You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

3 Services PROFESSIONAL SERVICES OF FUNERAL DIRECTOR AND STAFF Basic Services of Funeral Director and Staff Our basic service charge (Non-Declinable Charge) includes, but is not limited to, staff available 24 hours a day to respond to initial request for service; arrangement conference with family or responsible party; preparation and filing of necessary certificates and permits; notification to clergy; placement of obituary notices; planning of funeral arrangements; and coordination of service with cemetery, crematory, vault companies and others as required; staff assistance prior to and following services. Also incorporated in this basic fee is a proportionate charge for professional licensure, legal, accounting, clerical, secretarial and other administrative responsibilities, insurance, property taxes, utilities, maintenance and repairs. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Basic services of funeral director and staff... $4000 Embalming Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. This charge includes disposal of regulated medical waste and compliance with OSHA Bloodbourne Pathogen, Formaldehyde, Hazardous Material Communication and Health and Safety Standards/Regulations. Forwarding of Remains to Another Funeral Home Our charge includes: local transfer of remains to funeral home; basic services of funeral director and staff; embalming and preparation; arranging air transfer with carrier; notification to receiving funeral home; transfer to local airport and necessary authorizations. This charge does not include visitation, rites or ceremonies prior to transfer. 11 Additional Cremation Options All options below include: one trip to obtain death certificate from health care professional and one trip to file the death certificate with health department, local transportation of remains to the funeral home within a 25 mile radius of Glen Burnie, if outside of local area, $3.50 per mile. Additional charges may be incurred for remains in excess of three hundred pounds, out of town transportation and extra personnel for difficult removal situations. The options below do not include cash disbursements or anything not expressly stated as being provided. Direct Cremation... $3,625 Our direct cremation option Includes; basic service of funeral director and staff, local transportation of remains from place of death, to and from crematory, preparation for Identification; basic crematory fee* and minimum cremation container. With Direct Cremation, payment is due in full at the time of arrangements. Memorial Service at Funeral Home... $5,935 Cremation Option B includes; basic service of funeral director and staff, use of facilities for memorial service, transportation to and from crematory, preparation for Identification, register book package (includes 200 prayer cards, register book and 50 acknowledgement cards), a video tribute and memorial portrait, internet memorial obituary, basic crematory fee* and minimum cremation container.
