1 Carnegie-Stout Public Library 360 West 11th Street Dubuque, Iowa (563) office (563) CARNEGIE-STOUT PUBLIC LIBRARY REQUEST FOR PROPOSALS FOR HVAC MAINTENANCE The Carnegie-Stout Public Library, 360 W. 11 th Street, Dubuque, Iowa, requests proposals from qualified contractors for a total coverage maintenance agreement for heating, ventilating and air conditioning system maintenance as listed in Schedule A. Proposals must be received no later than Monday, June 12, 2017, at 11am, at the Administrative Offices of the Library at the above address. All proposals must be signed, sealed, and marked on the outside with HVAC Proposal. Contract period will begin July 1, 2017 and extend until June 30, The Library Board of Trustees reserves the right to reject any and all proposals. A tour of the Library building can be arranged by calling Dave March, at (563) , ext or the Administration Office at (563) Section A: Specifications The Board requests proposals for a three (3) year contract for periodic total maintenance coverage on the Library s heating, ventilating, and air conditioning equipment. Such maintenance will include: Detail maintenance is provided in each attachment taken from the operations manual for each piece of equipment. A summary of this

2 information is provided in attachments one through eight (1-8). Full operations and owner s manuals are available for review in preparation of the request for proposal and vendors are invited to add or suggest coverage items on the equipment. The Library has recently undergone a major renovation and most equipment listed on Schedule A is under 6 years old. Vendors should provide an estimated recommended life expectancy of equipment, which will allow the Library to develop a replacement schedule accordingly. Regular and systematic examination, adjustment, lubrication, and calibration of all thermostats, humidity controls, temperature controls, pressure controls, valves, relays, motors, and accessories directly related to the temperature control systems listed in Schedule A. Regular and systematic lubrication of such components as air compressors, damper bearings, linkages and switches directly related to the temperature controls systems listed on Schedule A. Regular and systematic examination, adjustment, calibration, and lubrication of all equipment listed on Schedule A. Lubrication of materials for equipment listed on Schedule A. Boilers as listed on Schedule A: Maintenance inspections will include but not be limited to: Boilers as listed on Schedule A. Tasks related to preparing the boilers for inspection and tasks related to getting boiler back on line after inspection. Coil cleaning is done by Library Maintenance Staff one to three times per year or more often if needed. Training and or assistance for the cleaning to be provided by HVAC vendor as needed. Filters are purchased and replaced by Library Maintenance Staff. Training or assistance for replacement of filters to be provided by HVAC vendor as needed.

3 Emergency and trouble call coverage to be provided 24 hours a day 7 days a week, 365 days a year included. A minimum of four to six times a year for mechanical planned maintenance service on equipment listed and described in Schedule A and as prescribed by manufacturer s recommendation, equipment application and equipment run time. Planned maintenance will include, but not limited to: cleaning, adjustment, lubrication and calibration of all equipment, automatic temperature control service, operational analysis of the system, and cleaning of equipment. Parts that are worn, failed or have doubtful components and parts will be replaced with like or current design to minimize system depreciation and obsolescence. Repair, replacement, and emergency services provisions apply to the systems and equipment listed on Schedule A. Service documentation will include all scheduled and unscheduled service work showing the time, date, name of service technician, equipment identification and description of work. BOILER/CHILLER WATER TREATMENT This agreement does not cover any water treatment or testing for the boiler or cooling tower. 4 C. Qualifications including a list of past clients, and description of similar experience. D. Names and background of personnel assigned to project. E. Description of scope of services. The Library s hours of operation are Monday through Wednesday 9am to 8 pm, Thursday, 12 pm to 8 pm; Friday, 9am to 5pm; Saturday, 9am to 5pm; and Sunday, 1pm to 5pm.
