2 10 Feb 2014 Introduction to the Report The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by the Alliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment was conducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard, which is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) for the Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technical requirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairly evaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used by RMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on action steps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements through technical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wage support for workers if factories are closed for remediation. The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute or continued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to provide a non-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. Neither the QAF nor the Alliance can certify or guarantee the quality, outcome, or effectiveness of actions taken in response to the report. 5 10 Feb 2014 ASSESSMENT FINDINGS Electrical System Maintenance A transformer oil analysis is routinely completed on main service transformers. Low A transformer oil analysis shall be routinely completed to endure uninterrupted service without major shutdowns. Visual Assessment: Oil test report not found. Complete an oil analysis on applicable transformers at appropriate intervals based on voltage and power. Alliance Standard Part 10 Section Electrical Inspections Electrical System Conditions Shielding or additional insulation is provided for wiring exposed to external heat sources. High In order to avoid the effects of heat from external sources one of the following methods shall be used to protect wiring systems: (1) shielding; (2) placing 900 mm (36 in.) from the source of heat; (3) selecting a system with due regard for the additional temperature rise which may occur; (4) local reinforcement or substitution of insulating material. Visual Assessment: Insulation is not provided for the cables and wiring where needed. 6 10 Feb 2014 Non-Compliance Level: 3 Capacity labels shall describe the maximum number of protective devices that can be accommodated, and the capacity of the boxes shall be described on the information label. Means to prevent installation exceeding the designed number of devices is described in section of, NFPA Per NFPA , a panel board shall be provided with physical means to prevent the installation of more over current devices than that number for which the panel board was designed, rated, and listed. A 2-pole circuit breaker or fusible switch shall be considered for two over current devices; a 3-pole circuit breaker or fusible switch shall be considered for three over current devices. Ensure switchboards and/or distribution boards provided with physical means to prevent the installation of more over current devices than that number for which the panel board was designed, rated, and listed following NFPA 70 section Alliance Standards Part 10 Section 10.7 Main Switch, Switchboards and Metal Clad Switchgear Each circuit is provided with a dedicated neutral. Non-Compliance Level: 2 Circuits are not provided with a dedicated neutral for each load. The use of common neutral for more than one circuit is not permitted by the standard. Location: LT Panel /GF, DB-1/GF, SDB-1/1st Floor, DB-1/2nd Floor, SDB- 1/2nd Floor Visual Assessment: Circuits arenot provided with a dedicated neutral for each load. Provide dedicated neutrals for each circuit. Alliance Standards Part 10 Section 10.3 Electrical Wiring and Cabling Stranded conductors having a nominal cross-sectional area 6mm2 or greater are provided with cable sockets. Conductors below 6 mm2 without cable sockets, all strands at the exposed ends are soldered together or are crimped using suitable sleeve or ferrules. Non-Compliance Level: 2 In MDB/DB/SDBs cables shall be attached using splicing joint at their busbar with using cable lugs. Per and (c) of BNBC-2006 Cables shall be connected to terminals only by soldered or welded lugs, unless the terminals are designed to securely clamp without cutting away cable strands. Low Phase separators are required to provide for the terminals of circuit breakers to prevent arcing and short circuit between phases. Location: SDB-01/2nd FL, DB-01/2nd FL, SDB-01/1st FL, DB-01/GF, LT-Panel/GF Photograph: Phase Separator is not fixed on circuit breakers. Install phase separators between terminal connections at the noted locations. Alliance Standard Part 10 Section Electrical Connections Emergency Power System Are inspection, maintenance, and testing procedures of the emergency generator being completed and documented? Low Inspection, maintenance, and testing procedures of the generator shall be documented. Visual Assessment: Inspection, maintenance, and testing procedures of the generator were not found on site. Establish a routine maintenance and testing program for the emergency generator. The program shall be based on all of the following: (1) Manufacturer's recommendations (2) Manufacturer's Instruction manuals (3) Requirements of NFPA 110 Chapter NFPA 110 Chapter 8 Are inspection, maintenance, and testing procedures of the UPS being completed and documented?
