Technical Services, Tokio Marine Management (TMM) Inc. on December 3-4, This survey was conducted by Sergio M. Barrueto, P.E., Sr. Risk Engineer, TMM, on November 20-21, 2013 and focused on the following: occupancy, special hazards, fire pump testing, management programs, housekeeping, building construction, management attitude, construction and fire alarms, tour & discussing of existing recommendations. MAJOR CHANGES Major Changes Since Last Visit: A new updated fire alarm system is being planned and will be installed in TCSOB Bldg A. Plans will be submitted for review. A business continuity plan is in process at this time for the entire Sony Company. It is understood this could take from 3 to 8 years to complete. The project to replace all old-style fire sprinklers has been finalized. The through penetrations should be sealed at the raised floor in order to retain the gaseous extinguishing concentration. Combustible loading and housekeeping are very critical items in a computer room. The equipment is very susceptible to smoke contamination, heat, and water damage. 8 Page 8 of 9 c. All ceiling tiles in the drop ceiling should be installed where they are missing in order to retain the gaseous extinguishing concentration. d. COMPLETED: Housekeeping including the area below the raised floor should be maintained at all times. e. COMPLETED: Combustible loading within the room should be limited at all times, only one day operating material should be in the room at one time. f. It appears urethane foam is being utilized to seal openings in the ceiling. This is undesirable since the foam is an added combustible and when ignited creates a very toxic & corrosive smoke which can cause damage to the equipment within. Ensure fire resistant material is used. g. Plastic trash cans should not be stored or used in the computer room. Metal trash cans should be used equipped with self-extinguishing lids Sprinkler Protection Important Automatic sprinkler protection should be extended to the Revised following areas. a. Throughout the West End Trailers and platform under the trailers located directly across from the Overland Parking Structure and adjacent to the Scene Dock #5. UPDATE 11/21/13: The trailers will be removed by October b. COMPLETED: The room on the mezzanine, southwest corner of the Grip Department. The room was converted from an office area to a storage room containing a drop ceiling with no sprinkler protection. The area contains considerable combustible storage and a fire in this area could spread to the combustible loading on the exterior overtaxing the existing sprinkler protection. In lieu of sprinkler protection remove the drop ceiling which would expose this area to the existing sprinkler system at the ceiling. UPDATE 11/21/13: Sprinklers were added. Also, this area is scheduled to be rebuilt between April 2014 and February c. Sprinkler protection should be extended to cover the 20'X20' cool box located on the mezzanine area of the Stock Expendable Building. This room is used for temperature sensitive materials. UPDATE 11/21/13: Sprinklers will not be added to the cooler due to the water and fire damage estimated to be equal at an LE of $200, Tote storage in the Grip Dept Important Plastic totes containing storage are stacked to 15 ft. high. Revised The sprinkler protection is this building is not designed to handle this height of totes. Therefore, the totes should not be stored and lowered to a maximum height of 5 ft. UPDATE 11/21/13: The Grip Dept will be demolished and a new 4 story building will be built between April 2014 and February Mr. Scot Falkenstien is reviewing the design and fire protection systems. indicated that he does not have control over these areas, but he and his team make an effort to correct the problem since there is no dedicated personnel to manage the room. Mr. Falkenstien has requested a separate letter covering these areas for enforcement purposes. COMPLETED: The area mentioned in item (b) contains considerable combustible storage and a fire in this area could spread to the combustible loading on the exterior overtaxing the existing sprinkler protection. In lieu of sprinkler protection remove the drop ceiling which would expose this area to the existing sprinkler system above. CONTINUES: The area mentioned in item (a) is all wood frame construction and it also exposes the Scene Dock #5 which is of wood frame stucco construction and the building houses a hazardous operation containing the painting of background canvas. It is understood the trailers will be removed and replaced with a new structure in the near future and will be sprinkled. Since the trailers will be removed, this item is not being considered. REIVSED: The cool box obstructs the ceiling sprinklers. The cool box contains combustibles materials such as cardboard boxes with CDs and VHS tapes wrapped in plastic. This was discussed with Scot Falkenstien and he indicated there is a shortage of space. LE=$250,000 PHY CC=$50,000 LEC=$20,000 LE=$350,000 PHY CC=$10,000 LEC=$25, Fuel gauge on day tank for diesel pump in the Sony This deficiency was discussed with the LE=$0 HE

9 Page 9 of 9 Important Picture Plaza Bldg. Revised It was observed during the survey that the fuel gauge on the day tank for the diesel pump registered empty. This should be repaired immediately. UPDATE 11/21/13: A malfunction with the linkage to the gauge was determined to be the problem, however the tank is full. This is a 25 gallon diesel tank that is automatically supplied by a 500 gallon diesel tank located outside the building Metal Halide Lamps - High Intensity Discharge (HID) Important Lights New The Barbara Streisand Building seems to be provided with open fixture Metal Halide Lamps which are High Intensity Discharge (HID) lights. Plan management should investigate if they are in fact HID lights and if proven positive, management should implement a program to implement one of the following options: 1. Provide the fixtures with containment barriers, 2. Replace the lamps with shrouded bulbs, sometimes called shatter-proof bulbs, with an O -rating, in accordance with FMDS 5-21, Section , and tested in accordance with ANSI C standard, or 3. Replace the fixtures with fluorescent lights Electrical Room - Sony Picture Plaza Building Important The electrical room on the first floor of the Wolfgang New Puck kitchen in the Sony Picture Plaza Building should be improved by implementing the following: 1) The boxes located next to the electrical panels should be removed and a 3-foot clearance all around the panels should be maintained. 2) An additional fire sprinkler next to the entrance to the electrical room should be installed since the next sprinkler head is over-spaced. 3) 18-inch clearance from the top of all storage should be maintained for proper water penetration if the fire sprinkler system were to operate. building engineer on duty and he was aware CC= of the situation and indicated that it would be LEC= repaired immediately. He assured the tank was full of fuel and will be checked daily until the gage is repaired. There is an electric vertical turbine pump available as a backup. These lamps have a history of failure and are linked to igniting fires and causing a total loss to the Facility and injuries to personnel when the lamps burst and hot glass falls on top of the person, combustible materials, cardboard boxes and storage. Bulbs with an O -rating tested in accordance with ANSI C standard and developed by NEMA industry guidelines are approved for use in open fixtures and are designed to contain shattered particles. Based on the referenced standard and guidelines, these bulbs are expected to contain arc tube fragments in the case of a violent arc tube rupture. Fire protection and prevention would be improved by implementing the items mentioned. Maintaining the area around the electrical panels clear of combustible materials will reduce the possibility of a fire ignition due to an electrical short circuit. The maximum spacing for a fire sprinkler head to a wall is 7'-6" and the sprinkler noted is at a greater distance than 7'-6".
