2 The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the goods and services you selected. Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $ 2, This fee for our basic services and overhead will be added to the total cost of funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding and receiving remains.) This fee includes the following; personnel available 24 hours a day, 7 days a week to respond to initial call for service, arrangement conference and coordination of clergy, cemetery or crematory and other parties involved with the final disposition of the deceased, clerical staff for securing and recording the death certificate and disposition permit. PREPARATION OF THE BODY: Tahara and Tachrichim (Ritual Purification of decedent and White Burial Garments) No Charge Refrigeration (California law requires refrigeration of unembalmed remains within 24 hours. $ This fee includes up to three (3) days of usage; thereafter it is $75.00 per day. Embalming $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other Preparation of Remains $ (washing, dressing, normal cosmetology and casketing) Special care for autopsied remains $

4 FORWARDING REMAINS TO ANOTHER FUNERAL HOME: $ 2, This charge includes basic services of funeral director and staff, transfer of the deceased to the funeral home, refrigeration and transportation to airport within a 40 mile radius. This charge does not include preparation of the remains, visitation, funeral services, merchandise, shipping charges or other cash advance items. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: $ 2, This charge includes basic services of funeral director and staff, transfer of the deceased to the funeral home from airport or other common carrier and transportation by funeral vehicle to the cemetery or crematory within a 40 mile radius. This charge does not include preparation of the remains, visitation, funeral services, merchandise, shipping charges or other cash advance items. IMMEDIATE BURIAL: $ 2, to $ 17, This charge includes basic services of funeral director and staff, transfer of deceased to the funeral home, refrigeration, traditional preparation and funeral vehicle to cemetery within 40 mile radius. Burial will be scheduled at service provider s convenience with no service. This charge does not include any other preparation of remains, visitation, funeral service, merchandise, cash advance items, cemetery fees or taxes. $ 2, Immediate burial with container provided by purchaser. (Must meet all cemetery and regulatory requirements.) $ 4, Immediate burial with Nahum, solid pine traditional construction, unsanded unfinished natural pine flat top, no interior as provided by the funeral home. $ 2, Immediate burial with casket purchased from our facility, in addition to casket cost. DIRECT CREMATION: $ 1, to $ 16, If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without outside covering.) The containers we provide include containers made of wood, pressed wood, and composite materials. You may use your own container (provided it meets crematory and regulatory requirements of rigidity, opaqueness and is leak proof), or a wood casket. This charge includes basic services of funeral director and staff, transfer of deceased to the funeral home, refrigeration and service vehicle to crematory within a 40 mile radius. Cremation will be scheduled at service provider s convenience with no service. This charge does not include any preparation of remains, visitation, funeral service, merchandise, cash advance items, cemetery/crematory fees or taxes. $ 1, Direct cremation with container provided by purchaser. (Must meet all crematory and regulatory requirements.) $ 2, Direct cremation with pressed wood container, without interior as provided by the funeral home. $ 2, Direct cremation with poplar veneer plywood cremation container as provided by the funeral home. $ 1, Direct cremation with wood casket purchased from our facility, in addition to casket cost.
