2 I. INTRODUCTION Alfred University is regulated as a Small Quantity Generator (SQG) of hazardous waste. As a generator of Hazardous Waste the potential exists for an unplanned sudden release and non-sudden release of hazardous materials and wastes through an unforeseen fire, explosion or other accident (e.g., spill, leak, discharge, reaction, etc.). This Emergency Response Plan for Hazardous Material Release document sets forth the steps to follow in the response to a hazardous material/waste release from the campus hazardous waste storage area, laboratory, satellite accumulation area, or maintenance area. The regulatory authority is the Environmental Protection Agency (EPA), pursuant to the Resource Conservation and Recovery Act (RCRA). II. PURPOSE The purpose of this Emergency Response Plan is to provide University staff and response personnel with specific set of procedures in case of a hazardous substance or waste release. These procedures describe the arrangements with state and local emergency responders, fire departments, contractors and EMT personnel. These arrangements will minimize hazards to human health or the environment if a hazardous substance or waste release occurs. The roles and responsibilities of Alfred University s response personnel, during a hazardous materials release, are described herein. Upon receiving a report of a hazardous material release, University response personnel will immediately activate the Emergency Response Plan. In addition, a building evacuation may be initiated, if necessary. III. ROLES AND RESPONSIBILITIES A. University Staff and Faculty Will follow the procedures outlined herein. Use, store and handle hazardous materials and/or generate, transport and dispose of hazardous wastes in their work place in accordance with Alfred University policy and procedures.
3 B. Safety Department The functions outlined below will be conducted outside the incident response zone designated as the clean zone. 1. The Security Office Will serve as the initial line of communication within the Emergency Response Plan 24 hours a day, 7 days a week. 2. Director of Safety Will assist the Environmental Health & Safety Coordinator or other responding agency. Provide guidance and support to staff during an emergency response situation. Keep University officials current on the status of the incident, by reporting any information received from the Emergency Coordinator. Engage in site activities, such as securing the inventory list of chemicals stored in the area of concern or retrieving and transmitting material safety data sheets (MSDS), etc. C. Environmental Health & Safety Office The following job categories are done by trained and experienced Hazardous Material Technicians when the Emergency Response Plan is initiated. The Environmental Health & Safety Coordinator or designated alternate will respond initially to any incident involving the release of a hazardous material or waste. If warranted, additional responders (including internal or external response personnel) will be assembled and activated. 1. Internal Response Personnel (Hazardous Material Technicians) Will prepare for cleanup of the incident, after assessing the situation. Serve as backup to an external response team.
4 2. First Responder Will contain release from a safe distance Will assist other internal emergency responders 3. Emergency Coordinator (Environmental Health & Safety Coordinator) Coordinate the internal emergency response efforts. Assess the incident to determine the feasibility of cleanup by the internal emergency response personnel. Determine whether assistance from an external emergency response contractor is necessary. Assist the external response team as needed. 4. Back Up Emergency Coordinator Assume Emergency Response Coordinator responsibilities in his/her absence. Assist the Emergency Coordinator with all aspects of the emergency response. D. Physical Plant This Department is comprised of several divisions including Maintenance, HVAC, Electrical, and Plumbing. The functions outlined below will be conducted outside the incident response zone designated as the clean zone. 1. Director of Physical Plant Notify Maintenance, HVAC, Electrical, and Plumbing and other shops as instructed by the Emergency Coordinator for assistance during a hazardous material release. 2. HVAC Personnel Monitor and adjust the ventilation near a hazardous material release, as requested by the Emergency Coordinator.
5 3. Electrical Personnel Assist in identifying electrical conditions that may pose a threat to emergency responders during an incident response, and adjust as necessary. 4. Plumbing Personnel Assist in preventing a hazardous material release from entering the storm/sanitary sewer system. Assist emergency response personnel with remediation of a hazardous material release that has entered the University s plumbing/sewer system. E. Central Administration Will receive periodic status reports from the DSST during a hazardous material release. Communicate and disseminate information to the general public if community notification is required. F. External Agencies 1. Alfred Volunteer Fire Department Will provide emergency response, backup the internal emergency responders, or serve as Incident Commander during a hazardous material/waste release or fire incident. 2. External Emergency Response Vendor Provide emergency response services as requested by the Emergency Coordinator. (See Appendix A) IV. PROCEDURES A. Staff and Faculty University employees are responsible for cleaning up routine incidental spills that occur during routine use, storage, and handling of hazardous materials within their place of work. A routine incidental spill is a spill where there is no increase in hazard to the worker or the surrounding individuals.
6 Make determination if the spill is a routine or non-routine incidental spill. Evacuate and secure room in event of a non-routine incidental spill. Report the non-routine incidental spill to ext immediately. For small-localized fires (e.g. a fire the size of a waste paper basket) that do not involve hazardous materials, any employee trained in the use of the fire extinguisher in accordance with the building-specific Evacuation Fire/Safety Plan can do the following: Activate the nearest fire alarm Use the appropriate extinguisher to put out the fire Once the fire is extinguished, the incident must be reported following Alfred University procedures. The employee using the fire extinguisher must report the used extinguisher to the Director of Physical Plant for recharging. In the event of a fire where hazardous materials are involved or may become involved, the employee must do the following: B. Security Immediately evacuate the room/area. Close the door/windows behind him/her, whenever possible. Activate the nearest fire alarm. Notify ext Follow fire evacuation procedures posted in the area. Do not re-enter building until told to do so by a University official. A dispatcher is available 24 hours/day, seven days/week to receive calls at extension 2108 for hazardous materials releases requiring cleanup assistance. The following questions will be asked of the University employee reporting the release incident: Employee name Work location and telephone extension Location and room number where fire, explosion, or release occurred Material spilled Were any injuries sustained
7 The dispatcher will call the Emergency Coordinator during normal business hours. Beyond regular working hours, the operator will contact the Emergency Coordinator using the current call list or the emergency coordinator s cell phone number (see Appendix A). If injury or fire occurred, the dispatcher will contact 911 emergency services. The dispatcher may be asked by the Emergency Coordinator to contact any of the following during a release incident (Refer to Appendix A for complete Emergency Response Directory) Alfred Volunteer Fire Department Alfred Ambulance Emergency Response Vendor 1. Director of Safety, Security and Telecommunications Request additional services from external agencies to assist with traffic and crowd control. Collect pertinent information during and after the spill incident. Provide assistance to the Emergency Coordinator, Alfred Volunteer Fire Department or any other external agency. C. Alfred University Emergency Response Team 1. General Practices The Emergency Coordinator and/or alternate receive information from the Security Dispatch Operator about the reported hazardous material release. The Emergency Coordinator may contact the person who originally reported the release to confirm the report and gain any additional information concerning the incident, including but not limited to: Employee Name Work Location Location and room number where the fire explosion or other release occurred Specify information regarding the material spilled Amount Spilled Source and extent of the spill Actions taken to isolate the area Whether any injuries where sustained as a result of the incident and
8 Other extenuating factors If injured personnel are to be transported to a medical facility, the Emergency Coordinator or Designee will furnish a Material Safety Data Sheet (MSDS) if exposure to hazardous materials were involved. The Emergency Coordinator will ask if the employee cordoned off the area (i.e., close the door) to secure it from ingress. The Emergency Coordinator will determine the hazards of the substance. Review may include the MSDS and other applicable reference sources. The Emergency Coordinator will request an additional evacuation or partial evacuation in accordance with the Emergency Evacuation Plan for that building if the release threatens the health and safety of individuals. 2. Reporting Requirements If the release constitutes a threat to human health or the environment, the Environmental Health & Safety Coordinator will contact the New York Department of Environmental Conservation (NYDEC) Emergency Response Team, and the Environment Protection Agency s (EPA) National Response Center (NRC). 9 The Environmental Health & Safety Coordinator will give the NYDEC the following information: Chemical discharged Location of discharge Extent of discharge Actions that will be taken to contain, clean, and remove the discharge and Any other information requested If the release is into a sink or the sewer system the Alfred Wastewater Treatment Facility will be notified immediately. A follow up letter will be sent describing how much of what material spilled and the corrective actions taken. The letter should be sent to the following address: Village of Alfred North Main St. Alfred, NY (607) A written report of the incident will be submitted to NYDEC within fifteen days after the incident or as advised. The report will include: Identity of material released Estimated amount of material released Exact location of material released Type of container involved in release Area material was released to (asphalt, soil, storm drain etc.) Name and address of spiller Notification to OSHA within 8 hours after the death of any employee from a work related accident or the hospitalization of three or more employees as a result of a work related injury: Contact OSHA at (800)
10 The following information will be given to OSHA when the notification occurs: 3. Spill Response Protocol The establishment name Location of the incident Time of the incident Number of fatalities or hospitalized employees Contact person Brief description of the incident Other information, as requested Upon being notified of a hazardous material release requiring assistance the Emergency Coordinator will determine if the Alfred University Emergency Response Team will respond or whether outside assistance is needed. This decision will be based on the type of material spilled, the amount and extent of the release incident, and whether the Alfred University Emergency Response Team can adequately conduct the cleanup with the expertise and equipment they have at their disposal. a. If the Emergency Coordinator determines that the release warrants an internal cleanup response, the listed procedures will be followed: The Emergency Coordinator will designate a person to remain at the location to prevent unauthorized access into the area. The Emergency Coordinator shall assemble a response team and backup team to perform the cleanup of the incident. The Alfred University Emergency Response Team will collect the equipment and supplies needed to stage the clean up response (Refer to the Alfred University Emergency Response Team Equipment List available in Appendix B). The MSDS and other reference materials will be used to determine the appropriate PPE and spill cleanup equipment. The spill will be neutralized/adsorbed, containerized, and labeled as hazardous waste with the chemical constituents and date. Contaminated PPE and equipment cleanup will be decontaminated or disposed of as hazardous waste. The containerized waste will be labeled and then stored in Hazardous Waste Central Accumulation Room.

If the Emergency Response Coordinator determines that the release incident warrants an external cleanup response, the following procedures will be followed: The Emergency Coordinator will designate a person to remain at the location to prevent unauthorized access into the area. The Emergency Coordinator will contact the Back up Emergency Coordinator or designated individual to assist with the incident. The Emergency Coordinator will contact the DSST or designated alternate to assist with the incident. The Emergency Coordinator will specify a designated arrival site to the external response team coming on to campus to assist with the response. The Emergency Coordinator will communicate information on the status of the emergency response to the DSST periodically. The DSST will contact the local agencies to assist with the incident such as traffic and crowd control, if necessary. The DDST will contact the Director of the Physical Plant to assist coordination with other shops within the Physical Plant Department to aid in the response. The DSST and/or Emergency Coordinator will meet the external response team at the designate arrival site and escort them to the site on the incident and update them on the situation. The back up emergency coordinator will stay at the site of the release. D. Physical Plant 1. Director of the Physical Plant The Director of the Physical Plant will coordinate the communication efforts for the Physical Plant when requested by the Emergency Coordinator during an emergency release.
12 2. HVAC The HVAC personnel will proceed to the location specified by the Director of Physical Plant if HVAC services are needed during a hazardous materials release incident. The HVAC personnel will assist Emergency Coordinator by adjusting the ventilation to minimize the release of hazardous vapors throughout the building, or to bring fresh air into the building via intake system. 3. Electrical The Electrician will proceed to the location specified by the Director of Physical Plant if electrical services are needed during a hazardous material release. The Electrician will assist the Emergency Coordinator by alternating the electrical system during the hazardous material release. 4. Plumbing The plumber will proceed to the location specified by the Director of Physical Plant if plumbing services are needed during a hazardous material release. The plumber will assist the Emergency Coordinator during the remediation of a discharge of a hazardous material that has entered the plumbing system The plumber will assist the Emergency Coordinator in preventing a release of a hazardous material from entering the storm/sanitary sewer system. E. External Emergency Services 1. Emergency Medical Services The DSST will call the Alfred Volunteer Fire Department or Alfred Ambulance for support when needed. The St. James Mercy or Jones Memorial Hospital will be notified if their assistance is needed.
13 2. Alfred Volunteer Fire Department The Alfred fire Department will be called for emergency response to a hazardous material release when their assistance is needed to investigate, or assess the release. The Alfred Fire department will be notified to investigate, assess and/or remediate any fire or explosion incident, regardless of the magnitude. If requested the Alfred Fire Department will assist in crowd and traffic control. 3. Contracted Emergency Response Vendor The emergency response vendor under contract with Alfred University is available to provide 24 hour a day emergency response and spill cleanup service during a hazardous material release. (See Appendix A) The Emergency Coordinator or DSST will instruct the Vendor where to arrive for any hazardous material release response. The vendor will meet the Emergency Coordinator at the designated area and be escorted to the scene of the hazardous material release and receive information pertaining to the release. V. ARRANGEMENTS WITH OUTSIDE AGENCIES A. Arrangements have been made to familiarize police, fire department and emergency response teams with: Layout of the facility Properties of the hazardous materials handled at the facility Places where facility personnel would normally work Facility entrances and possible evacuation routes B. The Alfred Fire Department has received a copy of the Emergency Response Plan and hazardous chemical inventory. The Emergency Response Contractor is available to respond to a chemical spill or hazardous material emergency. The contractor will be available 24 hours a day, 7 days a week.
14 VI. TRAINING Training in the execution of this Alfred University Emergency Preparedness Procedure is reviewed annually. Fire extinguisher training is in accordance with the University policies and procedures. Training of Staff and Faculty working with hazardous chemicals/substances is in accordance with the 29 CFR Hazard Communication Standard. All members of the Alfred University Emergency Response Team will receive an initial 24 hours of training in the Hazardous Material Technician Level prior to being a member of the team and refresher training annually pursuant to 29 CFR Physical Plant and other employees designated by Alfred University will receive Emergency Response Awareness or Operations level training upon being hired and annually thereafter.
