1 Application to change a Practice Area Received from Elm Tree Medical Partnership, Shrivenham Consultation Process Purpose PURPOSE & BACKGROUND Drs Crockett & Partners, Elm Tree Surgery, High Street, Shrivenham, SN6 8AG have requested approval to amend their practice area. They wish to decrease their practice boundary. The following describes the areas from which Elm Tree Medical Practice would like to withdraw: Small towns and villages In Oxfordshire: Faringdon, Coleshill and Great Coxwell. These are also served by the White Horse Practice, Faringdon, and the Fern Hill Practice, Faringdon in Oxfordshire. Coleshill is also covered by Westrop Surgery, Highworth in Swindon Borough, and the Medical Centre, Lechlade in Gloucestershire. In Swindon Borough: Highworth, which is predominantly served by Westrop Surgery, Highworth and is also in the area of Merchiston Surgery, Swindon. The Elm Tree Medical Partnership practice intends to retain existing patients from all the above areas but not register any new ones, except, as required, for family members at the same address. Elm Tree Medical Practice will also continue to register staff and partners from other GP practices if the policy of those practices is for staff and partners to register at a practice other than the one at which they work. The area changes can be seen at appendix 1. Background The major patient registration areas for Elm Tree Medical Partnership are the Oxfordshire villages of Shrivenham and Watchfield, plus the surrounding small villages and hamlets. The area is largely rural at present, but note paragraph three below about the large housing development planned. As at 31 st Sept 2014 the practice list size was 6,742. The list changes can be seen at Appendix 2. The practice has experienced a recent increase in list size related to the development of housing in Shrivenham and Watchfield. Page 1 of 5
2 Consultation This includes In the area between Swindon and Shrivenham a new development of 8,000 to 9,000 houses is planned. It is reasonable to assume that this will have its own practice(s) when the trigger points are reached, but the occupants of the early houses developed will have to be accommodated by existing local practices, e.g. Elm Tree Surgery in Shrivenham and Merchiston in Swindon (14,100 list size, an increase of 300 since Sept 2013). Elm Tree Surgery s catchment area is a natural fit for this development of houses and the practice has previously expressed an interest in accommodating some of these patients. The practice registers the civilian families of the military personnel working at the Military Academy at Shrivenham. Military personnel are registered with the GPs provided by the Military Academy. Elm Tree Medical Practice also has a small number of patients from the Swindon areas of Covingham and Stratton St. Margaret (covered by Merchiston, Whalebridge, Hawthorn, Eldene and other practices), and Blunsdon (covered by the Blunsdon branch of Westrop Surgery). These patients are outside the current area of the practice and not adjacent to the area affected by the boundary change. They will not be affected by the boundary change.. Westrop Surgery, Highworth The White Horse Medical Practice, Faringdon Fern Hill Practice, Faringdon Merchiston Surgery, Swindon Lechlade Medical Centre NHS England, Thames Valley Area Team, Oxford. Wessex LMC Swindon CCG Healthwatch Swindon There is no overlap with the area of Bampton Surgery, Oxfordshire. The area of Wantage Surgery, Oxfordshire, touches the boundary of Elm Tree Surgery for a short distance next to the area that will remain open, but not the area of planned closure. Page 2 of 5
