2 Factory 09 Apr 2014 Introduction to the Report The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by the Alliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment was conducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard, which is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) for the Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technical requirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairly evaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used by RMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on action steps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements through technical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wage support for workers if factories are closed for remediation. The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute or continued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to provide a non-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. 5 09 Apr 2014 ASSESSMENT FINDINGS Fire Protection Construction Are openings and penetrations through rated walls and/or assemblies protected? 01.Exits are not fully protected by continuously fire rated assemblies. 02.Penetrations were found at each landing of stair Opening was found between store and production floor at 5th floor, between production floor and stair at 4th, 2nd & 1st floors. 04.Penetration was found on stair wall at 4th, 2nd & 1st floors. 05. Opening was found between factory and residential building at exterior wall. 06.Opening of generator room was not fire protected. Internal stair was found from 3rd to 2nd and 2nd to 1st floor, where floor openings were not fire protected. Photograph: Unprotected internal stair. Provide 2-hour fire-resistive rated construction barriers at exit enclosures with 1.5-hour fire-rated opening protection (Door). The new fire rated door will sidehinging swinging outward opening type, with auto closure and panic bar and without locking arrangement. Minimum width of new fire rated door needs to be 1.00 m. Doors need to be free from general locking arrangements. Consult a qualified fire protection engineer to design the required rated construction barriers with opening protection. Reference Alliance Standards Part 4 Section 4.5 Separation Are exit enclosures provided with fire-resistive rated construction barriers? The building was 7 story, with walls of all stairwells having 125 mm (5 in.) masonry brick wall and collapsible gates were installed in most of the exits leading to the stairwell. Factory building and adjacent residential building are separated by a 125 mm (5 in.) brick wall on the North side of the building. Exterior wall on the South side of factory building and adjacent residential building was constructed by 5 in brick wall having 1 ft. separation between those buildings. 2. Store and production floor was not separated by proper fire rated barrier on 4th floor and 1st floor. 3. Unprotected boiler room was found at 4th, 3rd and 2nd floor. 4. Generator room was not fire rated construction. 5. Portable generator was open to the stair at 1st floor. 6. Non fire rated construction was found at 4th and 5th floor of exterior wall. 7. No fire rated separation was found at childcare with warehouse and dining at 3rd floor. All of these items are not compliance with Standards Part 4 Section 4.5 Separation; Part 3 Section Boiler or furnace rooms; Storage, Generators. Photograph: 1. North and south side of building, 2. Class-II standpipes (40 mm dia) were available inside at all floors except Ground Floor to 2nd floor. No Class-I (65 mm dia) hoses were found in the factory building. 25 mm dia standpipe hose connections were available inside the floor at 1st & 2nd floors. Minimum pressure of 65 Psi was not tested during the audit. Further evaluation is required by the owner. Alliance Standard Part 5 Section Does the building have a fire pump? Not in compliance. No dedicated fire pump was found on the day of inspection. Visual Assessment: No fire pump was noticed during this assessment. Provide a dedicated fire pump in accordance with NFPA 20 to supply the demands of water to the connected fire protection systems along with a stored source of water sufficient to meet the demands per NFPA 22. Alliance Standard Part 5 Fire Protection Systems Are notification and initiation devices for the fire alarm system installed at required locations based on occupancy type? Not in compliance: Pull stations at egress points: Manual fire alarm devices were installed. Piano switches were used as fire alarm call points. Smoke detectors: No centralized detection system was available. Only standalone smoke detectors were available in a very few locations in this factory building. NFPA 25 Chapter 6 Standpipe and Hose Systems Are fire department connections provided and clearly identified for the Fire Protection Systems? Not in compliance. No fire department connections are provided per the requirements of Alliance Standard Part 5 Section Fire Department Connection. Visual Assessment: No fire department connection was noticed during this assessment. Fire department (Siamese) inlet connections need to be provided to allow fire department pumper equipment to supplement the fire protection systems. Fire department outlet connections need to be provided to allow fire department pumper vehicles to pump water into the standpipe system and another to draft water from ground-level or underground water storage tanks. Connections need to match the Fire Service and Civil Defense hose thread standard. Also The Alliance for Bangladesh Worker Safety Page 11/28

12 09 Apr 2014 need to ensure reservation of the required amount of water for fire-fighting. Consult a qualified fire protection engineer to design this requirement. Alliance Standard Part 5 Section Fire Department Connections Portable fire extinguishers have been selected based on potential fire class and hazards? Portable fire extinguishers have been not selected per potential fire class and hazards in accordance with NFPA 10 Chapter 5. Visual Assessment: No foam type extinguisher has been found during this assessment. Need to install Portable fire extinguishers per potential fire class and hazards in accordance with NFPA 10 Chapter 5. NFPA 10 Chapter 5 Fire extinguishers are inspected, tested, and maintained as required. Not in compliance. There were maintenance check lists with each extinguisher and the acting fire safety officer checked only the expiration date on a monthly interval without a proper checklist covering all the issues as per NFPA 10. While interviewing the person responsible for this it was found that only pressure gauge (where available) and expiration dates are checked. Not in Compliance per Alliance Standard Part 5 Section No centralized detection system was found. Only standalone smoke detectors were available in a few locations in the factory building. Photograph: Standalone smoke detector Centralized fire alarm and detection system needs to be installed and the control panel of this system shall be monitored by a central station monitoring service or directly connected to the Fire Service and Civil Defense. Trouble or alarm notifications should be indicated on the fire alarm control panel. Until that time that a central station monitoring service or direct connection to the Fire Service and Civil Defense can be set up, a person need to be assigned to contact the fire department in the event of fire alarm activation. An annunciator needs to be located in a constantly attended location to alert this person. 27 May 2014 Alliance Standard Part 5 Section Monitoring Are inspection, maintenance, and testing procedures of the standpipe and hose system documented and up to date? Including inspection and testing of hoses if provided. Low Non-Compliance Level: 1 No documents of inspection, testing, and maintenance procedures of the standpipe system were found during the day of inspection. Document Review: No document was provided during this visit. Establish an inspection, testing, and maintenance program for the standpipe and hose system. Signage must comply with NFPA Jun 2014 Reference NFPA 14 Chapter 6 Means of Egress Occupant loads on each level (floor) do not exceed the capacity of the available means of egress. The total workers of 2nd floor is 366. Total width of stair is ( ) = 3.12 m and landing widths ( ) = 2.81 m. The effective widths of stair was reduced for providing landing widths as per Alliance Standard Section Required total widths of stair 2.93 m per occupant 8 mm per BNBC Table 4.3.2, which is more than effective widths of stairs. Therefore per Alliance Standard Part 6 Section 6.4, the occupant loads on each level (floor) exceed the capacity of the available means of egress at 2nd floor. Visual Assessment: Counting the occupants, measuring the stairs and calculating the capacity. Reduce the occupant load per the capacity of the floor and the available means of egress. Alternately, provide additional exit capacity or provide automatic sprinkler protection throughout the building. 13 May 2014 Alliance Standard Part 6 Section 6.4 Occupant Load Aisles are provided with the minimum unobstructed clear width of 0.9 m (36 in) based on occupant loads. Alliance Standard Part 6 Section 6.5 Egress Width The path of egress along the means of egress is not reduced at any point along the path of travel and is sufficient for the occupant load. The width of each flight of stairs were not same in a single floor at stair-1, 2 & 3. The path of egress along the means of egress was reduced at all stairs as the exit doors (collapsible gate) of these stairs were fixed at the end of the stair. This is not in compliance with Alliance Standard Part 6 Section Means of Egress Continuity. Photograph: Collapsible gate at stair. Ensure the path of egress along the means of egress remain uniform as per requirement of occupant load. Alliance Standard Part 6 Section 6.5 Egress Width and BNBC Table Exit discharge is directly to the exterior of the building, unless the requirements of are met, at grade or provides direct access to grade. Exit discharge shall not reenter a building. Exit discharge of stair 1 & 2 terminated inside the market place, which was not separated by rated walls/assemblies per Alliance Standard Part 6 Section 6.17 Exit Discharge. The exception is where an exit terminates at an interior exit passageway that is constructed to meet the same rating requirement as the exit stair. The rated assembly or sprinkler system need to be approved and/or designed by a qualified fire protection engineer. Alliance Standard Part 6 Section 6.17 Exit Discharge. See Section and for exceptions. Exit access corridors serving an occupant load exceeding 30 are separated by walls having a fire-resistance rating of 1 hr. The exit access corridors of the ground floor serve more than 30 persons, the corridor's walls were made of 5 in. brick wall with non-rated openings. These corridors were not separated by proper fire rated barrier per Alliance Standard Part 6 Section Photograph: Unprotected corridor at ground floor. Exit access corridors serving an occupant load exceeding 30 shall be separated by walls having a fire resistance rating of 1-hour and with 0.75-hour opening protection or provided with automatic sprinkler protection throughout the building as per NFPA 13. The rated assembly or sprinkler system need to be approved and/or designed by a qualified fire protection engineer. Alliance Standard Part 6 Section 6.3 and Part 4 Section 4.5. Does not apply if an automatic sprinkler system is installed throughout the building. Means of egress are free from impediments, obstructions, and stored materials. 17 09 Apr 2014 Section Reliability Doors are not locked in the direction of egress under any conditions. All hasps, locks, slide bolts, and other locking devices have been removed where required. Doors in the path of egress were found unlocked in the direction of egress only in working hour. Locking devices were provided on doors. Collapsible gates were installed at many of the exits. Photograph: Collapsible door All roll-down, collapsible, sliding gates and shutters in the means of egress need to be replaced with fire-rated, outward-opening, side-hinged swinging, self-closing type doors per Alliance Standard Section: 6.8. Doors must be free from general locking arrangements. Alliance Standards Part 6 Section 6.8 Doors and Gates Stairs have a minimum width of 0.9 m (35 in.). Minimum width of stair was measured to be less than 0.9 m (35 in.) at 3rd floor. This internal stair was also a main stair for egress of the 3rd floor. Landings are provided on both sides of doors used along the path of egress. Doors do not swing out over stairs. A collapsible gate was fixed at the end of the stair- 1 & 3 of building hence no landing available at that location. Photograph: Stair-1 without landing, Stair-3 without landing. Remove the collapsible gates from the above locations. Alliance Standards Part 6 Section 6.8 Doors and Gates Interior exit stairways and ramps terminate at an exit discharge except where terminating at a rated exit passageway. Need to terminate both of these stair discharges directly outside the building or need to construct 1-hour rated exit passageway of both stairs which leading directly outside the building or need to provide with automatic sprinkler protection throughout the building as per NFPA 13. 19 09 Apr 2014 Doors along the path of egress have a minimum width of 0.8 m (32 in) and have required ratings. Minimum widths of exits were found less than 0.8 m (32 in) at 3rd floor at stair- 3 & ground floor at stairs 2 & 3. Photograph: Minimum widths of exits at Ground Floor of Stair-1. Alliance Standard Part 6 Section Minimum Widths. Increased occupant loads will require a door width greater than 0.8 m. All paths of egress are provided with compliant means of illumination. Install appropriate means of illumination at required locations in the means of egress per the Alliance Standard. Upon installation, begin verification and testing of the emergency illumination. If battery operated lights are used, these lights shall be tested on a monthly basis. Functional testing of battery powered lights shall be provided for a minimum 90 min once per year. 20 09 Apr 2014 Handrails are provided on both sides of each stairway. Intermediate handrails are provided when the stair width exceeds 2.2 m (87 in.). Handrails are not mounted lower than 760 mm (30 in.) or higher than 1100 mm (44 in.). Main stair: Handrails have been provided on only one side of each stairway having minimum height 762 mm (30 in.). Maximum width of stair was found to be 1.37 m (54 in.). Internal Stair: Handrails are provided on both sides of stairs; the existing handrails height were more than 30 in. but spacing of vertical members were more than 200 mm (8 in.) in all of the stairs. A minimum height of 865 mm (34 in.) and a maximum height of 965 mm (38 in.) as measured from the leading edge of the tread need to be maintained when installing new handrails. The spacing between vertical members should not exceed 200 mm (8 in.).
