Impacts resulting from spills/releases shall be investigated and cleaned up as soon as practicable. The Director may require additional activities to prevent or mitigate threatened or actual significant adverse environmental impacts on any air, water, soil or biological resource, or to the extent necessary to ensure compliance with the concentration levels in Table 910-1, with consideration to WQCC ground water standards and classifications. 9

9 Pipeline COLORADO OIL & GAS CONSERVATION COMMISSION COGCC REPORTING REQUIREMENTS Spills/releases of E&P waste or produced fluid exceeding five (5) barrels, 1 including those contained within lined or unlined berms, shall be reported on COGCC Spill/Release Report, Form 19. (SEE PAGE 2-9) Spills/releases which exceed twenty (20) barrels of an E&P waste shall be reported on COGCC Spill/Release Report, Form 19, and shall also be verbally 2 reported to the Director as soon as practicable, but not more than twenty-four (24) hours after discovery. Spills/releases of any size which impact or threaten to impact any waters of the state, residence or occupied structure, livestock, or public byway shall be 3 reported on COGCC Spill/Release Report, Form 19, and shall also be verbally reported to the Director as soon as practicable, but not more than twenty-four (24) hours, after discovery. COGCC REPORTING REQUIREMENTS (CON T) Spills/releases of any size which impact or threaten to impact any surface water supply area shall be reported to the Director and to the Environmental 4 Release/Incident Report Hotline ( ). Spills and releases that impact or threaten a surface water intake shall be verbally reported to the emergency contact for that facility/pipeline immediately after discovery. For all reportable spills, operators shall submit a Spill/Release Report, Form 19, within ten (10) days after discovery. An 8 1/2 x 11 inch topographic map showing 5 the governmental section and location of the spill shall be included. Such report shall also include information relating to initial mitigation, site investigation, and remediation. The Director may require additional information. Chemical spills and releases shall be reported in accordance with applicable state and federal laws, including the Emergency Planning and Community Rightto-Know Act, the Comprehensive Environmental Response, Compensation, and 6 Liability Act, the Oil Pollution Act, and the Clean Water Act, as applicable. 10

10 Rev 2 Federal Notifications NATIONAL RESPONSE CENTER USCG HEADQUARTERS WASHINGTON, D.C. 24 Hour Phone (800) The NRC is the sole federal point of contact for reporting oil and chemical spills which enter or threaten to enter the navigable waters of the United States. If you have a spill to report, contact the NRC via the toll-free number or visit the NRC Web Site ( for additional information on reporting requirements and procedures. For those without 800 access, please contact the NRC at All notifications to the National Response Center must be made at the earliest practicable moment following discovery but no later than one hour after confirmed discovery. ENVIRONMENTAL PROTECTION AGENCY REGION 6 DALLAS, TEXAS Region 8 Spill Hotline via NRC Region 8 Denver Office Calls to the National Response Center will automatically be forwarded to the appropriate EPA Regional Office. However, a courtesy notification can be made directly to the applicable region. The Environmental Protection Agency must be notified of the following: 1. Any unanticipated bypass exceeding effluent limitation by permit. 2. Any upset condition, which exceeds any effluent limitation in permit. 3. Violation of maximum daily discharge limitation or daily minimum toxicity limitation. 4. Chemical spills of a reportable quantity. Oral notification is required within 24 hours of the incident. No written report is required. 11

12 E.6 Qualified Individual Duties 1. Activate internal alarms and hazard communication systems to notify all pipeline personnel, 2. Notify all response personnel, as needed, Identify the character, exact source, amount and extent of the release, as well as the other items needed for notification, Notify and provide necessary information to the appropriate Federal, State and local authorities with designated response roles, including the National Response Center, State Emergency Response Commission and Local Emergency Planning Committee, Assess the interactions of the discharged substance with water and/or other substances stored at the facility/pipeline and notify response personnel at the scene of that assessment, Assess the possible hazards to human health and the environment due to the release. This assessment must consider both the direct and indirect effects of the release (i.e. the effects of any toxic, irritating, or asphyxiating gases that may be generated or the effects of any hazardous surface water runoffs from water or chemical agents used to control fire and heatinduced explosion.), Assess and implement prompt removal actions to contain and remove the substance released, Coordinate rescue and response actions as previously arranged with all response personnel, Use authority to immediately access company funding to initiate clean-up activities; and 10 Direct cleanup activities until properly relieved of this responsibility. 13

16 Scenario #2: Pipeline Release with Fire This is an example edit as needed 1. Discontinue all tasks in progress. 2. Attempt to extinguish incipient stage fires, if trained to do so. 3. Call Report the condition to the Pipeline Foreman and take further defensive actions as instructed. 5. Evacuate personnel to designated assembly areas. 6. Account for personnel. 7. Engage emergency shutdown systems and/or manually (from a safe distance) isolate fuel sources, shut down engines and heaters. 8. Initiate rescue activities as necessary, if properly trained. 9. Notify Pipeline Supervisor and make appropriate notifications to local fire and EMS. Make other internal management contacts as appropriate. 10. Establish a secure perimeter around the area to prevent unauthorized entry. 11. Initiate ERAP. 12. Continue measures to contain the fire, apply water from a safe distance to protect adjacent equipment, if necessary. 13. Make appropriate government agency notifications. 14. 18 South Area Gas Pipeline E.14 Safety and Training Review Each year, all employees are provided with all necessary information concerning the proper handling of a pipeline emergency. This information shall be in the form of a review sheet detailing proper procedures, which shall be signed and dated by each employee when all items are understood. Also, proper use of cleanup and containment tools and procedures will be demonstrated, if necessary.
