Diablo Boulevard Lafayette, CA (925) FD1523 The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. All funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This General Price List does not include prices for certain items that you may ask us to purchase for you, such as newspaper notices or flowers. The prices for any of these items, which are selected, will be listed on the statement of charges. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. BASIC SERVICES OF FUNERAL DIRECTOR, STAFF AND OVERHEAD: $ 2, This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) It includes: Professional staff available 24 hours a day, 365 days a year to respond to initial calls Arrangement, conference and coordination of services planned with family, clergy, cemetery and/or other parties involved in the final disposition of deceased Preparation and filing of the death certificate, obtaining proper permits and authorizations, and ordering certified death certificates A proportionate share of our taxes, insurance, maintenance, utilities, equipment, inventory and administrative costs PREPARATION AND CARE OF DECEDENT: A. TAHARA and TACHRICHIM (Ritual Purification of decedent & White Burial Garments) No Charge B. EMBALMING... Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. DIRECT CREMATION WITH NO FUNERAL SERVICE: This basic fee includes; a proportionate share of our taxes, licenses, insurance, utilities and business expenses; services of Funeral Director and staff; temperature controlled holding room (additional fee of $ per day will apply after 3 days), preparation and filing of death certificate and securing authorizations and permits; transfer of the deceased to Funeral Home and transfer to crematory (transfer to and from Sinai includes a 40 mile radius from Funeral Home. Additional distance will be charged $ 2.00 per mile.) If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are pressed wood.
