1 Terms and Conditions 5th International Students Conference Sound Ambiguity 1. The Organiser of the 6th International Students Conference Sound Ambiguity, hereafter referred to as the Conference, is the Research Association of Composition and Music Theory Students at the Faculty of Composition, Conducting, Music Theory and Music Therapy of Karol Lipiński Academy of Music in Wrocław. 2. The objective of the conference is to improve interdisciplinary knowledge of music, broad the students horizons, share experience and innovative techniques used in research on art. 3. The Conference is addressed to the students of bachelor s, master s and doctoral programmes in the field of music theory, musicology and other academic disciplines who are interested in the topic of the Conference, and graduates in the aforementioned fields. 4. The Conference is managed in terms of the content by Prof. Anna Granat-Janki. The Conference is managed in terms of organisation by the Research Association of Composition and Music Theory Students at Karol Lipiński Academy in Wrocław presided by its Board composed of: Mateusz Winsław, Karolina Pawlik, and Ignacy Wojciechowski. 5. The Conference will involve the academic part in the form of papers and artistic part in the form of a concert. 6. The Conference will be held on April 2018 at the Karol Lipiński Academy of Music in Wroclaw, pl. Jana Pawła II No Fees and expenses related to the participation in the Conference shall be covered by the participants or delegating institutions. 8. Active participation involves: participation in all Conference events, the participant s paper (time for presentation: maximum 20 minutes), conference materials, certificate of active participation in the Conference, joint dinner, and treats during coffee breaks. 9. Passive participation involves participation in conference panels as part of the audience, conference materials, certificate of passive participation and treats during coffee breaks. 10. In order to apply for Participation in the Conference, a candidate shall complete the application Form available at send its scan to: ambiguity2018@amuz.wroc.pl and enclose a bank transfer receipt. The original completed Form (the paper version) shall be delivered to the Organisers no later than on the day of the Conference. The participant s application Form is enclosed hereto. The important part of the application Form includes the abstract of the proposed paper. The positive rating of the abstract shall be the basis for qualification for the Conference. 11. Due to the international nature of the Conference, the prerequisites for active participation shall include preparation of a multimedia presentation in English. 12. Applications for participation in the Conference shall be submitted by:

Persons applying for active participation shall be informed by the Organiser of the Conference Agenda by 26 March The Conference fee for active participants shall amount to: PLN 50 for participants from Poland, EUR 13 for foreign participants. 16. Passive participation is free of charge. 17. Fees for active participation in the Conference shall be paid to the bank account of the Karol Lipiński Academy of Music in Wrocław between 10 March 2018 and 30 March Bank transfer data: Account No , Bank Zachodni WBK, transfer description shall consist of: participant s name and surame, Konferencja Wieloznaczność Foreign transfer data: Account No./IBAN: PL SWIFT: WBKPPLPP 18. Should a participant relinquish active participation in the Conference after 30 March 2018, the conference fee shall not be reimbursed. 19. Upon the submission, candidates for participants in the Conference consent to: 1) photographs including their image being taken during the Conference, recorded, used and re-used, processed and copied using any medium for promotion and archiving by the Organiser without need for consent on a case-by-case basis, 2) their personal data being processed by the Organiser of the Conference pursuant to the Personal Data Protection Act of 29 August 1997 (Dz. U. 1997, item 922) as required for participation in the Conference, the participants also confirm that they have been informed that: a) data controller of data included in this application is the Karol Lipiński Academy of Music in Wrocław, pl. Jana Pawła II No. 2, Wrocław,

3 b) personal data provided by the participant shall be processed for purposes related to the organisation of the Conference and shall not be made available to entities other than the Organiser, c) the participants have the right to access and amend their personal data, d) the personal data is provided on a voluntary basis, but refusal to provide it shall prevent the candidate from applying for participation in the Conference. 20. The Organiser reserves the right to amend these Terms and Conditions. Any amendments to these Terms and Conditions shall be published on the website of the Karol Lipiński Academy of Music and The Organiser reserves the right to cancel the Conference for important reasons, should this be done, the fees paid by the participants shall be reimbursed.

I consent to use of audiovisual recordings including my image as a participant during the Conference by the Organiser of the Conference free of charge in the following fields: (a) in regard to recording and reproduction digital recording of sound and image, production and publication in the form of a CD and/or DVD or in other media exclusively for non-commercial purposes, (b) in regard to circulation of copies including the recorded image marketing, lending or hiring of the original or copies, (c) in regard to dissemination in way different from ways referred to in (b) broadcasting, rebroadcasting and performance, and provision of the image to the public on websites. I consent to my personal data being processed by the Organiser of the Conference pursuant to the Personal Data Protection Act of 29 August 1997 (Dz.
