2 03 Apr 2014 Introduction to the Report The following report contains a site profile and summary of non-conformities identified during an onsite assessment commissioned by the Alliance for Bangladesh Worker Safety (Alliance) and conducted by a third-party Qualified Assessment Firm (QAF). The assessment was conducted against the Alliance for Bangladesh Worker Safety Assessment Protocols (APs) and Fire Safety and Structural Integrity Standard, which is harmonized with the factory assessment guidelines developed by Bangladesh University of Engineering and Technology (BUET) for the Bangladesh National Tripartite Plan of Action (NTPA). The goal of the Alliance process is to provide clear and practical technical requirements by which Bangladeshi Ready Made Garment (RMG) Factories producing for Alliance members may be consistently and fairly evaluated for fire, structural, and electrical safety in a non-duplicative manner. Each assessment will prompt action plans that will be used by RMG factories to systematically and sustainably improve safety conditions for garment workers. Beyond tracking and reporting on action steps taken in a transparent manner, the Alliance organization and its members will seek to further support factory improvements through technical assistance, training, implementation support for functional Worker Committees, and in some cases financial assistance and wage support for workers if factories are closed for remediation. The contents of the report do not constitute a guarantee of compliance with the applicable laws, the Alliance Standard or the absolute or continued safety against fire, electrical and/or structural integrity issues that may lead to injury or loss of life. The report is designed to provide a non-exhaustive summary of risk issues, based on a limited sampling and duration of time onsite by the named QAF. Non-Compliance Level: 3 No updated as-built drawing was found on the site. Document Review: No documentation available. Have a qualified electrical engineer develop as-built electrical drawings providing detailing key components of the electrical system according to Alliance Standard Part 10 Section Alliance Standard Part 10 Section Electrical System Maintenance Have workers that operate and maintain the electrical system received electrical safety training? Is training documentation on site? Workers got electrical safety training but documents concerning the training were not found during assessment. Insulation tests shall be made between open contacts of circuit breakers, switches etc. and between each phase and earth. Survey all distribution boards for higher rated circuit breakers and have a qualified electrical engineer determine the appropriate size of MCCB that should be used based on the current carrying capacity of the wiring. Multiple wires connected at a single point and looping of wiring was found at circuit breakers in the distribution boards on every floor. Photograph: Main incoming terminal of MDB has multiple cable connections. Multiple cables should enter in one point either on Bus Bar or MCB, MCCBs. Remove all instances of multiple wire connections and looping of wiring in all distribution boards. Connect all metal in the building to the building's earthing/grounding system by approved means. Provide proper grounding(earthing) connection for all switchboards and/or distribution boards. Clear and permanent identification marks are required to be painted in all distribution boards, switchboards, sub main boards and switches as necessary. Circuits in several distribution boards are not provided with a dedicated neutral for each load. Visual Assessment: Visual assessment Provide dedicated neutral for each circuit. The use of common neutral for more than one circuit should not be allowed in the standard. 16 03 Apr 2014 Electrical wiring and conduit is properly supported. Electrical wiring should be well and properly supported to avoid sudden falling down. There is no support for service cables wiring in this factory. Photograph: Conduits are not properly supported for service cables. Survey all wiring to identify areas where the wiring/cables are not properly supported. Provide proper conduit for electrical wiring. (Ref:Alliance Standard Part 10 Section , , and ) Alliance Standard Part 10 Section , , and Stranded conductors having a nominal cross-sectional area 6mm2 or greater are provided with cable sockets. Conductors below 6 mm2 without cable sockets, all strands at the exposed ends are soldered together or are crimped using suitable sleeve or ferrules. In MDB/DB/SDBs some cables has not been attached using splicing joint at their busbar with using cable lugs. Visual Assessment: Cable lugs not used to terminate the cables in MCCB for some of the distribution boards. Need to reconnect with proper size of cable sockets. Electrical connections to equipment were not securely connected. Visual Assessment: Conceal conduit cables were found without covers at roof ceiling, Also extra cable were drawn from main breaker point. Cable lugs were missing in some cable termination points of main distribution boards. Visual Assessment: Missing cable lugs Survey all electrical wiring to identify wiring that is not terminated or has not been terminated properly. Remove all abandoned wiring and ensure all wiring is terminated at its intended point of connection in a compliant manner. Low Phase separators were not provided at most of the MCCB within the distribution boards on each floor. Visual Assessment: No Phase Separators Observed. Survey all distribution boards and install phase separators between terminal connections at the missing locations found during the survey. Phase separators are need between terminals of circuit breakers to prevent arcing and short circuiting between phases. Low Non-Compliance Level: 1 No instruction board for first aid and artificial respiration was found in the generator room. Visual Assessment: Instruction board not found Provide instruction board for first aid and artificial respiration located in substation room and generator room. Instruction board should be displayed for restoration of persons suffering from electrical shock and artificial respiration in case of electric shock at any place in the factory. 04 Jul 2014 A sign detailing electrical shock first aid procedures should be installed in these rooms. Non-Compliance Level: 3 Emergency power switchboards, distribution boards can not be identified. Visual Assessment: No identification evident here in all areas. All boxes and enclosures (including transfer switches, generators, and power panels) for emergency circuits shall be permanently marked so they will be readily identified as a component of an emergency circuit or system. The required marking can be by color code, the words emergency system, or any other method that identifies the box or enclosure as a component of the emergency system. 19 Jul 2014 NFPA 70 Chapter 7 Article Wiring, Emergency System Are cable trenches properly covered? Emergency lights are connected with battery Visual Assessment: Emergency lights are served by battery. Life safety loads (fire alarm, fire pump, elevators, emergency lighting, exit signage, etc) should be connecting to the emergency power system to ensure an uninterrupted power supply to these equipment.
