2 Introduction 26 brought exciting changes and recognition for the Clive Fire Department. 24-Hour Staffing - A major change in operations commenced on September 1 st with 24-hour staffing. This change utilizes paid-on-call personnel staffing a minimum of one twelve-hour shift per week. Historically, paid-on-call staff have typically responded from home or work, with a few in-station positions being staffed during daytime hours. This change enhanced our ability to provide emergency services to the community of Clive in two primary ways: 1.) We are able to guarantee an initial response of an ambulance and an engine. Prior to this change, we frequently struggled with staffing an engine during evenings and weekends. 2.) Response times at night were improved considerably. Using a national standard benchmark of response times for ninety percent of all calls, we improved our night fire response (first engine or ladder on scene) by four and half minutes and improved our night advanced life support response (first ambulance or paramedic) by one and a half minutes. Career EMS Service of the Year - In October of 26, the Clive Fire Department was recognized as the Career EMS Service of the Year in Iowa by the Iowa Emergency Medical Services Association. This award was the result of a lot a hard work, a wonderful group of personnel who are open to change and improving service, consistent efforts, and a genuine desire by our staff to be the best. It is also a strong reflection of the vision and desire by the City Manager, Mayor, and Council Members to support EMS in the City planning process. On November 1 at the Iowa Emergency Medical Services Associations annual Conference and Trade Show, The Clive Fire Department was recognized as the Career EMS Service of the Year for 26 in Iowa. After a nomination process, this prestigious award is given to one ambulance service each year for extraordinary services to the community. The competition for this award is formidable but in 26, the Clive Fire Department improved ambulance and fire truck response times by implementing a unique in station staffing plan that places 6 people in the fire station, poised to respond rapidly twenty four hours a day. This change has significantly reduced response times to those needing emergency care or a fire truck. Additionally, the EMS division has rolled out a unique quality assurance and improvement program to assure that the citizens of Clive are getting the best care and possible in their time of need. This program ensures that each and every patient contact is subjected to a peer review process that provides positive feedback to the EMT or Paramedic. The process builds on strengths and exposes weaknesses in order to ensure top performance from all EMTs and Paramedics. The Clive Fire Department is very proud of these achievements and is proud to have been recognized as the 26 Iowa EMS Service of the year. Asst. 10 Year in Review January City of Clive All-Hazards Emergency Operations Plan A comprehensive update to the All-Hazards EOP was completed. This work greatly enhances our preparedness planning and operational assignments. In January, the Council took formal action to adopt the updated plan. Pandemic Awareness Training Asst. Chief Brad Madsen provided pandemic awareness training for the City Manager and Department Directors. The training reviewed the history of pandemics worldwide, the focus and concern on the Avian Bird Flu, the precautions staff should take on an everyday basis to protect against normal seasonal type illnesses, and a conceptual discussion on how pandemic events can impact City operations. Recruit Class Graduation The West Metro Fire Training Academy Graduation was held in January at Valley High. Clive graduated three recruits. The academy requires five months of classroom and practical training, meeting twice a week and a few weekends, and completing certifications as Fire Fighter I, Fire Fighter II, Hazardous Materials Operations, Weapons of Mass Destruction Awareness, and the Candidate Physical Ability Test. Graduates for Clive were Mindy Bruecken, Neil Weglarz, and Beau Feris. Residential Fire Urbandale, January 8 Clive assisted Urbandale with a residential fire in the 7 Block of Horton. required extrication. All three passengers sustained significant injuries. Apartment Fire Urbandale, February 26 Clive assisted Urbandale with an apartment fire in the 32 Block of 86th. Hazardous Materials Assignment Clive, February 27 Clive and Des Moines Fire Department Hazardous Materials Teams responded to the Des Moines Water Works Storage Facility (near th ) for a reported chlorine leak. Two employees found the leak and one encountered enough fumes to be transported to the hospital. A 15 lb. cylinder of chlorine was leaking inside of the main building. HazMat Technicians were able to secure the leak and the building was safely ventilated. March Death of Police/Fire Chaplain Dr. David Goodwin After a short, but difficult battle with cancer, Police/Fire Chaplain David Goodwin passed away in March. We were fortunate to have Chaplain Goodwin part of the Clive family for three years and benefited enormously from his compassion and support. New Fire Engine Clive took delivery of a new 26 Toyne 15 GPM engine. The engine is a rescue engine style with high side and top storage for extrication and technical rescue equipment. This engine replaced a 1985 Seagrave, which was sold to a small community in Missouri. Residential Fire West Des Moines, January 1 Clive assisted West Des Moines with a residential fire in the 5 Block of 34 th Street. Apartment Fire Urbandale, January 2 - Clive assisted Urbandale with an apartment building fire in the 87 Block of Meredith. February City of Clive All-Hazards Emergency Operations Plan Training Overview training of the updated All- Hazards Emergency Operations Plan was delivered to City Staff and Elected Officials. Extrication Accident Clive, February 19 Clive crews responded to a motor vehicle crash in the 2 Block of 86 th Street. Two vehicles collided head-on. The driver and passenger of one vehicle both Clive Virtual Vision Lt. Gene Schaller, Denise Harlan, and John Schiefer, staffed a Fire Department display at the 2 nd Annual Clive Chamber of Commerce Virtual Vision Tour, Tuesday night, at the 7-Flags Events Center. They coordinated a presence for the Clive Fire Department, encouraging AED implementation in businesses. Page 1 of 16

11 FF/Medic Adam McGilvra to Iraq Firefighter/Medic Adam McGilvra and his company were shipped to Iraq for a month tour. Sgt. McGilvra is a NBC NCO (which stands for nuclear, biological, chemical, non-commissioned officer). Residential Fire Urbandale, March 7 Clive assisted Urbandale with a residential fire in the 158 Block of Ridgemont. Apartment Fire Clive, March 15 Clive crews responded to an apartment fire at 8415 Franklin Avenue. One apartment sustained fire, heat and smoke damage from a small fire. The fire was accidental and caused from incense burning sticks that were not properly secured. Assisting Clive were the Windsor Heights and Urbandale fire departments. Damage was estimated at $5. Arson Fire Clive, March 19 Clive responded to a reported house fire in the 4 Block of Hammontree. A fire was out upon arrival and apparently started from juveniles playing in the area. Clive PD located two juveniles who were responsible for lighting the fire. Commercial Fire Clive, March 3 Clive responded to a fire at the Briggs Corporation. A paper bailer had caught fire and charged a warehouse/processing area with smoke. Fire crews were able to cut power to the machine and overhaul a smoldering paper fire. Responding to assist Clive were the Windsor Heights and Urbandale Fire Departments. Damage was estimated at $3,. Residential Fire Clive, March 3 Clive responded to a residential fire in the 14 Block of 91 st Street. A family of five was able to safety evacuate after smoke detectors alerted them of a fire. Crews were able to quickly extinguish a small fire in the basement. Although the fire damage was minimal, there was smoke damage throughout the house. April Officer Promotional Assessment Center After an officer promotional process, two officer appointments were made. John Fiedler was promoted to EMS Training Officer and is responsible for EMS Training. Kyle Frizzell was promoted to Safety Officer and is responsible for the Safety Committee and related departmental safety efforts. Easter Egg Hunt On April 15 th, the Clive Fire Fighter s Association hosted the 4 th Annual Easter Egg Hunt at Linnan Park. The weather was perfect and a large crowd enjoyed the activities. Those children finding specially marked eggs were able to claim a small prize. Prior to the hunt, a Clive Fire Department family breakfast was held. Conclusion of MetroCom Efforts After a two year effort to structure a consolidation of Polk County and WestCom communication centers, the project was deemed not feasible at this time for financial and political reasons. Encountering Trauma: In the Field and in the Clinic On April 2 th, the Clive Police/Fire Chaplains (PJ McDonald and Terry Wilkinson), along with Chief Cox and Chief Roe, made a presentation on Clive s Chaplain program at the National Association of Social Workers Conference in Urbandale. Chaplain PJ McDonald coordinated the presentation and focused on the uniqueness of trauma in fieldwork as compared to a clinical setting. Biological Hazard Scare Des Moines, April 5 Clive assisted Des Moines Fire Department Hazardous Materials team at a suspicious powder scare at 5 Mulberry. Clive houses one of two Mass Casualty Trailers in Polk County. Each trailer includes a large tent that can be used for patient treatment or private decontamination. Residential Fire Urbandale, April 8 Clive crews assisted Urbandale with a house fire in the 33 block of Melanie. Residential Fire Windsor Heights, April 18 Clive crews assisted Windsor Heights with a house fire in the 2 block of 69 th. Residential Fire Windsor Heights, April 24 - Clive crews assisted Windsor Heights with a house fire in the 17 block of 69 th. Residential Fire Windsor Heights, April 25 - Clive crews assisted Windsor Heights with a house fire in the 15 block of 71 st. May CFFA Funding Station Improvements Work began on station improvements, under a project being funded by the Clive Fire Fighter s Association. In preparation for 24-hour in-station staffing (beginning in September of 26), the kitchen area was enhanced with additional cupboards, new countertops, a new refrigerator, and dishwasher. The CFFA provided $3, for this project. Page 11 of 16

12 Cardiac Care Changes - Because of changes in the American Heart Association standards of cardiac care, the following operational changes were initiated: Complete revision of the Clive Fire Department EMS protocols. New medications were added and some medications removed. CPR guidelines were changed as well as AED/Defibrillation guidelines. U updating of all City of Clive AED software. EMS Week Activities - EMS Week activities included recognition of staff, neighboring EMS/Fire Agencies, WestCom Dispatch, and the Clive PD. An EMS Week Cookout was held with approximately 3 people in attendance. Holmes Safety Safari The Clive Fire Department participated in the annual Holmes Safety Safari on Saturday, May 2 th. Firefighters and medics assisted with the Blank Children s Hospital Fire Safety House and displayed fire and EMS apparatus. Residential Fire Windsor Heights, May 1 Clive assisted Windsor Heights with a fire in the attic of a single-family residence (12 Block of 71 st ). Commercial Fire Grimes, May 7 Clive crews assisted Grimes at a newly constructed commercial building fire (12 Block of NW 54 th ). Vehicle Fire Clive, May 15 Clive crews responded to a vehicle fire in the 115 Block of Hickman Road. The vehicle sustained $2, damage. Hazardous Materials Spill Urbandale, May 2 - Clive crews assisted Urbandale with a herbicide spill in the 42 block of 69 th. The spill originated from an overfilled tank. Vehicle Fire Clive, May 22 - Clive crews responded to a vehicle fire at Hickman and 156 th Street. Vehicle damage was estimated at $2,. June Clive Festival The Clive Fire Department was very involved in the annual Clive Festival activities, including Wednesday night set-up, Thursday safety inspections, Friday volunteers and EMS staff, Saturday assistance with the bike ride and fireworks. Machinery Fire Clive, June 5 Clive responded to a machinery fire at a construction site near 9999 University. A tiling machine was heavily involved in fire. Damage was estimated at $15,. Vehicle Fire Clive, June 13 Clive crews responded to a SUV on fire in the parking lot of 121 University. The engine compartment was fully engulfed. Damage was estimated at $4,5. Commercial Fire Urbandale, June 29 Clive assisted Urbandale at an apartment fire in the 79 Block of Dennis. The fire was confined to the kitchen area of one apartment. July Steve Bunce Retirement An open house was held for Steve Bunce on July 29 th to honor his retirement after 3 years of service. He started on July 1, Steve remains very active with the Clive Fire Department as a photographer. Application Update - 19 applicants were tested and interviewed in July for paid-on-call positions. 11 additional paid-on-call personnel were hired. The addition of these personnel was necessary for the 24- hour staffing initiative. Deputy Chief of Training - Lt. Scott Lyon was promoted to Deputy Chief of Training. This position is responsible for coordinating all EMS and Fire training for Clive. The training needs of Fire/EMS have historically been very significant and continue to increase with federal mandates such as NIMS. City of Clive Tornado Table Top Drill - In July, the first tabletop disaster drill was held for the City of Clive. The specific responsibilities of the All-Hazards Emergency Response Plan were put to test in this tabletop disaster drill simulating a tornado causing significant damage in Clive. Vehicle Fire Clive, July 7 Clive responded to a vehicle fire inside of a car wash at 85 Alice Avenue. A passenger car had a small fire in the engine compartment. Damage is estimated at $5. Apartment Fire Urbandale, July 1 Clive assisted Urbandale with an apartment fire in the 45 Block of 86 th Street. Vehicle Fire Clive, July 2 Clive responded to a vehicle fire at the Kum & Go 156 th & Hickman. Urbandale Fire Department was on scene and initiate the fire control. Damage is estimated at $2,. Residential Fire Windsor Heights, July 2 Clive assisted Windsor Heights with a residential fire in the 11 Block of 66 th Street. Page 12 of 16

13 Fatality Fire Clive, July 3 Clive responded to a fire alarm at Kids Company, th Street around 11:3 p.m. The fire arriving crew found a vehicle on fire inside of the building. The automatic fire sprinkler system in the building was very successful in keeping the fire from spreading to the structure. Three sprinkler heads activated to control the fire. One occupant was found in the vehicle and deceased. The Clive Fire Department coordinated with the Clive Police Department in the fire investigation. August Recruiting Open House On August 1 th, a recruiting open house was held at the Harbach Station. Over six potential applicants were interviewed. After the interview and background work, we had four recruits participating in the recruit class. Joint Council Presentation on Fire/EMS Collaboration On August 14 th, a joint meeting of the Clive, Urbandale, and WDM City Councils was held in Clive. The Matrix Consulting Group made a presentation on the results of the Fire/EMS Collaboration Study. The recommendation from this study was to not pursue full consolidation at this time. Funding levels were significantly different between communities and separate Fire and EMS operations in one City were noted as the major challenges. The consultant stated that closest unit response between Clive and West Des Moines may be a feasible option in the near future. Clive s 5 th Birthday Celebration On August 26 th, the Clive Fire Department assisted with the Clive 5 th Birthday Party. EMS and fire apparatus were on display with children s educational activities for crawling low in smoke. Recruit Class Begins On August 29 th, a joint recruit academy for fire and EMS training started. The class is coordinated by Clive and WDM and includes other students from Waukee and Windsor Heights. The course requires nearly eight months of training, averaging two nights per week. At the completion of the academy, staff will have certifications and training at the following levels: Fire Fighter I, Fire Fighter II, EMT- Basic, Hazardous Materials Operations, Candidate Physical Ability Test. Clive had four personnel in the academy. Preparation for In-Station Staffing - August was the final month of preparation leading up to in-station staffing in September. A significant amount of planning and preparation had taken place. Highlights are: Informal discussions with personnel on staffing and response challenges and potential solutions Early Fall 25 - Formal presentation to personnel on an in-station staffing concept. Late Fall 25 - Formal presentation to City Manager, Citizen s Budget Committee, City Council. Early 25 Council approval for budget, approving September 1, 26, implementation. May 25 Department wide staffing survey to determine potential hiring needs. June 25 Recruiting drive to hire approximately 1 additional POC staff, requiring existing certifications/experience with fire and EMS. July 25 Assessment/hiring process. 19 personnel tested and 11 hired. August 25 Training and orientation for 11 new POC personnel. Policy manual updates identified, records management system changes identified, operational changes identified, facility changes initiated, scheduling of 5 POC personnel completed for September, department wide information sharing. September 25 In-Station staffing begins. Commercial Fire Clive, August 7 Clive responded to a reported fire at Boston s Pizza (1241 University). Crews arrived to find light smoke in the building. Further investigation found burning/smoldering cleaning rags in a dryer. The debris was overhauled and we assisted the business with ventilation. Damage was estimated at $825. Hazardous Materials Spill Clive, August 11 Clive responded to a report of an unknown fluid leaking from a semi trailer at Flying J (1182 Hickman). The fluid was producing an unusual foaming on the ground. Des Moines Fire Department Hazardous Materials Team was called to assist. The product was found to be a linseed oil product used to treat cowhides. Hazardous Materials Spill Clive, August 18 Clive responded to a report of an overflow of cleaning chemicals from Iowa Quality Meats (275 NW 92 nd Court). Chemicals were in the street and a small amount entered the storm sewer. The occupant worked with Hydro-Kleen, Iowa DNR, and Clive Public Works to clean the area, including storm sewers. Commercial Fire Johnston, August 24 Clive assisted Johnston during a natural gas fueled townhome fire. Page 13 of 16

14 Vehicle Fire Clive, August 31 Clive responded on vehicle fire at 156 th & Hickman. A 2 Galant was well involved in the engine compartment. The vehicle was a total loss at $6,. No injuries were reported. September Commencement of In-Station Staffing Of utmost significance for the month of September (and probably the entire year of 26) was the commencement of in-station staffing on September 1 st. The goals of in-station staffing were to: 1. Guarantee a minimum response of an engine and an ambulance 2. Enhance response times The minimum response of an engine and an ambulance are guaranteed with our staffing level of six personnel. New Ambulance Clive took delivery of replacement ambulance that was placed into service on September 12 th. Ambulances are on a seven-year replacement cycle. Cost of the vehicle was $118,31. Clive. There were no deficiencies reported and the Bureau of EMS commented on the high caliber of the operation of the EMS division of the Clive Fire Department. Healing Field - The Healing Field was displayed in September at Homesteaders Life at Westlakes Parkway and Westtown in WDM. In addition to assembling and placing nearly 3, flags that will remain on our campus for a week, there was a memorial ceremony on Monday, September 11, 26. Thousands of people from across the Midwest visited during the weeklong commemoration. The Clive Fire Department assisted with set-up and a suspending a large American flag from a ladder truck during the memorial ceremony. Commercial Fire Urbandale, September 3 Clive assisted Urbandale with a reported fire and explosion at a retirement facility (27 block of 82nd). Dumpster Fire Clive, September 6 Clive responded to the 92 block of Swanson for a commercial dumpster on fire, impending on the enclosure and an adjacent building. The fire was easily extinguished. Damage was estimated at $5. Cause of the fire was discarded rags saturated in linseed oil. Vehicle Accident with Extrication Clive, September 9 Clive responded to motor vehicle accident in the 8 block of Hickman Road. One patient with minor injuries required extrication from the vehicle. Video Conferencing System A video conferencing system was implemented that allows Clive, Urbandale, and West Des Moines to share training. The system has state-of-the-art AV equipment that is also very beneficial for everyday training needs. Bureau of EMS Site Visit - The Clive Fire Department passed the periodic (once every 3 years) Iowa Department of Public Health Bureau of EMS onsite inspection, certifying the department to continue to provide EMS service to the citizens of Residential Fire Waukee, September 13 Clive assisted Waukee with a significant house fire in the 8 block of Maple. Vehicle Accident with Extrication Clive, September 2 Clive and Windsor Heights extricated a passenger from a vehicle in the 1 block of 73 rd. The passenger car was literally wrapped around a light pole. The extrication took nearly one hour to complete. Commercial Fire Clive, September 28 Clive and Windsor Heights responded to a reported deck fire at an apartment in the 83 block of Harbach. A deck was on fire and had been primarily extinguished by occupants prior to our arrival. Crews conducted additional overhaul to confirm extinguishment. Discarded cigarettes caused the fire. Damage was estimated at $25. Page 14 of 16

15 Fatal Accident/Vehicle Fire Urbandale, September 29 Clive assisted Urbandale with a motor vehicle accident/fire on I8/35 Southbound, between Douglas and Hickman. October Fire Prevention Week - The Clive Fire Department was very active with this year's National Fire Prevention Week from October 8-14, led by Fire Marshal Tony Collins. This year's theme was "Prevent Cooking Fires: Watch What You Heat". An Open House highlighted kitchen safety tips, fire truck displays, fire prevention week poster contest winner list and posters, and the Blank Children's Hospital Fire Safety Education Trailer. The Clive Firefighters also performed Fire Safety Puppet Shows. Career Lieutenant Promotion - Eric Kallem was promoted to Career Lieutenant, effective October 22, 26. Eric began as a Paramedic/Firefighter with Clive in October of 21. Pandemic Influenza Lecture - On October 19 th, Asst. Chief Brad Madsen spoke to over 4 members of the Iowa Municipal Financial Officers Association at their annual meeting on pandemic preparation for local governments. High Angle Rescue Windsor Heights, October 3 Clive assisted WHTS and West Des Moines Fire Departments with a high angle rope rescue for a construction worker injured on top of SAM s Club. The patient was secured in a basket stretcher and hoisted/lowered to the ground safely using specialized rope rescue techniques. Hazardous Materials Release Clive, October 14 Clive responded to the 19 block of 9 th Street for gasoline that was drained from an automotive gas tank during repairs and washed down the street/storm sewer. Assisting with the incident were the Clive Police and Clive Public Works Departments. Some product could be contained prior to the storm sewer; however, significant quantities had entered the system prior to our arrival. A private contracting firm was hired to flush and recover product from the storm sewer. Fire Investigation Assistance Grimes, October 16 Clive provided two fire investigators to assist with initial investigation efforts in Grimes. A large home had been damaged by fire. at 8415 Franklin Avenue. A cooking fire extended into the wall and produced a fair amount of smoke. The fire was easily extinguished and structure damage was minimal. Damage is estimated at $15,. One occupant was transported for smoke inhalation. Vehicle Accident with Extrication Urbandale, October 24 Clive assisted Urbandale with extricating a passenger from a vehicle in the 79 block of Aurora. Mutual Aid Foam Response Perry, October 25 Clive provided a mutual aid foam response to assist the Des Moines Fire Department Hazardous Materials Team at a tanker rollover, North of Perry on 144. A tanker hauling ethanol was involved in a single vehicle rollover accident. The foam suppresses product vapors and also serves as a cooling lubricant when hazmat crews drill into the tanker for access to the recover the product. Clive crews coordinated the application of 22 gallons of foam (using over 3,6 gallons of water). Clive Public Works provided two pickups for transporting foam. November Clive Fire Department 26 Career EMS Service of the Year - The Clive Fire Department was selected as the Career EMS Service of the Year for 26! West Clivendale Heights Thanksgiving Meal - The annual tradition of the West Clivendale HeightsHeights Thanksgiving Meal continued this year. On the Wednesday prior to Thanksgiving (November 22 nd ), the Clive Fire Department hosted a gathering of our fire/ems neighbors. Each agency provided a portion of the meal, with crews rotating in over a two hour period. MDA Fill The Boot Recognition - The Clive Fire Department/Clive Firefighter's Association was noted in the MDA Winter 26 Newsletter as providing a $ donation through our boot drive at the Clive After 5 events. Bomb Scares Clive Clive Fire assisted with two separate bomb scares during the month. Both events were precautionary based on unusual items found in unusual places. The Clive Police Department was the lead agency, assisted by the Des Moines Police Department Bomb Squad. Apartment Fire Clive, October 19 Clive, WHTS, and Urbandale crews responded to an apartment fire Page 15 of 16

16 Vehicle Fire Clive, November 9 A Jeep was reported on fire at 156 th and Hickman. The fire was contained to the passenger compartment and easily extinguished. Damage was estimated at $4,5. Commercial Fire Urbandale December 8 Clive Fire assisted Urbandale with a commercial building fire. The fire was originating from a commercial trash compactor. Vehicle Fire Urbandale, November 11 Clive assisted Urbandale with a vehicle fire in the 4 Block of Greenview. Commercial Fire Wes Des Moines, November 17 Clive assisted West Des Moines with a fire in a large commercial office building. Vehicle Fire Clive, November 19 Clive responded to a vehicle fire in the area of 86 th and University. A fire was found in the engine compartment and was extinguished by FD personnel and bystanders with portable extinguishers. Damage was estimated at $2,. Residential Fire Clive, November 22 Clive responded to a residential fire in the 141 Block of Lakeshore Dr. Crews found a freezer that had caught fire. Damage was estimated at $1,. Commercial Fire Clive, November 3 Clive responded to the Clive Hotel for a report of smoke. Crews found a cooking fire that created significant smoke. December 44th Annual Santa Day The Clive Fire Department hosted the 44th Annual Santa Day on Saturday, December 16th. Five crews of Santa were busy covering the entire city! It took five hours for the crews to make the rounds. The fire service in rich in tradition and this tradition started in 1962, the year the Clive Fire Department was formed. Regional PHTLS Course in Clive The Clive Fire Department hosted a regional Prehospital Trauma Life Support (PHTLS) course on December 8 & 9. The focus of the program was to improve a victim's chances of survival after traumatic injury by teaching medics how to quickly and effectively intervene and stabilize patients during rapid transport to a tertiary care facility. Recognition and treatment of hypovolemic shock, stabilization of spinal injuries, burn care and the effects of trauma on children and the elderly were all included in the course. Residential Fire Windsor Heights December 1 - Clive Fire assisted Windsor Heights with a significant attic fire in the 7 Block of Bel Aire.
