2 Participation in the Service The first FFT data to be submitted to NHS England will be the data collected during January After that, FFT data will be submitted monthly. Before the first submission date, NHS England will offer you the FFT Service through CQRS, allowing you to submit monthly data for the practice. Once NHS England has offered the Service it will appear in your CQRS Message Centre as a 'new task'. To view the offered Service either select the 'Task Summary' link or select the 'Participation Management' tab from the main menu. Select the checkbox to the left of the Service and then select the option 'Accept Service'. A confirmation message will be displayed to make sure that you wish to accept the selected service. Select the 'Yes' button to accept the Service. CQRS will then display a confirmation message that the FFT has been successfully accepted. You can then manually enter the achievement data. If you are expecting to be offered the Service, but it has not come through on CQRS, please contact the NHS England FFT team at england.friendsandfamilytest@nhs.net.

4 Manually entering achievement data for the indicators GP practices will need to enter achievement data manually for the indicators. To enter your monthly achievement data for the FFT you should first select the 'Data Submission' tab from the CQRS main menu. Select the 'Record Achievement' option. Make sure the financial year in the dropdown box is set to 2014/15 or 2015/16. Select the Friends and Family Test from the dropdown box. Select the end of month you are entering data for from the Achievement Date drop down. For example for the submission in February where you are entering the data that has been collected for January 2015 select the date 31/01/2015. Select 'Add New Achievement'. Click on the link Management Information. You will then be able to enter data for the following fields: FFT001 FFT002 FFT003 FFT004 FFT005 The number of responses to the FFT question in the response category: extremely likely for the month The number of responses to the FFT question in the response category: likely for the month The number of responses to the FFT question in the response category: neither likely nor unlikely for the month The number of responses to the FFT question in the response category: unlikely for the month The number of responses to the FFT question in the response category: extremely unlikely for the month

Where no responses fall within a specific indicator a value of zero should be entered. The sum of numbers submitted in boxes FFT001 to FFT006 should equal the sum of those submitted in FFT007 to FFT012, i.e. the sum of numbers submitted for the response categories should equal the sum for the collection methods. Once you have entered all your data click on 'Submit Achievement Data'. Your submission for the month is now complete.
