3 1. Unsafe Act, Unsafe Condition & Near miss What is Unsafe Act? Performance of a task or other activity that is conducted in a manner that may threaten the health and/or safety. Examples: Operating equipment without qualification or authorization Lack of/or improper use of PPE Operation equipment at unsafe speeds Bypass or removal of safety devices Using defective equipment Use of tools for other than their intended purpose Not following rules or procedures Unsafe Act: Walking under suspended load & no PPEs What is Unsafe Condition? A condition that is likely to cause injury or property damage. Examples: Defective tools, equipment or supplies Inadequate supports or guards Congestion in the workplace Fire and explosion hazards Poor housekeeping Hazardous atmospheric conditions Unsafe Condition: Rusty lifting gear

4 What is Near Miss? An event which could have resulted in injury or loss but did not in fact do so this time. All Near Misses involve people & some type of action. Examples: Tripping over loose wire but managed to get balanced Fall of material near persons while hoisting Slipping on wet floor but managed to get balanced Job fixed in Chuck jaw of lathe flew off when started Near miss: Suspended load falling near persons 2. Lessons Learned Division : Corporate Office Date of Incident : 03 rd Jul 15 Type of incident : LWDC Incident Time :10:45 Hours Location : Station A, QAIT Office, First Floor, Hall No. 1 Description of Incident: The employee s leg got tangled into the hanging keyboard cable of his Desktop PC while he started walking after getting up from his chair. Due to entanglement of cable, employee fell and hit the side corner of his worktable.

5 Root Cause Analysis PHYSICAL FACTORS : In attention to footing & surrounding area Inattention / lack of awareness HUMAN FACTORS: Inadequate workplace layout SYSTEM FACTORS : Inadequate audit / inspection / monitoring Recommendations: 1. Computer & other cables need to be properly clamped & rerouted in the footing area of worktables. 2. As far as possible CPU of Desktop PC to be kept on table & footing area to be kept clear. 3. Regular Safety Inspection of office area & reporting of unsafe conditions by Floor Safety Wardens.

Corporate Safety Highlights Conducted weekly audits Transmission (Borivali, Dharavi & Saki) Transmission Projects (245 kv GIS - Mahalaxmi & 145 kv GIS Versova) Trombay (Station-A 22kV GIS building work & U#4 chiller unit) Conducted Office safety inspection at Backbay, Powai, Dharavi, Carnac office. Mumbai Region Guest Houses (Electra, Hardik Heritage, Vashi) & Colonies (Ram tekadi, Vashi colony, Trombay Colony hostel building) Safety Inspections visits carried out with AOC representative Conducted CSM audit at CGPL SAP EHS module roll out was done Conducted SAP EHS training session at Hydros Supported MO Civil for reviewing of JSA for dismantling Conveyor belt (15A)

10 6. Security Highlights I. Trombay Training of permanent/mgb/solaris ESM guards carried out regularly on behaviour/technical aspects/isps /Drill etc in Trombay. Before & aftermath of hanging of a terrorist, Police took charge of Trombay gates. II. T&D Training of ESM/pvt guards carried out regularly on behaviour/technical aspects etc in T&D.

11 Mock drill on scenario of bomb threat conducted in coordination with the Police authorities and awareness session for emp also done at the end. III. Jojobera Mock drill on scenario of intrusion & use of incendiary by culprits. Finally, and apprehending the culprits. Training & Blood Donation by security Staff & families.
