3 General Price List The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Basic Services of Funeral Director and Staff and Overhead... $1,895 Our fee for the basic services of funeral director and staff includes but is not limited to: staff to respond to initial request for service, arrangement conference with family and responsible parties, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and coordination with those providing other portions of the funeral, (e.g., cemetery, crematory, vault companies and others as required). Also included in this charge are overhead expenses relative to our facilities such as: sheltering of remains (up to 5 days), professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and administrative expenses This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Care of the Family & Guests Facilities, Staff & Services A. Visitation or Gathering at the Funeral Home (Monday Friday, up to 4 hours) $ 495 Our services will include set-up, display of floral arrangements, supervision during the visitation or gathering and use of our facilities or staff & equipment for visitation or gathering at another facility. B. Private family identification at the Daniels location of your choice (with Trayview Container).... $ 594 C. Funeral, Life Celebration Service or Gathering at the Funeral Home (Monday Friday).... $ 495 Our services include coordinating the arrangements, supervision & direction of funeral, life celebration or gathering, supervision of committal or other disposition service, and use of facilities or staff & equipment for ceremony at another facility. D. Memorial Ceremony or Life Gathering at Funeral Home or Other Facility (Monday Friday)... $ 495 Our services include coordinating the memorial service, supervision of the memorial service and setup and use of facilities for memorial service or additional staff necessary to conduct memorial service or life gathering at another facility. Graveside Service (Monday Friday)..... $ 495 Our services include coordinating the funeral arrangement at graveside, supervision of the graveside service, equipment and staff necessary to conduct the graveside service. Santa Fe National Cemetery Committal Service $ 495 Our services include coordinating and verification of interment eligibility, supervision of the graveside service, equipment and staff necessary to conduct the graveside service, as well as scheduling of military honors. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 5 Cremation Options Formal Gatherings Funeral Ceremony The Funeral Ceremony follows prescribed formal religious rites. Your loved one is present in the casket and the format of this ceremony will be determined by the traditions of your chosen faith. The Funeral Ceremony package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care of your loved one including embalming, cosmetics, casketing & the cremation process, use of our staff, equipment & facilities for visitation and services in our chapel or your place of worship, and required professional vehicles. Your selections for your loved one s casket, memorial urn, personal keepsake, permanent placement and available catering services will be added to the package cost. $ 4,499 Life Celebration The Life Celebration, while still a more formal event, provides the opportunity to honor your loved one s life in a less structured way than the Funeral Ceremony. Your loved one is present in the casket and the Life Celebration will be tailored to your loved one s life and personality. The Life Celebration package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care of your loved one including embalming, cosmetics, casketing & the cremation process, use of our staff & facilities for the Life Celebration event in our facility, and required professional vehicles. Your selections for your loved one s casket, memorial urn, personal keepsake, permanent placement and available catering services will be added to the package cost. $ 3,999 Memorial Ceremony The Memorial Ceremony is a formal event that takes place in memory of your loved one. Rather than a having a casket present, a memorial urn, photo or other items serves as the focal point. It is typically held in our funeral home chapel or your place of worship. The Memorial Ceremony package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care of your loved one including sanitary care & the cremation process, use of our staff, equipment & facilities for the Memorial Ceremony event in our facility or your place of worship, and required professional vehicles. Your selections for your loved one s cremation container or casket, memorial urn, personal keepsake, permanent placement and available catering services will be added to the package cost. $ 3,499 Page 4

6 Cremation Options Informal Gatherings Funeral Gathering The Funeral Gathering is a less formal event than that of the Funeral Ceremony. It will be held at the venue of your choice and your loved one will be present in the casket. The Funeral Gathering package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care of your loved one including embalming, cosmetics, casketing & the cremation process, use of our staff, equipment for services at the venue of your choice, and required professional vehicles. Your selections for your loved one s casket, memorial urn, personal keepsake, permanent placement and available catering services will be added to the package cost. $ 3,999 Life Gathering The Life Gathering is a flexible way to honor your loved one s life in an informal setting. The gathering will be held at a location special to your loved one. We will also arrange for a private identification event to allow you a few moments for personal reflection. The Life Gathering package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care, the cremation process, use of our staff for the Life Gathering event at the location of your choice, and required professional vehicles. We will plan and coordinate all of the details associated with the gathering, from securing the venue and catering, to set-up and breakdown. Your selections for your loved one s casket, memorial urn, personal keepsake, permanent placement, available catering services and venue fees will be added to the package cost. $ 3,199 Family Gathering The Family Gathering is a simple way to begin the healing process. It allows an opportunity for family members to gather in ways that are individually unique and at a time and place of their choosing. We will first arrange for a private identification event to allow you a few moments for personal reflection. At a time and date of your selection, we will then deliver your loved one s cremated remains for the Family Gathering event. The Family Gathering package includes all costs associated with our bringing your loved one into our care (within our local service area), professional care of your loved one including sanitary care & the cremation process, and required professional vehicles. 8 Limited Services FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME... $ 2,895 This charge includes: basic service of funeral director and staff, transfer of deceased to the funeral home (within 50-mile radius), embalming and transportation to local airport or other place of shipment (within 50- mile radius). This charge does not include any charges for shipment, shipping container, visitation or ceremonies. This charge applies to shipment within the continental U.S. only. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME.. $ 2,105 This charge includes: basic service of funeral director and staff, transfer of deceased to the funeral home (within 50-mile radius) and transportation to the cemetery (within 50-mile radius). This charge does not include visitation, rites, or ceremonies. DIRECT CREMATION $ 1,900 to $ 15,899 The charge includes: basic service of funeral director and staff, transfer of deceased to the funeral home (within 50-mile radius), refrigeration, courier vehicle and crematory fee. This charge does not include the use of facilities and staff for any visitation or ceremony, public or private, prior to the cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are cardboard. A. Immediate burial with casket selected from our funeral home (plus casket selected).... $ 2,835 PAYMENT POLICY Payment is due prior to date of service. Our payment terms are as follows: A. Cash B. Assignable Insurance Policy: We use a third-party insurance company to process all insurance claims. There is a 5% processing fee for this service. Any policy less than two (2) years old cannot be accepted as payment. Please Note: We cannot accept a claim against an estate as a guarantee of payment. Therefore, the responsible party(s) will need to arrange to pay the funeral expenses and then file their claim for reimbursement with the deceased s estate. Page 7

IN MOST AREAS OF THE COUNTRY, STATE OR LOCAL LAW DOES NOT REQUIRE THAT YOU BUY A CONTAINER TO SURROUND THE CASKET IN THE GRAVE.
