4 Safety Focus Areas FOG The qualified Rock Engineer Visit to the working places it is still a challenge The reports do not reach the relevant people that it suppose reach Reports by Rock Engineers are to technical to be interpreted by Employees (i.e. Roof bolt Operators and CM Operators or General Workers) The recommendations by Rock Engineers do get implemented or the are hardly implemented The training issued to Employees does not address some of the items covered in the COP Safety Declaration report should be in pictorial format for easy of understanding Early Examination Sub standard support
5 Blasting SAFETY FOCUS AREAS There is a poor control of Short Exploders The procedures are drafted without consulting explosive manufactures or suppliers especially in smaller opencast operations We still find people working in sections with blasting certificates not for that class of mine

11 In January 2017, Gauteng reported most cases, followed by West Rustenburg, Mpumalanga, Free State, Northern Cape, North West Klerksdorp, Limpopo and Western Cape regions. In January 2016, Free State reported most cases followed by Gauteng, North West Klerksdorp, North West Rustenburg, Northern Cape, KwaZulu-Natal, Limpopo, Mpumalanga and Western Cape regions. KwaZulu-Natal did not report any occupational diseases/illness in January 2017 and the Eastern Cape region did not report any occupational disease/illness during the same period for both years.

13 Pulmonary tuberculosis (PTB) Pulmonary tuberculosis (PTB) cases have increased by 26.7% (27%) when compared to the same period during the previous year. Silicosis Silicosis cases reported have increased by 15.6% (16%) when compared to the same period during the previous year. Noise-induced hearing loss (NIHL) Noise-induced hearing loss (NIHL) cases have increased by 227.7% when compared to the same period during the previous year

Continue to follow up on instructions issued to mines to ensure compliance. Continue to enforce early entry exam and support to achieve compliance. Take alternative actions in terms of ensuring compliance in terms of repeat transgressions.
