7 EMERGENCY EVACUATION 7 On hearing the ALERT tone (beep beep..) Open office or lab door and listen to instructions over the PA Prepare to evacuate (save files, turn off equipment, secure personal items) On hearing the EVACUATION tone (whoop whoop..) Close but do not lock office/lab doors. Leave the building by the nearest safe exit. Follow instructions given by Floor Wardens (Yellow Hats) Proceed to the ASSEMBLY AREA (AA) AA (B19) Courtyard adjacent to entrance of first year Physics labs AA (B27) Lawn area, adjacent to B27 car compound. 10 ON ARRIVAL IN THE DEPARTMENT Before any keys are issued, all new members of the Department are required to undergo the Monash Safety Induction Program which is available on the web: All new members must visit the Materials Engineering Safety web page: and work through each point involved including the Powerpoint safety presentations. All work must be risk inspected before it is commenced and a Risk Assessment (RA) prepared and signed by the worker and supervisor. The Risk Control Program and Worksheets are available at: If a hazard has been assessed, and, after taking into account all normal methods of risk minimization, the risk is still medium or high then a Safe Work Instruction (SWI) must be prepared. All equipment must also have a SWI. A proforma of the SWI is at: 23.doc All RAs and SWIs with risks which remain as medium or high must be checked and countersigned by the Safety Officer or Deputy or their delegate. 10
11 TRAINING 11 All new members of the Department are required to attend one of two Safety Inductions held each year. These will normally be held in week 1 of each semester. Registration along with information regarding timing and venue must be sought from the Safety Officer. The Safety Induction will include a discussion of Risk Assessments and Safe Work Instructions. All new/borrowed or modified machinery and equipment must undergo a safety signoff procedure found in the document Risk Management for New and Modified Machinery and Equipment under Guidelines, Policies and Procedures at: All individuals must seek permission from the Academic in Charge of each lab before performing any experiments in it. All users of equipment must officially trained by the Academic responsible for the equipment or a delegate. All chemicals entering the Department must be accompanied with the official MSDS and the Officer-in-Charge of the lab must be notified of the chemical name, if Hazardous, the DG class, quantity and supplier. A copy of the MSDS must be provided to the General Office before they will order a chemical.
12 IMPORTANT INFORMATION 12 FIELD TRIPS Staff responsible for undergraduate field trips should consult the guidelines (OHS Field Trip Guidelines): Supervisors are responsible for the safety of any researcher who is working at Monash or at another site ELECTRICAL WIRING All electrical wiring (even 3-point plugs!) should be done by electronics workshop personnel. Similarly, if any fuses need to be replaced, electronics workshop staff need to inspect for equipment faults. Do not try to do any electrical wiring yourselves however competent you think you may be. All electrical leads and plugs must be checked regularly for fraying and bare wires. Always pull plugs out of equipment by the plug, do not pull the wire. Double adaptors and piggy back plugs must not be used in the University; instead power boards fitted with a cut-out should be obtained from Dr Ian Watkins.
13 IMPORTANT INFORMATION 13 INCIDENTS YOU MUST REPORT Any occurrence that leads to or potentially leads to injury or danger to health must be reported to the Safety Officer. Note: You will be issued with an Incident Report Form which you must fill out: Leaks, Property damage, maintenance issues, obstructed walkways must be reported to: Dr Ian Watkins (Ext 59607) or Mr Jim Mitchell (Ext 54927) Threats against staff, students or Monash Property are not to be ignored. On receiving or becoming aware of a threat, ring (Gate House) and ask to speak to the Head of Security. For threats which require immediate action, ring 333 EMERGENCIES Location of fire extinguishers, emergency procedures and evacuation, and assembly areas? See the emergency procedures notices & plans posted at the end of each corridor.
14 PERSONAL SECURITY AFTER HOURS 14 NORMAL WORKING HOURS Between 08:45 and 17:00 during university working days Undergraduates must comply with these times unless written approval is provided by your supervisor and the academic in charge of the lab AFTER HOURS WORK Between 17:00 and 08:45 on weekdays and at any time on weekends and university holidays. Please see: for guidelines on whether work is permitted during these hours Entering Buildings Permission may be required to enter labs after hours - see the Lab Protocol Sheet Leaving Buildings If you expect to the leave the building at night, as far as possible, take the following precautions: 1. If practicable, escort each other to the cars 2. Travel via the most well lit paths. 3. Carry your keys in your hand (maybe useful as a defensive weapon). 4. Never chock open external doors. This invites unlawful entry. 5. Walk confidently. 6. The night security bus service (runs until a.m. weekdays) will detour to take you to your car. The schedule is posted on the Safety Notice Bulletin Board (B19 adjacent to Room 151 or B69 opposite General Office, Room 110). 7. Security will escort you to your car after a.m. if you ring the Gate House on ext However, if security are on another call, you may have to wait for this service. 8. About 5.00 p.m. move your car, and park it as near to the building as possible. 9. Carry a whistle and/or a personal alarm.
