2 SECTION ADDENDUM #4 ( ) RE: Security Camera Upgrades at Multiple School Sites FROM: OWNER: FULTON COUNTY BOARD OF EDUCATION CAPITAL PROGRAM CONTRACTS 6201 Power Ferry Road NW Sandy Spring, GA TO: ALL PROSPECTIVE OFFERORS: This Addendum forms a part of the Contract Documents and modifies the Proposal Documents dated September 21, 2017, as noted below. Acknowledge receipt of the Addendum in the space provided on Document Proposal Acceptance Form. Failure to do so may result in the proposal being deemed non-responsive. The Addendum consists of 20 written pages and the documents cited below. A. CHANGES TO PROPOSAL REQUIREMENTS TABLE OF CONTENTS None B. CHANGES TO PRIOR ADDENDUM: None C. CHANGES TO PROPOSAL DOCUMENTS: 1. See revised Document 00400, dated 11/17/2017. This section has been revised to remove all Alternates. D. CHANGES TO CONDITIONS OF THE CONTRACT: None E. CHANGES TO SPECIFICATIONS: See Attachment 1 Changes to Drawings and Specifications F. CHANGES TO DRAWINGS: See Attachment 1 Changes to Drawings and Specifications G. OTHER CHANGES AS SET FORTH: G-1 QUESTIONS: 1. Question: How do Offerors obtain a copy of the revised drawings and specifications cited in this addendum? Answer: The revised drawings and specifications can be obtained from J&A Engineering. 2. Question: Z indicates alternative monitoring viewing location, and Security contractor to ensure connectivity. Is there current connectivity to this location? Is connectivity via fiber or copper? Will additional components be required to connect this location such as media converters, network switches, patch panels or cables? Answer: The Security Contractor is responsible for ensuring the connection of the local Network Video Recorder to the OWNER PROVIDED network (all cable and terminations to owner provided patch panel or switch) as well as all coordination with FCS IT Support to ensure that the NVR is connected to a active network port. Then coordinating a visit, with Paul Hildreth / FCS

3 Safety and Security, to the Alternative Monitoring and Viewing location (Security Command Center in the Administration Center located at 6201 Powers Ferry Road, NW, Atlanta GA) and providing all required programming and configuration so that all the North Fulton Center cameras are capable of being viewed / controlled from the Alternative Monitoring and Viewing location. 3. Question: B- Video Equipment Cabinets. Are all cabinets and racks to only be included as the add alternate and not part of the base bid proposal? Answer: Yes except where noted on the drawings at Alpharetta HS. There are REMOTE SWITCH LOCATIONS that require wall mounted racks / cabinets. 4. Question: E- PoE Inserters must be installed in wall mounted, lockable, metal enclosures with ventilation. The preference is for the PoE Inserters and the associated enclosures to be mounted in the MDF Room or the closest IDF Room. Will UPS be required for all of the PoE Inserters? Answer: Yes. On past projects, the Contractors have mounted the PoE Inserters and enclosure in close proximity to the UPS Unit that powers the Network Video Recorder. It would be acceptable to pull power for the PoE Inserters, from the UPS Unit powering the Network Video Recorders. 5. Question: Are UPS required for customer provided network equipment? Answer: The Security Contractor selected for this project shall not be required to provide UPS Units for OWNER / CUSTOMER PROVIDED network equipment. 6. Question: What is the format of the training DVDs? Answer: The Training DVD should be provided in one of the following formats: MP4, AVI, FLV or WMV. 7. Question: V, W, and X Indicates Wall mounts for monitors and PC workstations shall incorporate Small Form Factor PC and UPS unit. Does Fulton County Schools have a current standard model mount to incorporate both the UPS, PC, and monitor? Please provide model number. Answer: The Fulton County Schools does not have a standard for the mount. Clarification or any other notice of a change in the Proposal Documents will be issued only by the OWNER Contracts Department and only in the form of a written Addendum, transmitted by fax or to all who are known by the issuing office to have received a complete set of PROPOSAL Documents. Any other purported Addenda are void and unenforceable.

Offerors shall keep the Proposal Acceptance Form intact and return all pages when submitting the PROPOSAL. B. Failure to submit the complete Proposal Acceptance Form may invalidate the PROPOSAL PROPOSAL DUE DATE: By: 2:30 p.m. on Thursday November 30, PROJECT IDENTIFICATION: A. The undersigned is familiar with the terms of the Contract, the local conditions affecting performance of Contract, the cost of the Work at the place where the Work is to be done, and with the Drawings, Specifications and all other PROPOSAL Documents. Allowance for the supply and installation of additional Indoor Fixed Recessed Ceiling Mounted Dome, Wide Dynamic Range, 2 or 3 Megapixel Cameras with analytics. The camera shall be the Avigilon model 2.0C-H4A-DC1-IR or 3.0C-H4A-DC1-IR vandal resistant dome style camera with built-in infrared illuminator, variable focal 3 9 mm lens, Avigilon video channel license, 250 feet of CAT 6 cable, labor to install, configuration and programming of all analytics. 6. Allowance for the supply and installation of additional Indoor Fixed Dome, Wide Dynamic Range, 5 Megapixel Camera with analytics. The camera shall be the Avigilon model 5.0L- H4A-D1 vandal resistant dome style camera with variable focal lens, Avigilon video channel license, 250 feet of CAT 6 cable, labor to install, configuration and programming all analytics 7. Allowance for the supply and installation of additional Indoor Fixed Legacy Style, 8 Megapixel Cameras with analytics. The camera shall be the Avigilon model 8L-H4Pro-B camera housed in an Avigilon ES-HD CWS-LG legacy style camera enclosure and equipped with a Sigma LEFS183518SI (18-35 mm) variable focal lenses. The camera shall be the Avigilon model 9W-H3-3MH- DC1 dome style camera with three 3 megapixel imagers and mm variable focal lenses on each imager. The Avigilon video channel license, 250 feet of CAT 6 cable, labor to install, configuration and programming must also be included 9. Allowance for the supply and installation of additional Indoor Fixed Dome, Multi-Sensor, 12 Megapixel Cameras (no analytics available at this time). The camera shall be the Avigilon model 12W-H3-3MH- DC1 dome style camera with four 4 megapixel imagers and mm variable focal lenses on each imager. The Avigilon video channel license, 250 feet of CAT 6 cable, labor to install, configuration and programming must also be included 10. Allowance for the supply and installation of additional Indoor Fixed Dome, 360 Degree Panoramic, 12 Megapixel Cameras (no analytics available at this time). The camera shall be the Avigilon model 12.0L- H4F-DO1-IR dome style camera. The Avigilon video channel license, 250 feet of CAT 6 cable, labor to install, configuration and all programming must be included 11. Allowance for supply and installation of one (1) Indoor / Outdoor Pan-Tilt-Zoom Camera with analytics. The camera shall be the Avigilon model 2.0W- H4PTZ-DP30 dome style P/T/Z camera. If utilized outdoors, the camera must be equipped with heater and blower. 8 pendant mounted hardware. Also all cameras in this category, shall be equipped with motorized zoom lenses and provide 30 X optical zoom. Article 1 - Scope of Work The CONTRACTOR shall perform, within the time stipulated in the Contract Documents, all of which are incorporated herein and shall provide all labor, materials, equipment, tools, utility services, transportation and everything else necessary to complete in a workmanlike manner, and in exact compliance with the terms of the Contract Documents, all of the Work required in connection with the following titled Project: Security Camera Upgrades at Multiple School Sites RFP [Proposal Number/Project Number(s) to be filled in by Capital Program Contracts] Article 2 - Time for Completion The Work shall be commenced on the date stated in the OWNER Notice to Proceed. The time period for Substantial Completion of the Work shall be set forth in the Notice to Proceed issued by the OWNER and Exhibit A Milestone Schedule document. TIME IS OF THE ESSENCE. 14 Article 3 - Hold Harmless, Defense and Indemnification The CONTRACTOR, even if it is without fault itself, shall indemnify, defend and hold harmless the OWNER, the Board and its and their respective officers, employees, program administrators, representatives, agents and consultants, from every liability, claim, loss, cause of action, action, demand, penalty, cost, expense (including without limitation, attorneys fees) related to or arising from: 1. Any injury to person or property sustained by the CONTRACTOR or by any person, firm, or corporation, employed directly or indirectly by it upon or in connection with the Work; 2. Any injury to person or property sustained by any person, firm, or corporation, caused by any act, neglect, default, or omission of the CONTRACTOR or any person, firm, or corporation, directly or indirectly employed by it upon or in connection with the Work, whether the injury or damage occurs upon or adjacent to the Work; 3. The furnishing or use of any copyrighted or uncopyrighted composition, secret process, patented or unpatented invention, article, or appliance under the Contract Documents; and 4. As otherwise provided in the Contract Documents. The CONTRACTOR at its own cost, expense, and risk, shall defend all legal proceedings that may be brought against all such potential indemnities for any such liability, claim, loss, cause of action, action, demand, penalty, cost and expense, and satisfy any resulting judgment that may be rendered against any of them whether or not the liability, claim, loss, cause of action, action, demand, penalty, cost and expense (including without limitation, attorneys fees) was actually or allegedly caused wholly or in part through the negligence or other tortious conduct of any of them. OWNER shall have the right to approve counsel proposed for any such defense and shall be consulted with regard to any proposed settlement. This Article 3 is not meant to require the CONTRACTOR to defend, indemnify or hold harmless the potential indemnities from their own active negligence. Article 4 - Insurance CONTRACTOR will provide its own insurance coverage as to all types of insurance relevant to the Project in amounts of coverage and by carriers approved by the OWNER. Article 5 - Bonding If the amount of original award of the Contract exceeds FIFTY THOUSAND DOLLARS ($50,000.00), the CONTRACTOR shall furnish to the OWNER a Payment Bond (Material and Labor). CONTRACTOR shall also provide a Performance Bond. Both Bonds shall be for 100% of the Contract Amount and contain the terms and conditions required by Articles 5.10 and 5.11 of the General Conditions. 15 Article 6 - Provisions Required by Law Deemed Inserted Each and every provision of law and clause required by law to be inserted in the Contract Documents shall be deemed to be inserted and the Contract Documents shall be read and enforced as though it were included in the Contract Documents. If through mistake or otherwise any such provision is not inserted, or is not correctly inserted, upon application of either party the Contract Documents shall forthwith be physically amended to make such insertion or correction. 16 Article 7 - Contract Amount The OWNER shall pay, and the CONTRACTOR shall accept, in full payment for performance as required by the Contract Documents, the sum of (To be filled in by OWNER / Capital Program Contracts) ($ ), subject to any additions or deductions, if any, as provided in the Contract Documents. It is understood and agreed that all applicable taxes are included in the Contract Amount and that the Federal Excise Tax, from which the OWNER is exempt, is not included. The OWNER, upon request, will furnish the CONTRACTOR such Tax Exemption Certificates as may be required by the Manufacturer or Dealer. All of the above-named Contract Documents are intended to be complementary. Work required by one of the abovenamed Contract Documents and not by others shall be done as if required by all. DRAWING LV The P/T/Z Camera Mounting Detail on Drawing LV has been revised to require Drip Loop with liquid tight conduit. DRAWING LV Camera model number on Drawing LV has been revised. The camera is to be wall mounted. DRAWING LV , Key Notes #1, #2 and #3 Key Note #1 has been modified to read "Contractor to utilize existing 120 VAC / 30 AMP power circuit." Key Note #2 has been completely deleted. Key Note #3 has been modified to read "Security Contractor to provide and install two (2) shelves in existing two (2) post rack. 18 SECTION , PAGE 3, PARAGRAPH 1.2, ITEM K. As an ADD ALTERNATE, the Security Contractor shall provide and install one (1) 120VAC / 30 amp EMERGENCY POWER circuit. The circuit shall originate at the existing 120 /208 Emergency Power Panel and terminate into a locking, wall mounted receptacle within 6 feet of the rack in which the NVR(s) are mounted. SECTION , PAGE 3, PARAGRAPH 1.2, ITEM K has been deleted. SECTION , PAGE 20, PARAGRAPH 2.12 FULTON COUNTY SCHOOLS COPPER WIRING REQUIREMENTS, ITEM B. 2. "Cable must be manufactured by CommScope." SECTION , PAGE 6, PARAGRAPH 1.2, ITEM EE has been modified and is now ITEM CC.
