3 Whether you are planning a burial or cremation, there are countless ways in which a funeral ceremony can be tailored to the life and personality of the person you are honoring. A meaningful funeral is one that touches the hearts of all those who attend. Here are just a few things we can provide or arrange to help make the service a truly personalized tribute. Military Honors All honorably discharged Veterans are entitled to a military honors ceremony, including flag presentation and Taps. Provide us with the Military discharge papers (form DD-214) and we will take care of the rest. Special Music Talented musicians can enhance your memorial event. A bagpiper is an ancient and honorable tribute. A harpist can help create a beautiful, peaceful setting. Prices will vary depending on the musician. Video Memorial Tribute We use your favorite photographs of your loved one to create a personalized tribute, which can be shown at the visitation or service. Memory Table We will gladly help you arrange a table with favorite and meaningful mementos unique symbols of important parts of your loved one s life. Memorial Stationary A variety of guest books and memorial printing is available for your selection. We will work with you to customize printed tributes such as prayer cards or service programs with quality color printing.

4 CHOICES RELATED TO PAYMENT The loss of a loved one is a devastating experience. It is frequently traumatic and can seriously impact judgment. Therefore, it is important to be realistic with respect to funeral expenditures. We do not want your choices to be more than is affordable, nor less than you would like. It is important that arrangements be in keeping with a family s wishes and budget. Becker Funeral Home has established a uniform payment policy to serve all families fairly and to alleviate misunderstandings. A sound payment policy enables us to contain our costs to the families we serve. As a small business providing services and merchandise, we are totally dependent upon our families for prompt payment. Our general payment policy is that the entire amount of the funeral is due and payable prior to services being rendered. We realize that caring for the death of a loved one is often unexpected and unplanned. For your convenience, we offer the following payment options: Cash Check Personal or Business Money Order/Cashier s Check Assignment of insurance on verified life insurance policies. Policy verification must be done by funeral home and insurance must have been in force for at least 2 years from the date the policy was issued. There will be a 7% fee added to the total of the funeral contract. All cash advanced items must be paid by time of services and will not be included on any life insurance assignments. Application of funds from an Advance Funeral Plan from Becker Funeral Home. In certain instances we can also accept the funds from other funeral homes pre-planned agreements, though Becker Funeral Home does not have to honor the other funeral homes prices. Payments by Estate Due to the uncertainty regarding estate payments we require that a member of the family pay funeral expenses and file their claim for reimbursement with the decedent s estate. SPECIAL SITUATIONS As a community service, it is our policy to offer our services and facilities at no cost to infants less than one year of age. Services for children and those in public service who lost their life in the line of duty are offered at a reduced rate. It is our policy that no one be denied a service because of limited financial resources. If you would like to arrange a service and do not have sufficient funds to pay in accordance with the Payment Policy outlined in our General Price List, we will be pleased to discuss alternative arrangements.

5 CASH ADVANCES This General Price List does not include prices for certain items you may ask us to buy for you such as cemetery charges, death certificates and flowers. The prices for those items will be shown on the statement describing the goods and services you selected. As an accommodation, the funeral home will pay a third party that provides services and merchandise to the buyer in connection with the funeral, and will list those charges as cash advances on the Statement of Funeral Goods and Services Selected which is provided to the buyer at the end of the funeral arrangements. Notwithstanding the fact that the funeral home is arranging the payment to the third party, the arrangement is between the Buyer and the third party. The funeral home is not in control of the third party providing services or merchandise to the Buyer and will not be responsible for any of the third party s acts or omissions. CEMETERY FEES: Opening and closing of the grave. Most area cemeteries are open on Saturdays, Sundays and some holidays, but may have an overtime charge for the opening and closing of the grave on those days. CERTIFIED COPIES OF THE DEATH CERTIFICATE: Certified copies of the Death Certificate are $12.00 each. MEDICAL EXAMINER FEES: There is a fee for the Cremation Authorization required from the Medical Examiner that has jurisdiction over where the death occurred. NEWSPAPER OBITUARIES: The local daily paper (Orlando Sentinel) has two types of obituaries. A death notice, which is published at a charge of $25.00, and includes five lines of text. They also offer a paid obituary, which may include a picture, biographical information, survivors, etc., which is charged by the line. The Orlando Sentinel does require prepayment by credit card for a paid obituary. We will also be glad to place obituaries in out-of town papers at your request. Most newspapers will have a charge and require prepayment.

6 It is always our goal to provide the best possible services and support to the families that place their trust in us after losing a loved one. However, we are well aware that the price of a funeral or cremation can be expensive. This is why we strive to offer a variety of services and packages to meet the diverse needs of all families we serve. BURIAL PACKAGES The prices below represent existing packages. However, we understand that not all services may be relevant or necessary, so we are happy to discuss a customized package with you. Prices will reflect charges for the services and merchandise from our General Price List. Please note for the packages below, there are no credits or substitutions for any items not used. Additionally keep in mind that the prices below do not include cemetery fees, other cash advance items or any merchandise not specifically listed. Full Service Burials All of our full-service burial packages include the professional services associated with a traditional funeral, including: basic services of funeral director and staff; local transfer of deceased from place of death into our care; embalming and other preparation; use of facilities and staff for visitation at the funeral home; coordinating and directing funeral services either at your place of worship or at the funeral home; cemetery committal service; use of hearse, and delivery of flowers to church, cemetery or other location. Two-Day Traditional Service A complete funeral service, that includes up to four hours of visitation time. Selecting this package not only makes the decision process easier, it provides for exceptional value by incorporating a significant savings, which is not available by itemizing. 7 Limited Service Burials For some families, a service held in their place of worship or simply at the cemetery is more appropriate. These packages were designed for those families who want something a little simpler. All of our limited service burial packages include the following: basic services of funeral director and staff; local transfer of deceased from place of death into our care; use of refrigeration and other preparation; coordinating and directing services at your place of worship or the cemetery; use of hearse, and delivery of flowers to church, cemetery or other location. Church Only Service This package is in keeping with more traditional funerals, without a visitation. In addition to the services mentioned above, the Church Only funeral package includes: Wilbert Venetian Outer Burial Container Choice of memorial package with printing $300 floral allowance Package Price: $6495 plus cost of casket selected from our funeral home Graveside Package A simple graveside service without visitation or church service meets the needs of many families. In addition to the services mentioned above, the Graveside Package includes: Wilbert Venetian Outer Burial Container Choice of memorial package with printing $200 floral allowance Package Price: $5995 plus cost of casket selected from our funeral home Green Burial Service A green funeral can and should be as dignified as a traditional funeral. Our green burial package allows for a funeral to take place without the use of toxic and non-biodegradable materials and chemicals. For such arrangements, formaldehyde-based embalming is prohibited, as is the use of metal or hardwood caskets. This package provides everything necessary for a burial within a green cemetery. All of our green burial packages include the following: basic services of funeral director and staff; local transfer of deceased from place of death into our care; use of refrigeration and body preparation mentioned above; coordinating and directing cemetery committal service; use of hearse, and delivery of flowers to cemetery. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL STAFF SERVICES Basic Services of Funeral Director and Staff $1345 Our professional service charge includes: 24-hour availability; consultation with family; preparation and filing of authorizations, certificates and permits; and a proportionate share of our overhead costs. This fee for our basic services and overhead will be added to the total costs of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding and receiving remains.) EMBALMING Except in certain special cases, embalming in not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming you usually have the right to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial. Family Witnessed Cremation $ 250 For those that want the peace of mind of being present for the cremation, we will schedule the cremation at a mutually agreed upon time. This is in addition to the standard cremation fee. An upgraded cremation container is recommended. Celebrant Services $ 295 For families who prefer a personalized service or do not have a clergy member to officiate, we offer the services of a certified Celebrant. Our Celebrant will meet with your family and create an event that is personalized to your loved one. Our Celebrant Service is available to both faith-based and non-church affiliated families and may take place in our chapel, at the cemetery or other location. USE OF FACILITIES AND STAFF Includes use and setup of visitation area or chapel; coordinating and directing the service; display of flowers; and staff attendance and supervision. Use of Refrigeration (for unembalmed remains) $ 495 An additional fee of $50.00 per day charged for sheltering and refrigeration after 10 days. Use of Facility for Private Autopsy $ 395 This does not include Pathologist fees, which are paid by family to Pathologist. 12 OTHER SERVICES Direct Cremation $1325 to $1400 Our charge for a direct cremation includes: basic services of funeral director and staff, a proportionate share of overhead costs, transfer from place of death into our care, refrigeration and sheltering of remains, private family farewell; standard cremation fee and placement of obituary on funeral home website. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of fiberboard and composite materials. 13 DISPOSITION OF CREMATED REMAINS Release of Cremated Remains We will hold the cremated remains at no charge for up to thirty days. No Charge Hand Delivery of Cremated Remains $ 150 We will hand deliver cremated remains to a family residence or cemetery in our local area during normal business hours. Delivery of Cremated Remains to Florida National Cemetery $ 195 We will deliver cremated remains at the convenience of the funeral home. (Without Ceremony) Packaging and Mailing of Cremated Remains $ 175 We will carefully package and mail the cremated remains via USPS Priority Mail Express to any U.S. destination (please allow up to five days). International destinations will be billed on a case-by-case basis. It is the funeral home s policy to require an urn or plastic temporary container for mailing. Keepsakes, pendants and other items will be on a case-by-case basis. Unattended Scattering of Cremated Remains $ 295 We will scatter the cremated remains in either the Atlantic Ocean or the Gulf of Mexico at the convenience of the funeral home. Storage of Cremated Remains (After 30 Days and up to 120 Days) $ 75
