2 Professional and Staff Services Basic Professional Fee: (Requires Basic Facility Charge) Professional consultation with a licensed funeral director, which may include but is not limited to expert advice on options to meet your needs, including essential services relating to disposition, drafting newspaper & radio notices, planning and scheduling of services, advising regarding possible government death benefits. A basic professional fee is payable whenever we provide services to you. Co-Ordination of Rites and Ceremonies: Co-ordination of all rites and ceremonies, before, during and after they have been provided, including services and supplies provided by us, as well as those provided by a third party, such as flowers and death notices; and co-ordination of donation lists. Documentation: Completion and filing of all documents necessary to carry out the services and supplies requested, including but not limited to, death registration, burial permit, coroner=s certificate for cremation, documentation to ship a body outside of province/country; providing Funeral Director=s Proof of Death Certificate, Clergy Record, Canada Pension Plan Application for Death and Spouse Benefits, and our Help For Bereaved Families Aftercare package. Transport Remains Basic: (Requires Initial Transfer Vehicle) Staff involved in the transportation of the remains from the place of death to our premises, using the initial transfer vehicle, for distances not exceeding 40 kms. (Eg. Nursing home to Funeral Home, Hospital to Funeral Home) Basic Preparation of Remains: (Requires Facilities for Preparation and Shelter of Remains) The preparation of the body for placement into casket or cremation unit, including bathing, dressing and the setting of facial features for identification or viewing without embalming. Other basic duties include confirmation of identification, secure placement and inventory of jewelry and personal effects and check for pacemaker. Embalming of Remains: (Requires Basic Preparation of Remains) Embalming is the disinfection, preservation and restoration of the deceased. This procedure must be completed by a funeral director licensed in Ontario. Embalming is not a requirement by law in Ontario, however it is the policy of our Funeral Home to ask that embalming be completed if the family is requesting an open casket and public visitation, and/or the time between death and burial or cremation exceeds 48 hours. Staff Services for Visitation: Staff to co-ordinate and oversee the visitation or gathering at our premises for a period up to four hours. This may include but is not limited to secretarial staff to take donation, parking lot attendants, and funeral directors to answer questions or direct visitors. When visitation takes place at another location, this includes our staff and their transport to be offsite. Staff Services for Ceremony: Staff to co-ordinate and oversee the funeral ceremony or memorial service, and all that it entails. If the ceremony takes place at an offsite location, this includes our staff to be offsite. The ceremony is generally expected to include three hours from set up to completion. $ $ $ $ $ $ $ $460.00

3 Facilities Basic Facilities: Use of our general and common areas, parking lot, public washrooms, arrangement office, casket selection room, heating, lighting and maintenance. This charge applies whenever we provide services to you. Facilities for Preparation and Shelter of Remains: Facilities used to prepare and/or shelter the body. This includes specialized embalming equipment and embalming chemicals used for the embalming of human remains. Facilities for Visitation: (Requires Staff Services for Visitation) Designated facilities used for visitation for up to four hours. It also includes use of flower stands, easels and frames, slide show equipment, etc. It also includes use of the lounge during visitation periods. Facilities for Ceremony: (Requires Staff Services for Ceremony) Designated facilities at our funeral home used for funeral ceremony or memorial service. This includes but is not limited to chairs, podium, sound system, etc. The ceremony is generally expected to include three hours from set up to completion. $ $ $ $ Equipment, Vehicles & Supplies General Duties Vehicle: This includes the vehicles used for leading a funeral, transferring cremated remains from crematorium to funeral home, delivery of flowers and travel to obtain required documentation. Initial Transfer Vehicle: (Requires Transport Remains Basic) The vehicle used to transfer the deceased body from a local place of death to the funeral home within a 40 km radius. (Please note that a mileage charge of $1.50 per km over the 40 km radius will be charged for one way. This includes a specialized removal stretcher and personal protection equipment necessary to accomplish any type of body removal and is available 24 hours/7 days a week. Funeral Coach: Use of the funeral coach (hearse) for transportation of the deceased on the day of the funeral. Van: Use of a van in lieu of funeral coach for transportation of the deceased from the place of funeral to the cemetery or crematorium. Funeral Stationery: A completed register book, thank you cards, and pallbearer cards, and donation mailing labels. Funeral Equipment: Use of Chairs, Podium, easels, flower stands, church truck, etc. which includes delivery and pick up. $ $ $ $ $ $

4 Other Services & Supplies Nursing Home Visitation Period: If visitation is requested at a location other than the funeral home for part of a visitation period, this charge applies. (eg. Nursing home or related complex such at Nithview Home, Knollcrest Lodge, etc.) Limousine Rental: We do not include a Limousine in our service charge but we are glad to make the arrangements to rent one on your behalf. This would be considered a cash disbursement and the cost for such would be determined by the Limousine company. In Memoriam Cards: We have a selection of cards available that match our register books or you can personalize it with a photograph. These cards can have the obituary, a poem, and a colour photo. The setup fee is $50.00 plus $.35/copy. These are usually printed Ain house@ which allows us to print more as needed. Laminated In Memoriam Card Bulletins: Setup and printing are available through our funeral home. We do not stock bulletins at the funeral home, but encourage families to purchase bulletin covers from a Stationery Supply store, or we can create a bulletin on semi gloss white paper. Our setup fee is $50.00 plus $.50/copy. Laminated Bookmarks: We have a selection of bookmarks available which would have the obituary on one side and a poem or verse of your choosing on the other side. (No setup fee) Reception Room: Our funeral home is available for post-funeral receptions. The room can accommodate up to 100 people and the cost includes a hostess to serve, dishes, cutlery, coffee, tea, juice, cream & sugar. Food may be brought in on platters by the family, a church group or a caterer of your choosing. (This does not include the cost of food) Cash Disbursements: Cash disbursements are not direct funeral home costs but costs related to the funeral such as flowers, clergy honourarium, newspaper notices, cemetery or crematorium expenses, etc. Our funeral home will pay most cash disbursement if instructed to do so by the executor. This service is provided with no additional cost to the family. 15 Rental Casket Price List A rental casket is a specialized casket that has an interior pine box that is removed after the visitation and funeral. Each interior box comes with a new cloth interior & pillow for each person using the rental casket. It also comes with a wooden lid. The exterior of the casket may have been used before, and may be used again.
