DSTT and the northern portion of the KC Metro Rail line. Supervisory Control And Data Acquisition (SCADA) - Computer software used bylcc to monitor and control DSTT and KC Metro Rail systems. 3.0 REQUIREMENTS All Rules, Policies and Procedures must be followed by personnel working in the Link Control Center. All controllers shall become familiar with this procedure. 4.0 PROCEDURES 4.1 Link Control Center Access:

Access to the SCADA software requires a valid login user name and Password or activated card key r NFPA Access to stations shall be restricted to those persons directly concerned with the implementation and direction of emergency actions and procedures. 4.2 Link Control Center Staffing: SOP # 6.2 Page(s) Page 3 of 10 Date of lssue April 11, 2008 Revision # 0 RAIL ACTIVATION LINK GONTROL CENTER FIRE ALARM MONITORIN At least one chief controller and one supervisor controller shall be on duty at alltimes.(per NFPA 72, see below) One controller may be a runner. When a controller is acting as a runner, and not present at the LCC, he/she shall use a tnobile radio to maintain two-way communications with the LCC. (per NFPA 72, see below). NFPA At least two controllers (does NFPA allow for us to use controller throughout document?) shall be on duty at all times. One of the two operators may be a runner' o o NFpA T When the runner is not in attendance at the supervising station, he/she shall establish two-way communications with the station at intervals not exceeding 15 minutes. Exception: when two or more operators are on duty, a runner physically in attendance at a noncontiguous protected premise and immediately available via telephone or other approved means of communication shall not be required to

60 KingCounty Department of Transportation KG Metro Rail Section tsouruotnnrsn HDEfAEWAyE Standard Operating Procedure SOP # 6.2 Page(s) Page 4of 10 Date of lssue April 11, 2008 Revision # 0 RAIL ACTIVATION LINK CONTROL CENTER FIRE ALARM MONITORING AND RESPONSE maintain two-way communications at 15 minute intervals, if that runner is not responsible for another protected premises. 4.3 Controller Primary Duty: o The primary duty of the controllers is the safe operation of the rail/dstt facility by monitoring and operating the fire, ventilation and signal systems, and the reporting of and responding to automatic alarms and life safety emergencies (per NFPA 72, see below) o NFPA Primary duties of the operator shall be to monitor signals, operate the system, and take such action as shall be required by the authority having jurisdiction o NFPA The operator(s) shall not be assigned any additional duties that would take precedence of the primary duties 4.4 Alarm System Testing: Communications between SCADA and the FACP shall be tested every 24 hours to verify operations. Any communications failure shall be reported to the Metro Power Chief or on-duty Metro Power personnel immediately. (per NFPA 72, see below) Until SCADA is turned over to Metro Transit; SCADA failures shall be reported to the Sound Transit On-Call person and the Link Rail Superintendent or designee. r NFPA All communications and transmission channels between the supervising station and the protected premises control panel shall be operated manually or automatically once every 24 hours to verify operation. lf

(per NFPA 72, see below) r NFPA Indication of a fire shall be promptly retransmitted to the public fire service communications center or other locations accepted by the AHJ, indicating the building or group of buildings from which the alarm has been received, o NFPA The means of retransmission shall be accepted by the AHJ and shall be in accordance with Section 8.2 (central station), 8.4 (remote station), or Chapter I (public reporting systems). Appendix -intent of the code is that the operator has a secure means of immediately retransmitting any signal indicative of a fire to the public fire department _ communications center. Automatic retransmission using an

However, a manual means can be permitted to be used, consisting of either a manual connection following the requirements referenced, or for proprietary supervising stations serving only cbntiguous properties, a means in the form of a municipal fire alarm box installed within 50 feet of the proprietary supervising station in accordance with Chapter I can be permitted. 4.7 Automatic Fire Alarm Notification Procedure: Upon receipt of a fire alarm (other than Supervisory) the controller shall: immediately notify the Seattle Fire Department (SFD); dispatch a service supervisor, Metro Power electrician or other runner to the scene; and restore the fire alarm system as soon as the cause of the alarm has been corrected (per NFPA 72, see below) Upon receipt of a Supervisory or sprinkler system alarm the controller shall: dispatch a service supervisor, Metro Power electrician or other runner to the scene; and restore the fire alarm system as soon as the cause of the alarm has been corrected. lf a condition exists that causes all or part of a sprinkler system to be out of service for 8 hours or more, the Seattle Fire Department (SFD) shall be notified. Written notice shall be provided including the nature of the signal, time of occurrence and restoration of service. Fax the information to Attn: Confidence Testing Unit Lieutenant (per NFPA 72, see below) Upon receipt of a trouble alarm or other signals pertaining solely to alarm system equipment maintenance the controller shall: dispatch a service supervisor, Metro Power electrician or other runner to the scene; and restore the fire alarm system as soon as the cause of the alarm has been corrected. lf a condition exists for

NFPA 72-8.g Upon receipt of sprinkler system and other supervisory signals, the supervising station operator shall initiate action to: communicate immediately with the designated person to ascertain the reason for the signal; dispatch personnel to arrive within 2 hours.to investigate, unless supervisory conditions are promptly notify the AHJ when sprinkler systems are wholly or partially out of service for 8 hours or more; provide written notice to the AHJ as to the nature of the signal, time of occurrence, and restoration of service, when equipment has been out of service for 8 hours or rnore. Fax the information to Attn: Confidence Testing Unit Lieutenant NFPA Upon receipt of trouble signals or other signals pertaining solely to matters of equipment maintenance of the fire alarm system, the supervising station operator shall initiate action to: communicate immediately with designated persons to ascertain the reason for the signal; dispatch personnel to arrive within 4 hours to initiate maintenance, if notify the AHJ when interruption of service

In general, this requires monthly testing of communication channels and receiving equipment and visual inspeclions of equipment Testing and Maintenance of Fire Alarm System Transmission Technologies: This section spells out the testing procedures of transmission technologies from LCC end of things to the DSTT fire alarm equiprnent (per NFPA 72, see below)

(1999) NFPA Testing and Maintenance Requirements for All Transmission Technologies. ; Testing and maintenance of communications methods shall be in accordance with the requirements of Chapter DSTT - One Emergency Fan Out of Service When LCC becomes aware of an Emergency Fan that is not working or is being removed from service during DSTT public use hours the Seattle Fire Department (SFD) must be notified and the use of their Mobile Ventilation Unit (MVU) requested ( ) The controller will notify the Light Rail Superintendent or designee and the DSTT Metro Facilities Chief of the emergency fan situation and the request for the SFD MVU SFD will stage the MVU in a predetermined DSTT station center lane based on the location of the out of service emergency fan. SFD is solely responsible for the movement, placement and operation of the MVU f the SFD MVU is not available, the DSTT needs to closed and all transit service routed to surface streets. 5.0 RESPONSIBILITIES It is the responsibility of all Link Controllers and designated KCM responders to read and understand this procedure.

This procedure governs the safe movement of trains and buses, while in the DSfi, and provides a guideline for Bus/Rail Operators to follow. 2.0 DEFINITIONS Acknowledge - An appropriate response when a message is received and understood. Aspect - The combination of color and position of a fixed signal light or lights which conveys an indication as viewed by an Operator. Coordinator - The designated employee on duty in the bus control center having direct communications with Bus Operators, and acting as liaison between Bus Operators and Rail Control. Rail Controller - The designated employee on duty having authority over all train and bus movements in the DSTT, and other activities occurring on the light rail system. Rail Control Center The Operations and Communication Center responsible for the safe operation of the light rail system, including the DSTT, working in conjunction with Bus Control Center.

Right of Way (ROW) - In general, the area that exists within 10 feet of the nearest track and other property that is dedicated to Light Rail Operations (i.e. signal communications enclosures, substations, electrical distribution elements and other structures). Signal - A method of conveying a visual indication to train, bus and equipment operators concerning conditions affecting their movements in the DSTT. Signals include audible, bar, fixed, flag, hand and interlocking. Slow Zone - An area within defined limits where train, bus or any rail mounted vehicle speed is reduced for track work or other purposes. 3.0 REQUIREMENTS All Rules, Policies and Procedures must be followed by all personnel working in the DSTT. Bus and Rail Operators will be required to attend and pass a comprehensive training course. Bus Operators must pass a DSTT Bus Operators Training Course and Rail Operators must pass a Rail Operators Training Course. All Operators must adhere to signals that are used to control the movement of buses, trains and other vehicles within the tunnel, unless otherwise authorized by Rail Control. DSTT Bus/Rail Operators and Supervisors must familiarize themselves with the DSTT signaling system as indicated in the Operator Manuals. Bus or rail supervisors will be stationed in the DSTT during bus only service. Whenever possible, a supervisor will be at each entrance to the DSTT, and one in the mid-point, during bus and train operations. All of the DSTT supervisors must complete a DSTT Training Course, which includes

Rail controllers will also complete a Controllers Training Course which will include a DSTT Training Course. In the event of an emergency that requires immediate attention to safeguard persons or property, if safe to do so, any person must immediately notify Rail Control via radio (Bus Operators via Bus Control radio channels), Emergency Telephones (ETEL) or the Passenger Emergency Telephones (PETS). 4.0 PROCEDURE 4.1 Bus Supervisors Shall perform DSTT duties as outlined in the Superursor's Manual for Tunnel Operations Shall assist and follow the instructions of Rail Control during all incidents and emergencies Shall notify Rail Control prior to closing and opening any stations, ahd subsequently notify Rail Control when the station is secured. 4.2 Bus Operators Shalt perform operator duties as outlined in the Operato/s Manual for Tunnel Operations Shall follow instructions of their Coordinators Shall not pass any vehicles in the tunnel, unless authorized by Rail Control.

SOP # 6.4 Page(s) Page 4 of 5 Date of lssue March 7,2008 Revision # 0 RAIL ACTIVATION DOWNTOWN SEATTLE TRANSTT TUNNEL (DSTT) OPERATIONS PROCEDURE Shall be responsible for the safe, efficient and reliable operation along the entire Link system by controlling, monitoring and overseeing operators, supervisors, maintenance personnel when on/near the Link ROW, scheduled revenue service, special train movements and special events in accordance with established policies and procedures, safety rules, and government regulations. 4.g.2 Shall maintain two-way radio communications with Bus Operators by passing/receiving information to/from Bus Controllers, Supervisors, maintenance and other personnel in accordance with FCC regulations during systems integration testing, start-up operations and during revenue service operations Shall manage and communicate with all appropriate personnel and agencies during incidents or emergencies and initiate the call-out notification process Shall utilize Supervisory Control and Data Acquisition (SCADA) System and be familiar with tunnel operating systems, their use, their relationships, including entrances, exits, vehicle operating paths, restricted access areas and the location of all emergency systems Shall adjust service levels and headways/intervals as indicated by operating conditions and passenger loads.
