The manufacturer has indicated that, under unusual, adverse certain conditions, the original plastic access cover provided on these units can melt or ignite. Bock Water Heaters, Inc. has informed IAS of a field correction program for their model 100G-180 series water heaters manufactured between March 1, 1995 and July 18, 1995, and bearing serial numbers T through T. Some of these water heaters may have been constructed with improperly fabricated flueway fins, resulting in back-drafting at the burner access door and the production of excessive amounts of carbon monoxide. If any of these water heaters are encountered, you are requested to put them out of service and contact Bock Water Heaters, Inc., 110 South Dickinson Street, P.0. Box 8632, Madison, Wisconsin , Telephone CARRIER CORPORATION CARRIER CORPORATION has informed IAS of a potential hazard involving their 40 Inch, multi-poise direct vent furnaces. Carrier Corporation has determined that this hazard exists only on units installed in what is termed a horizontal right configuration. The problem involves leakage of condensate from the furnace inducer motor housing into the control box, which can result in an electrical short. Sparks from a short can cause igniting of plastic parts in the surrounding area. 3 CARRIER CORPORATION CARRIER CORPORATION has informed IAS of a potential hazard involving their 40 Inch, multi-poise, direct vent furnaces. Carrier Corporation has determined that a hazard may exist when a blower relay solder joint on the replacement control board (HK42FZO08) overheats. The overheat condition can then ignite the control board and attached wiring harness. The furnaces involved, include only those that: have had a control board replacement (HK42FZ008) after February 1, 1995: and the control is located in the vestibule section of the furnace; and the serial numbers are 2993A A The affected furnaces will display one of the following trade names with the indicated model number designation: Trade Name: Model No. Carrier 58MCA 58MXA Bryant. Some of these modified heaters bear the A.G.A. Certification Seal and the Paloma PH-24 model number, but the designs have not been certified by IAS. The Paloma Model PH-24 tankless water heater design has been granted A.G.A. certification. This applies only to the modified designs distributed by ENERSOL. The uncertified designs may also be identified by water flow rates exceeding 5 gallon/minute and the temperature adjustment locked in the 1owest position with the temperature adjustment knob removed. A program has been initiated to remove all reference to A.G.A. certification and to model number PH-24 from these uncertified designs. Box 261, Remus, Michigan 49340, Telephone: (517) GLOWCORE CORPORATION GLOWCORE CORPORATION has advised IAS of a retrofit kit program designed to improve the safe operation of Model Nos. UFO6OD14R and UFO8OD14R gas furnaces manufactured between 1984 and The program consists of installing a low water level s*011 in the water expansion tank in addition to the water pressure switch now provided. 4 KIDDE-FENWAL, INC. KIDDE-FENWAL INC., has informed IAS of a potential problem involving Hot Surface Gas Ignition controls series 05-31, 05-32, 05-33, bearing the date code 9235 through The controls employ a potentially defective capacitor. In the event that the capacitor is defective and there is a simultaneous or subsequent failure of the ignition source, there is a risk that the gas valve could remain open. This condition may allow gas to flow into the appliances and the surrounding area, causing an explosive environment. In addition, Kidde-Fenwal continues to investigate claims that Model ignition devices that were installed on boilers manufactured by Raypak, Inc. have malfunctioned. Kidde-Fenwal now believes any problems with the ignition module in the Raypak boiler to be limited to a single production run in February 1991, of this specific module, which was manufactured exclusively for Raypak. Kidde-Fenwal is therefore, in addition to the above recall period, recalling all units of the series ignition modules that were manufactured in the first quarter of 1991, date codes 9101 through 9113, installed on Raypak residential boilers. The manufacturer has initiated a field correction program to locate and replace the suspect controls. Information in possession of International Approval Services (IAS) indicates that an unknown number of deep fat fryers have been produced by Mercury Equipment Co., Inc. displaying an unauthorized model number and unauthorized construction in conjunction with the American Gas Association (A.G.A.) seal. IAS has requested that the company locate all such models and remove the A.G.A. seal. The model number is MDF2020. Pitco Frialator, Inc. has informed IAS of a field correction program for models 14C+ and 35C+ deep fat fryers manufactured between March 17, 1994 and August 18, 1994 and bearing serial numbers G94CBO565O to G94HB These fryers were equipped with misprinted rating plates, which did not include the required installation clearance of six inches to the left and right sides of the fryer. If these fryers are installed with side and/or rear clearances to adjacent combustible construction of less than six inches, a fire may result. If any of these fryers without the six-inch side clearance markings are encountered, you are requested to (1) ascertain that the fryers are presently installed with at least six inches of clearance to adjacent combustible construction at the sides and back of the unit, and (2) contact Pitco Frialator, Inc. for an adhesive overlay with the correct installation markings for the rating plate. Although some of these fryers may presently be installed with adequate clearances, it is important that the rating plates be corrected to help prevent improper installation in the future. 5 RAM ELECTRONICS CORPORATION RAM Electronics Corporation has notified the International Approval Services that 3,205 model number CDS- 2 ignition controls were produced from September 1996 through December 1997 that incorrectly displayed the American Gas Association certification symbol. The design of the number CDS-2 was not tested by the International Approval Services and therefore production models are not eligible to display the American Gas Association certification symbol. The AGA certification symbol was inadvertently displayed on the number CDS-2 ignition controls produced from September 1996 through December RAM Electronics sent out replacement labels to all customers who purchased the number CDS-2 ignition controls during this period. If any of the number CDS-2 ignition controls produced during this period are located, contact RAM Electronics Corporation at 1800 East Prospect Road, Fort Collins, Colorado (Telephone: ) SERVISOFT OF MIDDLEFIELD, INC. IAS has learned that SERVISOFT OF MIDDLEFIELD, INC. has distributed booster water heaters, under the trade name "SOFTHEAT," which are a modification of a Paloma Model No. PH-24 tankless water heater. Some of these modified heaters bear the A.G.A. Certification Seal and the PH-24 model number, but the design has not been certified by IAS. The Paloma Model PH-24 tankless water heater design has been granted A.G.A. certification. This applies only to the modified design distributed by Servisoft of Middlefield, Inc. The uncertified designs may also be identified by water flow rates exceeding 5 gallons/minute and the temperature adjustment locked in the lowest position with the temperature adjustment knob removed. A program has been initiated to remove all references to A.G.A. certification and to model PH-24 from these uncertified designs. 6 SQUARE D COMPANY SQUARE D COMPANY, has informed IAS that certain high efficiency gas furnaces distributed by Square D under the SUN DIAL name were produced by the Magic Chef Air Conditioning Co., and are subject to the same Inspection and Service Program presently being offered for the Magic Chef "Ultra 87" furnaces by Armstrong Air Conditioning Inc. The SUN DIAL gas furnaces were distributed between 1983 and 1988, and the nameplate on each furnace subject to this notice has a catalog number beginning with the letters HU or HC, followed by a series of numbers and letters. SUN DIAL gas furnaces with catalog numbers that do not begin with the letters HU and HC are not subject to this Inspection and Service Program. The venting specified for use with these furnaces may corrode and/or cause the heat exchanger to corrode, allowing the escape of furnace gases into living space. This may cause personal injury to the occupants. 7 WHITE-RODGERS DIVISION, EMERSON ELECTRIC COMPANY WHITE-RODGERS DIVISION, EMERSON ELECTRIC COMPANY has informed IAS of a field correction program for their models 37 and 37C series water heater controls manufactured between March 1994, and October 1994, and bearing date codes 9412 to These controls have the potential for the thermostatic valve to stick in a partially open position. This can cause improper burner operation, sooting of the water heater burner and combustion chamber and may lead to a fire hazard with regard to adjacent combustible materials. Suspect controls can be identified by a model number beginning with the number "37" on a white label attached to the right side of the control, in conjunction with a date code of 9412 through The date code is located on the upper left corner of the back of the control and may be observed by using a small mirror. Signs of a malfunctioning control include discoloration or black soot on the lower outside surfaces of the water heater, a pilot flame that keeps going out, and/or an unusual "burning" odor. If water heaters equipped with one of the above-noted controls are observed to exhibit any of these signs, you are requested to turn off the gas supply to the water heater until the problem is corrected.
