2 CHEROKEE NATION BID REQUEST CUBICLES & FURNITURE ADDENDUM 01 Companies completing mandatory site visit & eligible for award of project: Furniture Marketing Group Scott Rice Tahlequah Printing Company Progressive Interiors (DIRTT) Five Star Office Supply There are changes to the cubicle portion of bid per request of Youth Projects & Special Services Department. This replaces the original information for the cubicles. 6 Stand Alone Cubicles Preferred Measurement 9 x 9 x 8. The cubicles must have an open front that can be frosted or clear glass. Each cubicle would be 3 1 Stand Alone Cubicle Measurement 10 x 10 x 8. The cubicle would be 3 Power outlets are not necessary in any of the panels. Sliding glass door preferred. Please see the following picture. This is not the exact cubicle listed, but provides an idea of the appearance desired by the Department. The Product Information/Scope of Work has been updated to reflect the revised cubicle information (page 3 of this addendum). MANDATORY BID RESPONSE SHEETS HAVE BEEN UPDATED TO REFLECT THE OUTLINED CHANGES. PLEASE USE THESE FOR BID SUBMITTAL (pages 4 & 5 of this addendum). 2

Muskogee Avenue, Suite A & B Tahlequah, Oklahoma Mandatory Site Visit Information: Interested parties are to contact Jennifer Kirby at telephone number (918) or at jennifer-kirby@cherokee.org to schedule a site visit. Site visits are mandatory and must be conducted on July 27 or July 28, 2016 at time of appointment stipulated by Jennifer Kirby. General Information: The area is new office space, and the purpose of this bid is to procure cubicles and furniture, equal to or better, than the specifications provided. The cubicles and furniture are to be fully assembled, delivered, and installed. Specifications and photos of product bid must be submitted with bid response. Scope of Work/Product Specifications: All product to be assembled, delivered, and installed. o 6 (Six) Stand Alone Cubicles Preferred Measurement 9 x 9 x 8.
