4 Diagnostic Software, Service Manual HL7 The HL7 module of the custo diagnostic-system is implemented as from version 3.5 and can be used for import/export. Activation is made in the system installations for the office EDP connection. Currently, communication is being configured exclusively in files. A TCP/IP-connection will be available in one of the next versions. 1) Installation The installations of the HL7 module are made in the ServiceCenter. Here, the following options can be installed: Specification file of HL7 standard, pre-installation is version 2.3 Processing of order messages (ORM's) can be switched on and off. Import path, file name and file extension (type) Automatic counting up of import file names Automatic deletion of the import file Export path, file name and file extension (type) Automatic counting up of export file names Automatic deletion of the export file Semaphore control for the file communication via semaphore files 2) HL7 Specification By default, the HL7 module applies the HL7 standard specification of version 2.3. This specification is defined in the file CustoHL7MsgDef.dat and can be adapted to the respective CIS (Clinical Information System) individually. Thus specific deviations of the CIS from the HL7 standard can be considered. 4/ V01 DK-0729 custo med GmbH

Import of patient master data If patient master data are imported, the following ADT messages from the HL7 standard will be supported: ADT A01, stationary admission ADT A03, discharge ADT A05, pre-stationary, post-stationary and ambulatory admission ADT A08, change data ADT A11, cancellation of ambulatory, stationary and partially stationary cases ADT A13, cancellation of discharge ADT A34, merged personal data ADT A38, cancellation of pre-stationary/post-stationary cases Patient master data are always imported in the background. Thus the process does not affect work with the system. Orders, ORM messages By means of ORM messages (orders) the CIS can cause a new measurement with the custo diagnostic system. All orders received in the background are summarized in a waiting room list and can be activated from there with just a mouse click. A direct activation of a measurement in the custo diagnostic system on receipt of a HL7 order is not implemented, as HL7 messages do not arrive synchronously with the work on the system. The receipt of these messages can be switched on or off in the HL7 module. If the CIS does not send any orders (ORM s), measurements in the custo diagnostic system can only be activated manually. However, there is the option to receive the orders manually in the waiting room list. In the event of manual input, the data relevant for export (case number of the patient and/or order number of the measurement) must be registered in the end dialogue, in the further program process. custo med GmbH V01 DK /6

6 Diagnostic Software, Service Manual HL7 4) Export Export is made analogically to the already existing office EDV export via GDT/BDT, in the end dialogue, i. e. when finishing an evaluation display. The procedure of an export transaction can be adjusted. The following options can be selected: - automatic export only when exiting the evaluation for the first time - automatic export with each exit of the evaluation - export the evaluation depending on the installation, to be switched on or off with each exit of the evaluation The export (ORU_R01 event) of the data is made in OBX segments, on the basis of HL7 standard. Analogically to the GDT/BDT export, the following data are exported: Patient master data with case and order number (PID, PV1) Order information (ORC) Evaluation information (OBR) The findings entered by the physician Compressed summary table Summary table Single measurement values (OBX) with abbreviation, value, unit, upper and lower limit as well as standard value (as far as available) Graphics and sections of the print-out as a link to PDF file(s) in the export directory. Except for the patient master data and the evaluation information, the contents of the export file can be configured freely. The installations are saved as examination- and user-specific. The measurement values can be arranged individually or can all be exported. For the export under HL7 an order number is always required. This number is sent by the CIS as an ORM message (orders). If it lacks, the user has to enter the order number manually into the end dialogue, unless it has already been registered as a manual order in the waiting room list. 6/ V01 DK-0729 custo med GmbH
