The following information was relayed: Statement by Ferency: "I was standing at the salad bar and I saw the flames shoot up. I hurried out to the lobby area and pulled the alarm, (station pull)." Investigation: Abby Gold made the land line call to GSOC to inform them of the alarm at the Harry Cohn Building. GSOC toned it out and all units responded. Falkenstien, Arias and Parris responded to the Commissary and took fire extinguishers and put out the fire, located in the center of the Commissary, just behind the serving line, at the Wok Station. Vandenbree responded to the Harry Cohn Lobby and went up the northeast stairwell and notified GSOC that she was starting to conduct floor sweeps. Clay responded to the front of the Commissary to assist with crowd control. Puente and C. Magallanes responded to the west side of the Harry Cohn Building to also assist with crowd control. Falkenstien requested GSOC to notify CCFD to respond. L. Magallanes responded to the rear entered the kitchen where Arias and Parris were extinguishing the fire. The cause of the fire was hot cooking oil heating on the left side of the wok, and the hot water heating on the right side of the wok. Both were left unattended, kitchen employees went on break, and the hot oil splattered onto the wok, which caused a reaction which then sent boiling water back over onto the hot oil. The series of splattering reactions caused ignition of the hot oil and a large, hot flame ensued. This reaction triggered the fire sprinkler head overhead. Arias and Parris doused the flames with 2 ABC fire extinguishers. Arias advised Sam 1 to retrieve additional ABC fire extinguishers. Puente responded with the Parking fire extinguisher but the flames were already put out at approximately 1115 hours. Vandenbree reported floors 1, 2, and 3 were clear, but when she reached the 4th floor, there were people in a conference room. They told her they thought it was okay to go back into the building because the fire alarm had stopped. Vandenbree explained to them that there was an actual fire and only the fire units could give the order to return to the building and she directed them back out. Tubi and C. Magallanes assisted in making sure they cleared the building. CCFD Battalion Chief, Engine 1 and 3, ladder Company 2 as well as Rescue 1, arrived collectively on scene at approximately 1117 hours. 31 lot and then posted at Coffee Bean. C. Magallanes was posted at the rear entrance as well as Puente for traffic control on B and 4th Avenue, as well as making sure nobody entered the building not involved in the emergency. Reza assisted with crowd control from the evacuation and was posted in the Cohn Park Area. CCFD started with their assessments and conducted a clean sweep of all floors to ensure that there were no other occupants left in the building. All other trades were notified AC, Electric, Engineering buildings and grounds, and Janitorial and the clean up started to take place in all areas. CCFD started to ventilate the building with a high power fan and plumbing started pumping water out of the kitchen area. At 1114 hours, Falkenstien took over as the IC, (Incident Commander), and gave an update of the incident. He announced that a reset of the fire panel would take place once the smoke detectors were cleared and all smoke heads were replaced. Approximately 10 to 15 minutes later Parris attempted a reset but was unsuccessful. The manual pull station, that was at the time of the initial fire, was the cause of the panel not restoring. The Manual Pull station was reset as well as the alarm panel and all occupants were let back into the building. At approximately 1155 hours the all clear signal was given and all units were able to clear. CCFD, Fire and additional trades Plumbing AC and Electric remained on scene for investigative and precautionary purposes. Per Fire Guiterrez, five fire sprinkler heads were triggered and had to be replaced. The Commissary will remain closed until further notice. Sony Fire & Life Safety Narrative by Ray Arias, Senior Studio Fire Marshal (F-2): Incident Report: On 03/27/13, at approx 11:14:26 Hours, all available Fire, Security and Medical units responded to a reported kitchen fire, in the interior of the commissary s front line kitchen. The following was observed: Reporting (s) Statement: 03/27/13: At approximately 11:11:46 AM, Per Ashley, I was working in the rear kitchen when one of the cooks came running to me, screaming that there was a fire in one of the stoves in the front kitchen. I ran over and saw flames shooting up from one of the Woks. I walked to the stove and reached for the gas valve and shut it off. I was also going to try and douse the flames but suddenly I heard a bunch of popping above my head, I got scared and ran out of the kitchen. 03/27/13: At approximately 11:11:46 AM, Per Bruce Ferency, I was standing at the salad bar getting something to eat when I suddenly heard some popping noises coming from the kitchen; I looked up and saw flames coming from the kitchen area where they cook the Asian food. I then ran to the lobby and pulled the fire alarm, and then told the lobby security officer to call Dispatch and to tell them that there was a fire in the kitchen. 03/27/13: At approximately 11:12:30 AM, Per Abby Gold, I was at the console when suddenly Bruce Ferency came Local Print Date/Time: Page 30 of 37

32 running in, pulled the fire alarm and yelled at me to call Dispatch and to let them know that there was a fire in the kitchen. Event Chronology: Approx. 11:11:40 hrs: A smoke detector immediately adjacent to the front line kitchen of the commissary activates. Approx. 11:12:07 hrs: A fire alarm manual pull station in the main lobby (next to guard console) is activated. Approx. 11:12:55 hrs: A Water Flow Alarm is activated due to (2) sprinkler heads triggered. Approx. 11:23:15 hrs: A Duct Detector is triggered, indicating a possible fire in the front line kitchen s exhaust hood ducting system. Approx. 11:13:26 hrs: GSOC issues a radio tone-out, alerting all Fire, Security and Medical units to respond to the reported fire incident. Approx. 11:13:30 hrs: Fire 1, Fire 2, Fire 3 and Fire 4 confirm their response and proceed en route. Approx. 11:14:20 hrs: Fire 3 on scene at the rth interior rear kitchen area, and reports confirmed smoke in the front line kitchen, with light smoke moving rth. Approx. 11:14:26 hrs: Fire 1 and Fire 2 on scene at East exterior of main entrance to commissary and observe a considerable amount of fire and resulting heavy smoke overwhelming the kitchen exhaust hood, and quickly charging the ceiling perimeter, and also observe what appears as the activated kitchen hood suppression system. Approx. 11:14:26 hrs: Fire 1 confirms I.C. role and request CCFD response due to confirmed kitchen fire and potential for extension/spread. Approx. 11:14:32 hrs: Fire 4 on scene at exterior of main commissary lobby, also observes heavy smoke, and then advances to interior and positions with extinguisher in line of sight with Fire 2 and Fire 3. Approx. 11:14:40 hrs: GSOC advises Fire 1/I.C. that CCFD is responding. Approx. 11:14:48 hrs: Fire 1/I.C., after ascertaining that the smoke is confined to the first floor commissary area, orders security units to proceed into the upper floors of the building to check for occupants. Approx. 11:14:45 hrs: Sony Medical (Judy Drake and Karen Forsberg) advise they are on scene at command post. Approx. 11:14:56 hrs: Fire 2 vectors on scene with Fire 3 on the interior rth entrance to the front line kitchen. Approx. 11:14:58 hrs: Sony plumbing personnel on scene and on standby. Approx. 11:15:05 hrs: Decision is made to initiate fire attack, and Fire 2 and Fire 3 advance through the down-pour of the two activated fire sprinklers with portable ABC fire extinguishers. Fire 4 standing by as backup in proximity. Approx. 11:15:45 hrs: Fire 2 advises Fire 1/I.C. that the fire appears to be knocked down; and that Fire 3 will confirm fire is dead-out by using additional concentrations of ABC fire extinguishers. Approx. 11:16:26 hrs: Fire 2 confirms with Fire 1 I.C. that the fire is dead out (confirmed knock-down). Approx. 11:17:45 hrs: Fire 1/I.C. requests the shutdown of the Main Fire riser which feeds the activated sprinkler heads, thus minimizing water damage. Approx. 11:21:27 hrs: Main Fire Riser Valve is shut down. Approx. 11:21:00:45 hrs: CCFD Engine 1, Battalion Chief, Ladder Truck 3 and Rescue 1 on scene. Approx. 11:22:00 hrs: After conferring with Fire 1/I.C., CCFD initiated ventilation Operations to purge the commissary of smoke, and also begin their assessment of the entire building to confirm no fire extension to the building. Approx. 11:23:00 hrs: Fire 1/I.C., Fire 3 and CCFD ascended to the roof to assess the integrity of the kitchen exhaust system, and rule out fire extension. Approx. 11:25:02 hrs: Salvage/cleanup operations initiated by Sony facilities and janitorial staff. Approx. 11:32:59 hrs: Per Fire 1/I.C., CCFD still on scene ventilating the first floor of the building. Approx. 12:01:41 hrs: Per Fire 1/I.C., the Fire Alarm System was re-set successfully and the occupants are now permitted to return into the building. Approx. 12:02:03 hrs: Per Fire 1/I.C., Fire units and CCFD will remain on scene for incident investigation. n essential security units cleared to clear the incident/scene. Approx. 17:29:04 hrs: GSOC advised by Fire 1/I.C. that Fire Alarm and the Fire Sprinkler Systems are both back full online and operational. Commissary declared closed until at least Monday, April 1. 33 Initial Scene Size Up Upon arrival to the East exterior (Calley Park), a large group of building occupants were observed actively evacuating from the Harry Cohn building, to the designated evacuation area in the park. smoke yet visible at the exterior. Immediate size-up and visual of the interior confirmed heavy smoke overwhelming the kitchen hood system and a solid column of fire in and around the perimeter of the Wok station. Also observed is what appeared to be a single activated sprinkler head on the front area of the exhaust hood. Dark brown smoke quickly charging the surrounding ceiling area and spreading mainly in a rtherly direction. Initial Incident Investigation: Per witnesses in the vicinity of the Wok Station, the oil on the left side Wok was being pre-heated, as was the water on the right side Wok. Unfortunately, a general break was also called at that time and all of the kitchen crew/staff departed simultaneously, leaving the Woks operating, unattended with active burners. Source of the fire appears to be the Wok station. All preliminary signs suggest that the oil from the frying Wok became hot enough to then splatter into the Wok, which contained boiling water. The hot oil splatter then triggered a water response back into the hot oil Wok, which then erupted into a vicious cycle of fire and flame. The gas valve to the Wok burner was in the off position. The Ansul system fuse link closest to the Left Wok was activated (confirmed by link fragments found). The water faucet nearest the Left Wok was broken in half (steam build-up rupture). The fire stop ceramic tile nearest the Left Wok was shattered in many areas, exposing tile grout and some drywall. Several surfaces of the stainless steel hood and back splash are warped/deformed and stained from excessive heat impingement. flammable liquid/gas containers in the immediate area of fire or in the immediate exterior vicinity. unusual conditions observed (interior/exterior of structure). combustibles in the immediate area of fire. Several light fixtures within the exhaust hood system were destroyed by the fire. Immediate kitchen floor relatively clean and free of trash or debris. Kitchen Ansul system tanks found in cocked position, and not in the triggered mode. injuries reported at this time. Follow-Up incident Investigation: McCormick, Mike (CCFD Fire Inspector) Scot Falkenstien (Executive Director, Sony Fire) Arias, Ray (Sr. Sony Fire Marshal) Fire investigation continued on 03/28/13 to determine cause and origin. After careful investigation we believe the origin of the fire to be the Wok station super heated oil coming in contact with water, which produced a violent fire reaction. There are definite signs of where the fire started, as evidenced by the charring effect and the formed V pattern. The intensity and heat of the fire, though largely confined to the area of the Wok Station, was such that its progression is clearly indicated by increased depth of char and discoloration / destruction at the wall splash tile (approximately 1/8th of an inch at the point closest to the point of origin). Also there is metal deformation and discoloration to the stainless steel paneling and hood. Several tiles experienced complete burn through and fracturing, indicating the intensity and path of the flame spread. In addition, it was found that the Ansul system did not discharge, despite the one fuse link having melted/triggered. Local Print Date/Time: Page 32 of 37

35 Cause of Fire: At the Wok station, cooking oil was being pre-heated on the left side Wok, as water was also being pre-heated on the right side Wok. The entire kitchen crew went on break, and the Wok station was left unattended with the burners still fired. The hot oil in the left Wok eventually splattered onto the adjacent Wok, causing a violent reaction which then sent boiling water back over and onto the hot oil. Thus a series of violent splattering reactions caused the subsequent ignition of the hot oil and a very large and very hot flame ensued. Corrective Actions: Fire extinguisher training/retraining will be conducted for kitchen staff/crew. Ansul Kitchen Hood fire suppression system training will be conducted for kitchen staff/crew. Alarm and notification process training will be conducted.
