10 VIRGINIA DEPARTMENT OF TRANSPORTATION SPECIAL PROVISION FOR ASPHALT PATCHING OF EXISTING PAVEMENT October 28, 204 I. DESCRIPTION This work shall consist of repairing specified sections of existing pavements by removing all or part of the defective materials in the sections and replacing them with hot mix asphalt (HMA) paving material. The locations of the repairs will be specified in the Contract documents and specific locations as directed by the Engineer. II. SCOPE OF WORK Patching repair shall consist of the removal of areas of unsound pavement material as indicated on attached patching locations and/or as determined by the Engineer and replacing these areas with hot mix asphalt (HMA). Asphalt Patching shall consist of the removal of specified areas of the remaining asphalt thickness of the pavement section and replacing such areas with HMA as specified in the Contract documents or as directed by the Engineer. In the event an asphalt patch fails prior to overlay, the Contractor will be responsible for removing and replacing the failed patch at no cost to the Department. III. MATERIALS All hot mix asphalt (HMA) shall conform to the requirements of Section 2 of the Specifications and shall be IM-9.0E as specified within the Contract documents, unless otherwise approved by the Engineer. IV. EQUIPMENT Material in the areas identified for existing asphalt pavement patching may be removed by a milling machine, backhoe, or other excavating equipment as approved by the Department. V. PROCEDURES Asphalt patches shall be placed in accordance with the requirements of Section 35 of the Specifications. The existing defective pavement shall be removed with a minimum disturbance to the base material and the faces of the remaining pavement shall be cut to a smooth, vertical face without ragged edges. The existing pavement shall be removed by planning (milling), grinding, saw cutting or any other approved method to a minimum depth of 2 inches within the full perimeter of the designated area. Repair site shall be thoroughly cleaned of all loose debris and dust. A tack coat of CRS-2 (or other asphalt material approved by the Engineer) at a rate of 0.2 gallon per square yard shall be applied to surface and vertical faces of exposed pavement. Exposed base material shall be primed with liquid asphalt CRS-2 at an application rate of 0.4 gallon per square yard. 40

11 VIRGINIA DEPARTMENT OF TRANSPORTATION SPECIAL PROVISION FOR SECTION SUBCONTRACTING (FEDERAL FUNDED PROJECTS) July 2, 206_ SECTION SUBCONTRACTING of the Specifications is amended to include the following: (d) According to Commonwealth of Virginia Executive Order 20, the Contractor is encouraged to seek out and consider Small, Women-owned, and Minority-owned (SWaM) businesses certified by the Department of Small Business and Supplier Diversity (DSBSD) as potential subcontractors and vendors. Further, the Contractor shall furnish and require each subcontractor (first-tier) to furnish information relative to subcontractor and vendor involvement on the project. For purposes of this provision, the term vendor is defined as any consultant, manufacturer, supplier or hauler performing work or furnishing material, supplies or services for the contract. The Contractor and, or subcontractor (first-tier) must insert this provision in each subcontract and further require its inclusion in all lower tier subcontracts (excluding purchase orders, rental agreements and other agreements for supplies or services). The applicable requirements of this provision are incorporated by reference for work done by vendors under any purchase order, rental agreement or agreement for other services for the contract. The Contractor shall be responsible for compliance by any subcontractor, lower-tier subcontractor or vendor. The submission of a bid will be considered conclusive evidence that the Contractor agrees to assume these contractual obligations and to bind subcontractors contractually to the same at the Contractor s expense. When an approved Form C-3 Subletting Request is required according to IIM-CD , the Contractor shall indicate on the Subletting Request if a subcontractor is a certified DBE or SWAM business. The Contractor shall report all SWAM, and Non-SWAM/DBE vendor payments annually on June 30th (if not a business day, then the next business day), and within 30 days after final acceptance of the work, to the District Civil Rights Section. The Contractor shall provide the information in a format consistent with revised Form C-6 Vendor Payment Report, subject to the approval of the Engineer. DBE Participation and reporting shall be according to the Special Provision for Section 07.5 (Use of Disadvantaged Business Enterprises). The Contractor shall provide the information in a format consistent with Form C-63 Vendor Payment Compliance Report. If the Contractor fails to provide the required information, the Department may delay final payment according to Specification Section 09.0 of the Specifications. REVISION: FEBRUARY 6,207 42

N. of Rte. 40 (Doyle Blvd.), & from MM 55.5, 0.62 mi. N. of Rte. 605 (Hunnicut Rd.), to MM 6.44, 0.27 mi. S. of Rte. (Boydton Plank Rd.) with concrete patching, removal & replacement of UD-7, & overlay with 3.5 of SMA asphalt in the Richmond District in Dinwiddie County in the Petersburg Residency. Since the existing pavement will be overlaid & the grade is to be raised, all guardrails will be replaced. All work shall be performed within the existing right of way. See the Project Details & Typical Sections portions of the Assembly for additional information on Asphalt Overlay, Planing, Concrete Patching, Shoulder Stone, Guardrail, & Rumble Strips. 17 GENERAL NOTES. The quantity breakdown summaries contained in the plan assembly are only meant to represent a breakdown of selected items. For a complete list of all pay items, refer to the Schedule of Items. 2. All lane closures shall require use of a portable changeable message signs. Portable changeable message sign quantities are based on the use of up to three (3) portable changeable message signs for the duration of the project. Prior to the work zone when people, equipment, or materials are present, a portable changeable message sign shall be placed with the following display: Screen CENTER LINE, Screen 2 REDUCE SPEED TO 50 MPH. Should the contactor see a need to use more than the units & hours specified, a proposed plan shall be submitted to the Engineer for approval before supplying additional units. 3. Group 2 Channelizing Devices shall be used on transitions for lane closures. See SP If, after pavement is planed, roughness is present due to spalling, crumbling, potholes, etc., which could contribute to future deformation once asphalt is placed, spot repairs shall be made prior to opening to traffic. The Department shall have a minimum of one hour after the contractor has notified it that the milled area has been swept to examine the work area and notify the contractor that the roughness will require spot repairs. The type of pavement repair is to be determined by the Engineer at the time of discovery. 5. In an effort to avoid traffic control conflicts that may arise between this and any adjacent ongoing or planned projects, the contactor shall provide the VDOT Engineer with lane closure schedules a minimum of two weeks prior to the planned work to begin & updated every Thursday. No lane closures that impact adjacent ongoing or planned projects will be permitted without proper notification & approval by the Engineer. 6. Cost associated with removal of grass & shoulder debris shall be included in the bid price for other items. 7. In the event paving operations extend into a second paving season, the contractor shall ensure at the end of the first paving season that all work is complete on any location where work has commenced, including guardrail placement, asphalt curb & back-up material, patching, planning, paving all lifts in accordance with the appropriate typical sections, permanent pavement markings, rumble strips, shoulder stone wedges, etc. 8. Rideability applies to the mainline travel lanes of all routes within the contract. 9. The Contractor shall grade, seed, & mulch all disturbed areas. All costs associated with grading, seeding, & mulching shall be included in the bid price of other items. 5

18 0. The use of crossovers on interstate routes shall not be allowed without written permission from the Department. If it is desired to use the crossovers, the contractor must request in writing permission to use the crossovers, & describe the location of each crossover & circumstances the crossovers will be used during the life of the contract.. The pavement will be tied down before & after all in-line bridges to prevent the concrete bridge decks from being planned or overlaid. The pavement will also be tied down & the surface planed & replaced under overhead bridge structures to prevent a reduction in the overhead clearance of the roadway. The pavement will also be tied down at the project termini. All tie downs shall be in accordance with the Asphalt Concrete Overlay Transitions (ACOT-) specification. 2. The contractor shall set-up all lanes, shoulders, ramp closures, & detours, & provides all construction/work area protection signs & traffic control devices, to include aluminum & roll up signs, portable sign stands, post, mounting hardware, barrels, cones, arrow boards, & truck mounted attenuators. Cost for signs & other traffic control devices shall be included in Lump Sum MOT. Reference Special Provision Section 52 Maintaining Traffic. 3. All planed surfaces & asphalt pavement in excess of 220 lbs per square yard (2 ) shall be squared up at the end of each shift. All lanes shall be squared up before week-ends or holidays. 4. When paving adjacent to entrance & exit ramps, Contractors shall coordinate his paving work so as not to leave any longitudinal joints/lips to be crossed by motorists entering or exiting the Interstate following each days operations. Any joints/lips to be crossed at interface between mainline lanes and entrances or exit ramps shall be transverse (perpendicular) to the path of motorists. 5. Aggregate Material No. 2A or 2B will be utilized for both shoulder wedging and shoulder restoration at guardrail locations. 6. A temporary 2: safety wedge at the shoulder shall be applicable to areas where the planing edge exceeds 2, & spans the entire length as directed by the VDOT Engineer. The cost shall be incidental to other items. 7. The paving contractor awarded this project shall proceed forward with all planned work through the area of the VDOT Traffic Engineering Division s traffic count loop sensors (do not skip over sensors). Once the project is awarded & the paving contractor is allowed to begin construction, please coordinate with the Traffic Engineering Division a minimum of 4-6 weeks prior to work. The VDOT Central Office Traffic Engineering Division, Traffic Monitoring Section, will incur all costs associated with reinstallation of in-road sensors that are damaged during paving operations. Their contractor, presently Digital Traffic Systems Inc., will perform all necessary sensor reinstallation work after the contractor has completed planing, & prior to the final course being applied. They require a minimum of 2 nights per lane to complete roadway sensor installations. The paving contractor shall take this requirement in consideration when planning their schedules. 19 Traffic Engineering, Traffic Monitoring Section Contract Administrator Brooke Kordas at phone , via or Toll Free / VDOT will then put the paving contractor & sensor repair contractor in contact with each other to discuss scheduling & lane closure timeframes. 8. In addition to contacting Miss Utility per specifications, the Contractor will be responsible for marking VDOT underground utilities. The Department will provide the information necessary for the locate work to be accomplished (location of services, routing of cables, etc.). One () week notice is required prior to delivery of information. A current list of contacts will be provided at the pre-construction conference. The contractor will be responsible for repairing any damage due to a failure to locate, or improperly locating, identified VDOT facilities. The cost for this work is to be included in the price bid for other items. Contractor to call Richmond Traffic Operations Center (TOC): 804/ at least 48 hours prior to the beginning of work in order to locate ITS facilities. 9. All in-line bridge approach/runoff slabs (approximately 20 ft length each end) with an existing asphalt surface layer to be planed.5-2 to the concrete slab surface & repaved with asphalt surface mix as appropriated to each lane type (ramps, shoulders, mainline travel lanes, etc.). Planing depth & asphalt overlay application rate to be determined in the field or as directed by the Engineer. 20. The Contractor shall maintain positive drainage. The Contractor s planing & paving sequence of operations shall be planned & executed to maintain positive drainage & safe conditions during construction. 2. Protection of Items It shall be the Contractor s responsibility to protect all items in the roadway, shoulders, & Right-of-Way to include, but not limited to, drop inlets, drainage structure grates, drains, electrical junction boxes, manhole covers, & any adjacent concrete items. Failure to protect such items will result in their replacement &/or a thorough cleaning to the satisfaction of the VDOT Engineer at no additional expense to the Department. 53

20 GUARDRAIL GENERAL NOTES. All guardrail locations shall be field reviewed by the Contractor and the Department together prior to any work starting. The Contractor shall provide 72 hours notice prior to field review. 2. Hazards created by the removal of existing guardrail shall be left open for a period not to exceed four (4) days. The maximum length of guardrail to be removed at any given time shall not exceed 3/4 mile (3960 feet) in length. 3. Once work has begun on a section of guardrail, the work shall be continuously prosecuted to completion. The contractor shall use whatever means necessary to ensure that no blunt ends are left exposed to traffic during non-working hours. a. If, due to unforeseen circumstances, a section of guardrail cannot be completed in a given day, then the contractor shall continuously utilize a TMA to protect any run-on blunt ends or hazards and shall continuously utilize Group II Channelizing Devices in any uncompleted run-off sections as directed by the Engineer at no additional cost to the Department. b. In addition to the use of TMA s and Group II devices as describe above the contractor shall have in place a shoulder closure in accordance with TTC 4.0 of the 20 Work Area Protection Manual in areas of uncompleted guardrail as directed by the Engineer at no additional cost to the Department. 4. During non-work hours all areas in front of guardrail and within its deflection shall be free of any equipment and materials. 5. The contractor shall install GR-9 s with a maximum flare of one foot ( ), unless otherwise directed by the Engineer. 6. Bridge guardrail attachments (BRGR s) and fixed object attachments (FOA s) are designated as: Type I run-on and Type II run-off. 7. The Contractor shall field verify all materials for BRGR s and/or FOA s prior to ordering any materials. 8. Any postholes left as a result of removing existing guardrail in areas of concrete or asphalt shall be patched using material approved by the Engineer. Guardrail postholes shall be backfilled to the ground line with approved material placed in layers not more than 4 inches in height. Each layer shall be compacted by tamping. All cost for this work shall be included in the bid price for guardrail. 9. Cracks in the shoulder as a result of driving or removing posts shall be repaired. In asphalt paved or surfaced treated shoulders, cracks and voids around posts shall be filled, compacted and sealed with fine asphalt plant mix no larger than SM-9.5A. All cost for this work shall be included in the bid price for guardrail. 54

In areas of asphalt curbing, asphalt curb backup material or concrete curbing; any damage as a result of removing existing guardrail or installing new guardrail shall be repaired using material approved by the Engineer. All repairs shall conform to the original shape, line and grade as possible. All cost for this work shall be included in the bid price for guardrail.. Concrete spalls or holes that are left or created by removal and or attachment of fixed object attachments (FOA s or BRGR s) must be repaired or filled and all costs for this work shall be included with the price for fixed object attachments. 63 TRAFFIC MANAGEMENT PLAN INTRODUCTION Richmond District Maintenance proposes flexible pavement planning, concrete patching, shoulder stone, add UD-7, rumble strips, upgrade guardrail, with 3.5 asphalt overlay on the mainline on I-85 from MM to MM NB and from MM to MM 6.44 NB. All work shall be done in accordance with the Virginia Work Area Protection Manual and the Limitations of Operations. TEMPORARY TRAFFIC CONTROL PLAN General Notes The length of the project is 7.8 miles. The following traffic control specifications from the Virginia Work Area Protection Manual will be used: TTC-4, TTC-6, TTC-7, TTC-37, TTC-53, TTC-57, & TTC-58 as required. The traffic volume I-85 NB from MM is 2,082 VPD (205), & from MM is 3,705, & includes a wide variety of traffic types including trucks, buses, commuters, travelers, & residents. There are no identified areas within the right of way for the contractor to store equipment & materials. The contractor must request in writing permission to use the ROW and to describe the location of each ROW area and under what circumstances the ROW will be used during the life of the contract. No ROW usage will be permitted without proper notification and approval by the Engineer. Special Details There are no special details for this project that are not addressed in the Virginia Work Area Protection Manual or Limitations of Operations. TRANSPORTATION OPERATIONS PLAN ) The process to notify the Transportation Operations Center to place lane closure information on the 5 system and VA Traffic will be: a) Contractor is to provide the VDOT project inspector &/or Construction Manager with a tentative lane closure schedule a minimum of two weeks prior to the planned work to begin & updated every Thursday. b) Construction Manager to advise the Resident Administrator of proposed lane closure. The Resident Administrator is to have VA Traffic operator enter data into VA Traffic, and also advise Transportation Operations Center. 2) The following is a list of local emergency contact agencies: Virginia State Police (804) or or #77 Cellular Haz-Mat Center (if spill involved) - 9 3) Procedures to respond to traffic incidents that may occur in the work zone: a) Contractor to notify Virginia State Police and VDOT Inspector in charge and Smart Traffic Center. b) Depending upon severity of incident, contractor may have to shut down work. c) Upon arrival on scene, Virginia State Police to determine response necessary to allow traveling public around incident. d) Inspector to notify Construction Manager/Resident Administrator of incident and take pictures as necessary, especially pictures of contractor s work zone to verify the proper setup. 4) Process of notification of incident to be followed is: 97
