1 WORKPLACE SAFETY INSPECTION CHECKLIST (To be done once every 2 weeks) This form is to be submitted electronically in PDF format to TANCY@ntu.edu.sg A different form is to be filled for each room, for PIs with more than one lab. CIRCLE the appropriate response School/Department: Building & Room: Date of Inspection: Part (1) For All Workplaces Part (2) For Workplaces Handling Hazardous Materials Status During Inspection: occupied unoccupied Lab Equipment Room Other Part (1) General Postings and Policies Essential safety information displayed (contact details of PI and safety leader, SPMS safety policy etc.) Emeregncy escape route displayed Emergency phone numbers posted General Safety and Housekeeping General housekeeping acceptable (area is tided/clean, no trip/fall hazard) Width of evacuation route is at least 1.2m clear and free of any obstructions Shielding available on vacuum/pressurized glassware Food, drink, and cosmetics banned in lab Suitable lightings (sufficient illumination) Unused/empty cardboard boxes disposed of Division NSS local support staff are informed of the disposal of any damaged or unused equipment/instruments PPE including gloves, safety eyewear, labcoats are properly stored Electrical Safety Approved plugs, sockets and adapters used Clear access to electrical panels Are loose or frayed cords on electrical devices replaced or repaired Are sockets/plugs overloaded at one outlet Are electrical faults reported to NTU Fault Reporting Number 4777 Are cables which are running across the lab floor secured with cable protector ramps 1

3 Part (2) Hazardous Materials Storage and Safety Check All bottles/containers affixed with GHS logos and labels No storage of chemicals in the open and on the floor Chemicals stored together are compatible Liquids stored below eye level Peroxide forming chemicals dated/tested, e.g. ethers, ketones Flammable Liquids stored/used away from ignition sources Gas and Cryogen Safety Cylinders tightly secured on brackets and chains Capped when not in use 2 Flammable/02 tanks/lab Regulators shut off (when not used) Tubes dispensing cryogenics properly insulated Cylinders are chained individually Cylinders are individually labelled with PI s name Hazardous Materials Safety Equipment Check Fume hood sash in good working condition (Able to pull up and down) Fume hood bench uncluttered (obstructing airflow) Fume hood is clean and tidy Fume hood certified for use (Check for valid date on SMM sticker pasted on fume hood) Biological safety cabinet clean and tidy Showers and eyewash not obstructed Eyewash in good working condition when pushed (Water is clear) Spill kit available 3
4 Personal Protective Equipment Check Appropriate types of gloves provided (hot, cryogenic, acid, flammable, etc) Appropriate type of goggles provided (laser, chemical, etc) All lab users are dress appropriately (No slippers, no exposed toes, no shorts/bermudas) All lab users are donned up with the required PPE (lab coat,safety googles and gloves) Hazardous Waste Collection and Storage Hazardous waste containers are appropriately labelled Waste carboys segregated in secondary containments Waste containers up to 80% filled are promptly disposed Sharp objects managed properly, including disposal (razor blades, needles, broken glass, scalpels) No broken glassware to be used Containers sealed clean non-leaking wates are segregated according to compatibility Laser Safety Check PPE provided and applied (laser goggles, filtering glass, UV shielding glass) During the operation of lasers that are Class 3B and above,laser working area is enclosed with black curtain,laser warning light at lab entrance is turned on and laser working area is surrounded by laser safety barriers When laser beam is exposed, it is below eye level and the beam end is blocked High power and ultrafast laser beam is totally enclosed and not exposed Glove Box Check (1)Availability of 2 wall/ceiling exhaust points for glove box operation? (One for Regeneration Out tube and another for vacuum pump outlet) (2a) Is there a glass trap connected to the Regeneration Out tube? If yes to (2a), (2b) Is the glass trap connected to a wall/ceiling exhaust point via a plastic hose/tubing? If yes to (2b), (2c) Is the plastic hose/tubing properly connected to the wall/ceiling exhaust point, via hose rings or adapters? Using tape is NOT allowed (3a) Is the vacuum pump outlet connected to the wall/ceiling exhaust via a hose/tubing? (3b) Is the vacuum pump outlet connected to a DIFFERENT wall/ceiling exhaust as the one for the Regeneration Out tube? (3c) Is the vacuum pump tubing connected properly to the wall/ceiling exhaust, via hose rings or adapters? Using tape is NOT allowed. 4
5 (4) Are the exhaust points and tubes labelled correctly? (5) Is the glass trap protected? (i.e. using a holder or wrapped in a cloth/foam) General Comments: (Make note of particular safety issues.
