Foothill Boulevard Azusa, California Subject: Statement of Qualifications to Provide Planning Consultant Services Dear Mr. Christiansen: Thank you for contacting me about the opportunity to provide staff support services to the City of Azusa. In May 2014, I became an independent contractor after 28 years of working for private consulting firms. I am a recognized leader in policy planning, CEQA and NEPA studies, and community involvement for both public agencies and private developers in California and Arizona. I have considerable project management experience on a diverse range of policy, environmental, and private development projects, many of which were controversial and/or high-profile projects, during my 28 years of working for private consulting firms. My projectrelated responsibilities include analysis, technical review, and management of environmental documents for CEQA compliance, staff support for public agencies, and assistance to private sector clients in meeting governmental agency requirements. A summary of services Morse Planning Group can provide are summarized below. 3 A QUALIFICATIONS AND RELEVANT EXPERIENCE Morse Planning Group is pleased to submit this Statement of Qualifications (SOQ) to the City of Azusa. This brief submittal emphasizes my ability to provide comprehensive services, client satisfaction, and on-call services background with multiple jurisdictions. A1 Key Staff Involvement Ms. Collette Morse, AICP, will serve as the Project Manager and primary contact with City Staff. A resume is provided in Section C. A2 On-Call Planning and Environmental Services Over past 20+ years, I have worked with the following cities to provide on-call planning and environmental services while employed with several private consulting firms. City of Duarte, CA City of Cypress, CA City of El Segundo, CA City of San Gabriel, CA City of Santa Clarita, CA A3 Recent Project Experience The following is a summary of recent project experience on planning and environmental projects. Duarte Station Specific Plan and EIR Duarte, CA While with another firm, Ms. Morse served as Project Manager for the project team, which included RBF Consulting, Dahlin Group, and EPS. The team prepared the City of Duarte s first transit-oriented development specific plan to provide additional development opportunities for 19 acres adjacent to the Duarte Gold Line Station, anticipated to open in 2015/2016. The Duarte Station Area will become a vibrant, mixed-use transit village that has a focus on residential uses, office, hospitality, and urban green space. The development standards and guidelines of the specific plan focus on defining the scale and mass of the urban form, while allowing land use flexibility for ever-changing market conditions. The EIR prepared by RBF provides environmental clearance for the identified development scenario with appropriate aesthetic and traffic mitigation to ensure that the adjacent residential neighborhood is not significantly impacted. The project included technical advisory committee meetings, and updates to the fire hazards and seismic sections of the plan. Contact: City of Duarte 1600 Huntington Drive, Duarte, CA Irma Hernandez, Senior Planner Gothard Street Industrial Project MND Huntington Beach, CA While with another firm, Ms. Morse served as Project Manager for this project. The project applicant will demolish the entire site (structures and parking lot areas) of the former Randall Lumber site and subdivide the 6.5-acre site into two numbered lots and one lettered lot for the construction of two two-story industrial buildings (one on each numbered lot) totaling 142,300 square feet and associated parking. Lot 1 will consist of 2.11 acres and be developed with a 93,100 square foot building (Building 1). Lot 2 will consist of 1.12 acres and be developed with 49,200 square foot building (Building 2). Lot A will consist of 3.30 acres and be developed with 241 parking spaces and required landscaping areas. The project involved redevelopment of a 1.09-acre site with a mixed use commercial, residential, and live/work units project. In total, 9,300 square feet of retail and restaurant uses, 18 live-work units ranging in size from 880 to 1,200 square feet, 15 residential units ranging in size from 1,290 to 1,550 square feet, common area for residential uses, and surface and structured (below and above-grade) parking in two buildings totaling 56,400 square feet of building area and a floor area ratio of 1.8. 5 Contact: Larissa De La Cruz, Associate Planner Addendum to Valley Vision: Valley Boulevard Neighborhoods Sustainability Plan Final Environmental Impact Report San Gabriel, CA While with another firm, Ms. Morse served as Project Manager for this project. The work effort included the preparation of an Addendum to the 2006 Final EIR to review a number of amendments to the Specific Plan. These amendments were intended to clarify or modify language, delete outdated references, and include standards for uses not previously allowed in the Plan, including but not limited to: adding banks as a permitted retail use; providing development standards for hotels, which continue to require a conditional use permit for development; providing for live/work units at ground level of mixed-use projects and development standards for such uses, and specifying that parking for certain uses may be provided by valet parking. Her involvement on the project is continuing. Working for the Shopoff Group, Ms. Morse and the RBF Consulting Team are preparing an amendment to the El Corazon Specific Plan to allow stand-along multi-family development in a mixed use area, along with providing engineering support and technical studies. In addition, an MND will be prepared to address the 80 multi-family units and the specific plan amendment. Technical studies to support the MND include traffic, air quality, greenhouse gas emissions, noise, and water quality. Her involvement on the project is continuing. The project applicant is requesting an amendment to the 2006 Specific Plan, which includes a revised land use concept and revised land use summary. Compared to the 2006 Specific Plan, the project modification results in the same amount of commercial square footage (140,000) and the same number of multi-family residential units (260). 5

6 Contact: Shea Properties 130 Vantis, Suite 200, Aliso Viejo, CA Elizabeth Cobb, Vice President of Development Vantis Specific Plan Amendment Addendum Aliso Viejo, CA While with another firm, Ms. Morse served as Project Manager for the CEQA Addendum portion of the project. Working for Shea Properties, Ms. Morse prepared the Addendum for the second amendment to the Vantis Specific Plan. The requested General Plan, Specific Plan, and Development Agreement Amendments are proposed for the remaining undeveloped portion of the Vantis Specific Plan area, which constitutes approximately 10.9 acres and encompasses Lots 4 through 10 of Tract The amendment proposes to adjust the boundaries for the High Density Residential and Professional Office Districts, as well as add hotel as a use within the Professional Office District, and additional residential units within the High Density Residential District. In addition, two site development permits were requested for Lots 4 through 10. The project included a citywide Transit Oriented Development (TOD) Framework for the City of Stanton, and a Specific Plan and environmental clearance for a TOD site at the intersection of Beach Boulevard and the Southern Pacific Railroad right-of-way (an OCTA site). The third phase of the project included the CEQA analysis of traffic, noise, air quality; Initial Study and Mitigated Negative Declaration; public workshops and public hearings; Notice of Completion, mitigation monitoring and reporting program. Contact: City of Stanton 7800 Katella Avenue, Stanton, CA Kelly Hart, Associate Planner Mancara at Robinson Ranch Residential Project EIR Santa Clarita, CA While with another firm, Ms. Morse served as Project Manager for the EIR. Her involvement on the project began in 2006 and is continuing. The proposed project is a rural residential equestrian-based community involving the development of 99 single-family residential units and open space areas within 105 lots on approximately 172 gross acres of land. The project site is 6

7 located at the southeast corner of the intersection of Oak Spring Canyon Road and Lost Canyon Road, and is roughly bounded by the Santa Clara River to the north, Oak Spring Canyon Road to the west, the Robinson Ranch Golf Club to the south, and the Angeles National Forest to the east. The EIR is analyzing a number of topics, including aesthetics, light, and glare; biological resources; traffic and circulation; air quality; noise; hydrology and water quality; geology; and public services and utilities. The range of services and project types are illustrative of the services I can provide to the City of Azusa. In addition to the projects listed below, I prepared the City s General Plan and General Plan EIR. Recent planning and environmental documents I have managed for the City of Cypress include the following: Mackay Place Specific Plan and EIR (2014) Infill development on 9.7 acres consisting of 47 single-family detached units and a 2.9-acre City park site. Harmony 2015 Specific Plan and Mitigated Negative Declaration/Initial Study (2014) Infill development on 6.3 acres with 29 single-family detached units. 33-Acre Warehouse and Light Manufacturing Center EIR (2013) Proposed development of approximately 175,000 square feet of warehousing and light manufacturing uses in three buildings located within the Amended and Restated Cypress Business and Professional Center Specific Plan area. Cypress Park Specific Plan and Mitigated Negative Declaration/Initial Study - Infill development project consisting of 28 single-family detached units. CenterStone Specific Plan and Mitigated Negative Declaration/Initial Study - Infill development project consisting of 33 single-family detached homes. Wicker Drive Specific Plan and Mitigated Negative Declaration/Initial Study - Infill development consisting of 40 single-family detached homes adjacent to the Civic Center. Grindlay/Orange Specific Plan and Mitigated Negative Declaration: Conversion of office building to 17 single-family homes. Infill development across from Civic Center. Nevis Homes Mitigated Negative Declaration/Initial Study - Infill development consisting of 63 condominium units along Lincoln Avenue. 7

El Pollo Loco Mitigated Negative Declaration/Initial Study - 2,515 square foot drive-thru restaurant. Southern California Water Company - West Orangewood Water Well Project - Mitigated Negative Declaration/Initial Study - 1,000-foot deep water well and pumping facility. I have been preparing environmental documents for individual development projects within the 587-acre Cypress Business Park, which encompasses six specific plan areas: Cypress Business and Professional Center, Cypress Corporate Center, cypress View Limited, McDonnell Center, Warland/Cypress Business Center, and the Lusk Company Industrial Park. I prepared Mitigated Negative Declarations and Initial Studies/Environmental Checklists (IS/EC) for development within these Specific Plan areas, including: Shurflo Mitigated Negative Declaration/Initial Study - New campus facility for Shurflo consisting of a 50,000 square foot office building, a 91,618 square foot R&D building and a 100,000 square foot warehouse building. Project site is located within the Warland/Cypress Business Center Specific Plan area. Ushio America Mitigated Negative Declaration/Initial Study - A two-story, 72,417 square foot office/warehouse building. Project site is located within the Cypress Business and Professional Center Specific Plan area. Warland Investments - Speculative Office/R&D Building Chip Avenue - Mitigated Negative Declaration/Initial Study - 50,000 square foot office/r&d building. Project site is located within the Warland/Cypress Business Center Specific Plan area. Vertex Standard - Office/Warehousing Building Walker Street - Mitigated Negative Declaration/Initial Study - 95,800 square foot office/warehouse building. Speculative R&D Building - Parcel 7 - Mitigated Negative Declaration/Initial Study - 62,700 square foot office, warehouse and R&D building.
