The goods and services shown below are those we can provide to our families. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Minimum Services of the Funeral Director & Staff: This fee is for our basic services and will be added to the total cost of funeral arrangements you select. (This fee is already included in our charges for direct cremation, immediate burials and forwarding or receiving remains). This fee includes, but is not limited to the following: Personnel available 24 hours a day, 365 days a year to respond to initial request for service Arrangement conference with family or responsible party, with licensed funeral director Arrangements for funeral rites: * Preparation and filing of all necessary authorizations and documents * Preparation and filing of vital statistics * Preparation and filing of life insurance, (If applicable) Coordination with those providing other portions of the funeral service: * Newspaper for obituary notices; * Cemetery**, crematory and/or other parties involved in the final disposition of the deceased; * Clergy, rabbi, ministers or any other denominations involved for funeral service; * Motorized police escorts for funeral processions; * Floral arrangement for casket; * Vocalist and/or Guitarist; Also included in this charge are overhead expenses related to our facilities such as professional licensing, legal and accounting fees, insurance, building and utility expenses, ground maintenance, taxes, equipment, furnishings, inventory costs, secretarial and administrative expenses. $ **All cemetery arrangements must de done by the family at the cemetery of their choice. **

2 Preparation of the Remains: A. EMBALMING Embalming is not required by law. However, embalming may be necessary if you select a certain funeral arrangement, such as a funeral with an open casket. If you do not want embalming, you have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate disposition. OTHER PREPARATIONS: $ B. Cremation is a final and irreversible act and we feel it is our responsibility to family members to ascertain with absolute certainty the identity of a deceased family member prior to the actual cremation. In exercising our responsibility to our client families, it is our policy that an immediate family member who is familiar with the decedent make a positive identification of the decedent prior to cremation. For your convenience a fifteen-minute period will be reserved for you at a specific date and time at our chapel for the identification process. The identification process is not intended to be traumatic experience for you or your family, but rather a necessary procedure to provide piece of mind to you and our staff. We assure you that the decedent will be properly clothed in a gown or appropriate manner and a staff member will be present for assistance. We hope this process will not inconvenience you and that you will agree the request is in the best interest of you and your family. A. Direct Cremation Our charges for a direct cremation (without ceremony) includes removal of remains within a ten (10) mile radius of funeral home, care of remains, minimal required staff, necessary authorizations and transportation to local crematory. If you would like to arrange a direct cremation you may select an alternative container. Alternative containers encase the body and can be made of materials such as fiberboard or composition materials (with or without an outside covering). The containers we provide are of composite wood construction. Cremation fee is included. 1.
