9 TC5: Activities during 2014 Promotion of Safety Awareness & Accident Statistics Reviewed serious electrical Near Misses and Fatalities Promoted the implementation of a reporting system by the Fire Services in relation to fires due to Electrical Causes Identified the requirement to have responsibility assigned for investigation of the causes of Fatal Electrical Accidents in Domestic Premises Examined risks associated with high energy output electric fencers and inputted to H.S.A approach.

19 Fatalities caused by Electricity: A 22 year old man was electrocuted in an ESB HV Substation when he apparently made contact with live HV apparatus 2. A 14 year old teenager was electrocuted when he apparently suffered an electric shock when using a power washer on a farm
20 Fatalities caused by Electricity: A 50 old ESB Staff member was electrocuted in an ESB HV Substation when he apparently made contact with live HV apparatus 2. A 59 year old retired male was electrocuted when he apparently suffered an electric shock in his home

23 National Fire Statistics : 2012 Fires in buildings In the home Electrical causes , 986 7, * to to 11 Fires Deaths Smoke Detectors not in use in the majority of fatality cases (still the case)

30 Overhead Lines and Trampolines During a storm a trampoline was blown up onto transformer pole. MV transformer pole was broken by a trampoline. The trampoline blew up into the line mid span and the shock loading twisted the cross arm, splitting the pole from the top down. The transformer was left hanging precariously. 30 esbnetworks.ie
31 LV Cable Short- Circuit A short circuit on an LV cable head blew three 315A LV fuses at a 10kV sub. The flashover at the cable end left a large flash mark on an election poster. This highlights the huge safety risk of putting any posters on ESB poles. 31 esbnetworks.ie
32 Un-Authorised Generator Connections Customer had pulled single phase conductor from a broken pole next door (during storm) and connected it to a generator (see photo) OBC conductor connected to generator 32 esbnetworks.ie
33 Un-Authorised Connection NT while on MV hazard patrol entered a yard to gain access to the field behind when he noticed 2 trailing leads with 13amp water proof sockets attached lying on the ground beside an LV pole. These leads were hanging down the pole and were looped through the top stay bow for support. The connection onto the OBC was by means of PVC earth wire tails connected with OBC connectors and in turn to strip connectors. The yard was used by a circus for winter park-up and maintenance. 33 esbnetworks.ie
34 Un-authorised Connection at ESB Minipillar During a routine inspection, ESB came across an illegal connection from a MP. Flexible cable had been connected to the busbar using crocodile clips and the minipillar closed up again and partly hidden with broken branches. 5A flowing in the cable when it was discovered 34 esbnetworks.ie
35 MV Switchgear Magnefix Magnefix switch blew up and blew the substation door off the hinges. No injuries sustained by either staff or public and switch gear is being replaced. This incident is under investigation by Underground Section, Asset Management. Prior to commencing work, always carry out the pre-use checks using the Ultra Tev. 35 esbnetworks.ie
36 Farm Safety A Farmer working in a field with tractor and trailer struck a 20kV single phase pole. The side window opened on the tractor and when the driver turned to close it, he veered off track striking the pole. 37 Pole Collision - Rural Tractor which was handling round bales, struck an MV stay. Shock tension caused one MV conductor to break and fall to the ground across a metal trailer. Both MV fuses blew on short circuit. MV pole broke below the trafo mounting bolt and hung sideways in a very dangerous position. Landowner rang in to say there was a wire down but continued to drive over the conductor before the on-call NT arrived to make the site safe. 37 esbnetworks.ie
38 Pole Collisions Rural 38 esbnetworks.ie
39 Pole Collisions Urban Car struck a bundle LV pole Pole was snapped clean off at the butt and shoved 3 metres across the foot path Held by the aerials. No pedestrians were injured. 39 esbnetworks.ie
40 External Wall Insulation Customer call that aerial between the two properties was very low. External insulation had recently been fitted to a neighbour s house. The conductors were left in a dangerous manner and could very easily clash due to poor sagging. Also, how the aerials were now anchored to the newly insulated wall was suspect. Contractor was informed of the dangerous work practice. SEAI and NSAI were notified.. 40 esbnetworks.ie
41 Scaffolding Near LV Overhead Aerials Call received to insulate wires to allow work to proceed on a damaged roof. Scaffolding already erected Aerials had been covered in places with red ducting and garden hose and was cable tied to the uprights. Exclusion zone for insulated LV conductors is 1m. Aerials replaced and insulated 41 esbnetworks.ie
42 Meter Position Fault When an NT arrived at a no-supply call he found that the concentric cable had completely burned away from the cut-out and was sparking violently due to ingress of water. The meter cabinet was severely damaged. The service was disconnected and all apparatus replaced before reenergisation. 42 esbnetworks.ie
43 Meter Position Fault No-supply call. Customer getting a smell from ESB meter. Lights flickering and then the power went off. ESB fuse unit had melted but fuse had not blown. 43 esbnetworks.ie
44 Meter Interference SIRS An NT on revenue protection found a premises that was illegally connected. The potential for a fire in the house can be seen from these photographs. 44 esbnetworks.ie
45 Electrical Accidents NT received minor burns to his left palm and forearm following an electrical flashover. He was working on an LV three phase pole while restringing 3 phase LV conductor as part of a System Improvement at the time of the incident. An NT received a shock from 38kV station gates while attending the station for a 10kV earth fault. The NT was thrown backwards from the gate. An NT received an electric shock from the armour of an MV cable that he was in contact with following an adjacent dig-in into this cable by a third party.. 45 esbnetworks.ie
