2 GENERAL PRICE LIST These prices are effective as of December 17, 2018 and are subject to change without notice. ITEMIZED SELECTIONS The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF... $ 1, Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family and responsible parties; arrangement of funeral; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation; and coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies and others as required. Also included in this charge are overhead expenses relative to our facilities such as sheltering of remains, professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and administrative expenses This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) EMBALMING... $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF THE BODY A. Dressing, cosmetizing and casketing.... $ B. Sanitary care of remains that are not embalmed.$ C. Visitation at the Funeral Home (Monday thru Friday) $ Our services will include set up of visitation, use of visitation room, placement of encased remains, display of floral arrangements, parking lot assistance, and supervision and attendance during the visitation. B. Funeral Service at the Funeral Home (Monday thru Friday).....$ Our services include coordinating the funeral arrangements, supervision of funeral, staff to attend the funeral ceremony, supervision of committal or other disposition service, and set-up and use of facilities for funeral ceremony. C. Funeral Service/Visitation or other facility (Monday thru Friday)...$ Our services include coordinating the funeral arrangement, supervision of committal or other disposition service, and equipment and additional staff necessary to conduct event at other facility. D. Memorial Service at Funeral Home (Monday thru Friday)...$ Our services include coordinating the memorial service, supervision of the memorial service, staff to attend memorial service, and set-up and use of facilities for memorial service. Memorial Service at other facilities (Monday thru Friday)...$ Our services include coordinating the memorial service, supervision of the memorial service, and additional staff necessary to conduct memorial service at other facility. 5 LIMITED SERVICES FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME..$ 1, This charge includes transfer of remains, basic service of staff, necessary authorizations, embalming, use of preparation room and transportation to local airport or other place of shipment within a 50 mile radius (excluding shipping charges and shipping container). This charge does not include visitation or ceremonies. This charge applies to shipment within the continental U.S. only. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME... $ 1, This charge includes transfer of remains, transportation of remains to cemetery and basic services of staff. This charge does not include visitation, rites, or ceremonies. DIRECT CREMATION... $ to $ 14, The charge for a direct cremation includes transfer of remains, local transportation to crematory, necessary authorizations, crematory fee and basic services of the staff. This charge does not include the use of facilities and staff for any visitation or ceremony, public or private, prior to the cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are unfinished wood boxes and fiberboard boxes. A. Direct cremation with container provided by purchaser... $ B. Our payment terms are as follows: Cash, Personal Check, Discover, Visa & Master Card Proceeds from Avista Pre-Arranged Plans Assignable Insurance Policy - We use a third party insurance company to process all insurance claims, there is a 5% processing fee for this service. Please Note: Any insurance policy less than two years old cannot be accepted for payment. We do not accept estates as a guarantee of payment. Therefore, we require that the responsible party(s) pay the funeral expenses and then file their claim for reimbursement with the decedent s estate. 4
