11 The Bidder should have the following eligibility Criteria to participate in this EOI. 1) The Firm Alarm Panel should be EN54 VDS approved panel only. 2) Authorized System Integrators for National / International (OEM) Manufacturers for Fire Alarm Systems only to participate in this Expression of Interest. 3) The Applicant should either be the original equipment manufacturer or a Direct Authorized (Dealer / Distributor) of OEM (preferably manufacturer-cum supplier) of the proposed system or a subsidiary company of a manufacturer company for such system or an authorized supplier/channel partner of a manufacturer, of the proposed system. 4) The applicant should submit the authorization Letter from the principal / OEM whose products are proposed. 5) The bidder must visit/examine the site and it s surrounding on any working day by prior appointment with the concerned officer of the Corporation Electrical Division for the proper assessment of prospective assignment (scope of works) before submitting their offer. No claims later on shall be entertained. 6) Work Experience: The applicant should have past satisfactory 7 year experience in Supply, Erection, Testing, Commissioning, integration, maintenance and repair of Security and Fire equipments for at least 2 reputed organizations. Documentary proof should be provided in support of experience like order copy/contract copy and installation certificate/satisfactory performance certificate from customer. 7) The Bidder should not bid under any Consortium. No Consortium bid shall be allowed for this Tender. 8) The decision of the Corporation with regards to selection of Vendors for issue of Prequalification Application Form will be final. The Corporation is not bound to assign any reason thereof 9) Average annual turnover of the bidder in last 3 (Three) financial years ending on 31 March, 2016, should not be less than Rs Lakhs (Three Hundred Lakhs of Indian Rupees). Copies of Filed Audited Statements, IT Returns, are to be submitted to corroborate this. 10) The bidder should not be under a declaration of ineligibility for corrupt and fraudulent practices issued by Government of India or any State Governments in the country of India. (To be furnished under Notarized Affidavit made at Bhubaneswar) 11) The bidder must not have any history of defaulting in execution of work orders issued by Government of India or any State Government in the country of India. A self-declaration certificate to this effect should be enclosed. (To be furnished under Notarized Affidavit made at Bhubaneswar) 12) The bidder hasn t been blacklisted by any Central/State Government institution and there has been no pending litigation with any government department on account of similar services. 12 13) No Criminal Proceedings in any Court of Law should pending against the bidding Firm Or its Promoters Or its Directors Or its Executives. (To be furnished under Notarized Affidavit made at Bhubaneswar) 14) The Manufacturer of the equipment must have a sound and well documented Quality Framework. Applicant is to submit either a valid ISO/ISI certificate, BIS certified or any certified by the agency designated for the purpose by appropriate Government (NDMA, SFAC) and other required certifications for the manufacturer in support of this clause. 15) CODES AND STANDARDS In general the Fire Alarm System and all components shall be listed under the following standards and approvals as applicable. a. For Fire Alarm System (FAS) i. Standards : NFPA 72/IS /IS22189/IS-2175/EN54 ii. Approvals: VdS/ UL/ LPCB or equivalent b. For Cables The latest revisions of the following standards shall apply in design, testing and manufacturing of this cable. i. IS: 694: Specification for PVC insulated electric cables. ii. IS: 5608: Specification for Low frequency wires and cables with PVC insulation PVC and sheath. iii. IS: 5831: PVC insulation and sheathing for electrical cables. IS: 10810: Method of test for cables. iv. IS: 1554 Part and IS: and IS: for Insulation Testing v. Smoke generation test as per ASTM D vi. Oxygen Index test as per ASTM D vii. HCL Gas Emission Test as per IEC viii. Flammability Test 1. IEEE IEC SS Class F-3 16) Service Support Office: It is mandatory that the applicant should have an established office/service Centre at in Bhubaneswar state of Odisha for a period of minimum 5 years for providing 24 hours x 7 days post implementation support. Complete address, contacts and Documentary evidence for the same needs to be provided. The contact details, especially the telephone numbers, of service focal point(s) should remain unchanged during the period of vendor's relation with the Corporation. In case, these are changed, the vendor would inform the Corporation at least 15 days in advance.

13 17) Necessary supporting documents on fulfilment of eligibility criteria should be attached for authentication along with a signed copy of the EOI document including addendum/ corrigendum, if any, to indicate acceptance of all terms and conditions set forth in the tender. Organizations failing to provide complete information on any of the requirements are liable to be rejected. 18) Capability Certificate to the tune of 2 Crores is to be submitted by the vendor from any nationalised bank stating that the vendor is financially sound to execute orders of 2 Crores. 19) The bidder shall submit an undertaking from the respective OEM that the Fire Alarm System will be installed, commissioned, tested and Fault Corrected by OEM s Support Professionals and ensure direct and uninterrupted support of the OEM during the Warranty / Contract Period. 20) Should have sufficient qualified and trained engineers/technicians for supervision and execution of works. 21) The Bidder shall submit an undertaking that he will resolve complaints within Maximum 24 hour s resolution time for all support-request/problem / issues. 22) All the OEM / OEMs Authorizations and Certificates must be Specific to this Tender No- XXXXX, Dated XXXXX 2016, of XXXX Odisha. 23) Manufacturers Test Certificate to be submitted for Pumpsets of Fire Fighting System after successful installation of the Fire Fighting System. 24) Drawing: The successful bidder shall prepare layout, drawings, and fabrication drawings, isometric drawings and submit to the department for approval. The department may from time to time make certain changes in the drawings and specifications and issue additional instruction if required. The work shall be carried out in accordance with the specification and final drawing approved. All drawings specifications and copies thereof furnished, approved by department shall be property of department. They are not used in any other work & returned to department after completion of work. 25) The bids may include recommended spares, optional accessories etc. which may enhance the performance, availability and reliability of the instrument 26) Any damage to the civil works during activities of systems installation shall be rectified by the supplier at his own cost. 27) INSTALLATION: After placement of Purchase Order, the supplier s representative shall visit the site immediately and give final recommendation on the installation of detectors to be placed in various locations and should supply a loop drawing indicating details of various devices in the loops and suggest layout of cables. The responsibility of connecting the devices in the loops with required connectors and cables lies with the supplier. The supplier shall take approval of the Indenting officer for the drawing and install the Page 11 of 88 detectors, Fire Alarm (FA) control panel and other components connected to the system as per approved drawing.

14 28) COMMISSIONING: Supplier shall prepare a commissioning procedure for approval by the purchaser and test and commission the system as per approved procedure for working as per the specification. The supplier shall carry out any modification coming during commissioning and within the Guarantee period. 29) HANDING OVER Documentation: a. All documentation shall be written in good, simple and concise English using accepted technical terms, symbols and nomenclatures and hard copy shall be submitted. b. The document shall be updated regularly as the installation progresses. All changes in the installation layout, wiring, cabling and design shall be incorporated in its final edition. Three copies of every updated edition shall be handed over to OPHWC. c. The final edition of the hand-over documents (4 copies in CD) and one hard copy shall include design, installation, commissioning, operation and maintenance aspects, technical details of bought out components, as built drawings & device charts of the system. d. An O & M training program shall be arranged by the supplier with commissioning of system on O&M aspects of fire alarm system 30) Following documents (self-attested) shall be submitted along with the quotation a. List of past supplies made b. Performance feedback report from users c. Quality Assurance (QA) plan for the tendered item(s) d. Certification / declarations from OEM for continued support for minimum of 5-7 years after the commissioning. e. Non obsolescence of items offered shall be for at least 5-7 years. f. List of plant & machinery and other infrastructure facilities to complete the work in time. g. Details of manpower. h. Performance certificates. i. Proof of average annual turnover of not less than Rs.3 Crores during last three years. j. Profit and loss statement of last five years confirming that loss has not been incurred for more than two years in the period of last five years. k. PAN and Company registration certificate to be submitted l. The Bidder should have EPF Registration and ESIC registration m. The Bidder should have Service Tax Registration n. The bidder must have been registered with Sales Tax Authority and must furnish the VAT registration certificate, VAT Clearance Certificate

Service Centre Address and contact details of Odisha should be provided along with company details. 31) Force Majeure: The Bidder / Contractor shall not be liable for failure, non-performance or delays in supply or services resulting directly or indirectly from causes beyond its reasonable control, such as, Act of God, war, act of Terrorism, nuclear risks, riots, strike, political unrest, acts of Government, its agencies or officers, raw material shortage, labour shortage or any other legitimate cause beyond his reasonable control. However, due LD shall be levied on Bidder / Contractor on its failure to adhere to work schedule approved by OPHWC which signing the contract. ON Completion, the Bidder / Contractor shall hand over a complete detailed drawing of the work AS BUILT-IN with operation Manual etc for future reference. 32) Liability: except reasons covered under Force Majeure defined maximum liability of Bidder / Contractor may be up to complete replacement if the Product becomes nonfunctional within 2 years of commissioning for reasons covered in the clauses. Price Bid a. Non Interest bearing Security Deposit of Rs. 1 Lakhs to be submitted by the Vendors along with the EOI and will be refunded for approved bidders after 1 year. For those who have not been qualified will be returned back after the finalisation of approved bidders. b. Performance Bank Guarantee of 10% of the total project cost to be submitted valid till expiry of warranty period for selected bidder within 7 days of issue of LOI and will be released after satisfactory services.
