(HSAI), is pleased to submit our qualifications for providing professional consulting services to Escambia County Professional Services As Governed by Florida Statute Specifications. The following addresses the evaluation criteria as understood per our meeting with Mr. Nobles, Purchasing Manager. OFFICE LOCATION RESPONSIBLE FOR PROJECT The Gulf Breeze office of HSAI is responsible for providing the services required. With our staff located within fifteen minuets of Escambia County, HSAI can provide the level of service needed to serve Escambia County Professional Services As Governed by Florida Statute Specifications on all projects. KEY PERSONNEL The ability of a consulting firm to serve its clients is a function of the staff s extensive experience and the philosophy of the firm. HSAI is proud of the technical competence of its staff and the shared commitment to quality. Key HSAI staff proposed for this Professional Services as Governed by Florida Statute " contract is as follows: Martha A. Swift John L. Swift, PE, PLS Fountain E. Conner, PE Patrick Comeaux President Project Director/Engineer Senior Engineer CADD Principal-In-Charge/ Project Manager: John L. Swift, PE: Mr. Swift is Vice President for Engineering Services at HSAI. Mr. Swift has over 39 years experience in all aspects of planning and infrastructure-based engineering throughout the South. Mr. Swift also has broad roadway design and utility experience with all types of facilities ranging from interstate highway to local streets. He has successfully managed numerous parks and recreation, drainage and transportation projects throughout his more than 39 years of professional engineering experience. Mr. Swift will serve as Principal-in-Charge and Quality Assurance Officer as well as Project Manager for this contract. HSAI has been the Design, bid and inspection engineer for the City of Gulf Breeze, Florida for the past eight years, with all projects bidding at 10% below budget and final invoice being at 7 to 10 % below bid for over $ 10 million in construction cost and have saved the tax payers over $ 1 million over the same period of time. This has more then paid for HSAI s services. HSAI has also completed NPDES Florida Forever Grant Project ($ 1,000, Grant), Sidewalk and Parks & Recreation Projects for the City of Gulf Breeze, Florida, on time, under bid and with additional work, still being within bid. HSAI looks forward to serving Escambia County Professional Services As Governed by Florida Statute Specifications on these projects and showing you the type of professional services HSAI turns out on a daily bases. Presently Mr. Swift is managing the Gulf Breeze City Phase II Stormwater Drainage Improvement Florida Forever Grant Program ( $ 500, Grant) for the City of Gulf Breeze, Florida, for which all bids in the first phase came within five percent of the engineering construction cost estimate. This is the prime component to any City/County Program success, that being working within Program Cost Budgets.

Gulf Breeze FY-2001 total cost of project = $ 256,682.36; 3.01 miles 3. Gulf Breeze FY-2002 total cost of project = $ 281,335.46; 3.03 miles 4. Gulf Breeze FY-2003 total cost of project = $ 309,749.65; 3.20 miles 5. Gulf Breeze FY-2004 total cost of project = $ 294,439.94; 2.46 miles 6. Gulf Breeze FY-2005 total cost of project = $ 274,108.24; 2.04 miles 7. Gulf Breeze FY-2006 total cost of project = $ 298,981.69; 2.24 miles 8. Gulf Breeze Total Five Year Program = $1,977,979.97; miles The above cost is total, including engineering, bidding, inspection and construction for first six years of the program. This works out to be $ 52, per lane mile or $ 9.87 per lane foot over the past seven years or $ 7.40 per square yard. This is net savings under budget estimate of $ 122, over the seven-year period of the fifteen-year program. EVIDENCE OF SATISFACTORY COMPLETION OF SIMILAR Professional Services As Governed by Florida Statute "RELATED PROJECTS HSAI has provided, or is currently providing a range of services from reviewing plans for permitting, conducting public consensus-planning meetings, developing master conceptual regional recreation park plan which include a playground unit, picnic pavilions, open play fields, parking/access, sports lighting, site lighting, a Stormwater Management System, a nature trail and associated athletic buildings (concession/restroom and meeting/storage buildings), to final design of baseball park, tennis court improvement, multi-purpose sidewalk/boardwalk, concrete skateboard park, parking lots, roadways, bridges, site drainage, potable water/water main systems, and site planning. Also, HSAI has established relationships with Escambia County municipal agencies, in addition to regional, state, and federal regulatory agencies during its eleven-year history in Panhandle Florida. The following is a sample list of projects that Hernandez & Swift Associates, Inc. and/or team members have successfully completed within the past eleven (11) years: Relevant Work Experience: Gulf Breeze City Parks and Recreation Shoreline Park South- Master Plan, design, bid, and inspection of two new concrete boat ramps, three boat docks, boardwalk, pavilion/gazebo, swimming pier, restroom building, parking lot and picnic area. Amenities include landscaping, parking lot lighting, park benches, and trash receptacles. (Total Construction Cost $ 1,697,981.00) Gulf Breeze City Parks and Recreation Wayside Park East & West- Master Plan, design, bid, and inspection of boardwalk, two boat dock, fishing pier, parking lot, pavilion/gazebo and picnic area. Amenities include landscaping, parking lot lighting, park benches, and trash receptacles. ( Total Construction Cost $ 1,485,188.00) Gulf Breeze City Parks and Recreation Vista Park Fishing Pier- Design Deck and Pier Replacement, bid, and inspection. ( Total Construction Cost $ 168,785.00) Gulf Breeze City Parks and Recreation Baycliff Park Boardwalk- Design Deck and Pier Replacement, bid, and inspection. ( Total Construction Cost $ 166,918.00) Gulf Breeze City Parks and Recreation Shoreline Park North- Re-design three parking lots, bid, and inspection. (Total Construction Cost $ 316,008.00) Gulf Breeze City Parks and Recreation Softball Complex- Master Planed, designed, bid, and inspected to City s construction of five new softball fields. Other improvements included a new drainage system that eliminates water runoff on sidewalks and softball fields. ( Total Construction Cost $ 2,652,934.00) Gulf Breeze City Parks and Recreation Baseball Complex- Master Planed, designed, bid, and inspected to City s construction of five new baseball fields. Other improvements included a new drainage system that eliminates water runoff on sidewalks and baseball fields. ( Total Construction Cost $ 702,785.00) Gulf Breeze City Parks and Recreation Soccer/Football Complex- Master Planed, designed, bid, and inspected to City s construction of one new multi-purpose field. Other improvements included a new drainage system that eliminates water

3 runoff on sidewalks and multi-purpose field. ( Total Construction Cost $ 2,156,940.00) Gulf Breeze City Parks and Recreation Tennis Complex- Re-surfaced the existing courts by milling down areas were needed, placing a leveling course of FDOT Type S-1 Asphalt as needed, placing a geo-mat over the entire court, and then placing 1.5 inches of FDOT Type S-1 Asphalt over the site, topping with surface course material. (Total Construction Cost $ 275,785.00) Gulf Breeze City Parks and Recreation Multi-Purpose Sidewalk/Boardwalk- Design, bid, and inspection of a new sidewalk/boardwalk through the environmental wetland area at Shoreline Park South. Amenities include landscaping, path lighting, park benches, and trash receptacles. ( Total Construction Cost $ 216,314.00) Gulf Breeze City Parks and Recreation Concrete Skateboard Park- Design, bid, and inspection of a new concrete skateboard park at Shoreline Park North. ( Total Construction Cost $ 78,785.00) Gulf Breeze City Tri-School Multi-Purpose Sidewalk- Design, bid, and inspection of a new multi-purpose sidewalk, including landscaping, path lighting and trash receptacles. (Through a FDOT Grant for $ 115,000.00). Gulf Breeze City Phase I Stormwater Drainage Improvement Florida Forever Grant Program- Replaced an undersized Stormwater Drainage System at two sites and added two exfiltration/infiltration trenches and pre-treatment of positive discharges. (Total cost $ 1,875, of which $ 1 million was a Florida Forever Grant), written by HSAI. Gulf Breeze City Phase II Stormwater Drainage Improvement Florida Forever Grant Program- Replaced an undersized Stormwater Drainage System at two sites and added two exfiltration/infiltration trenches and pre-treatment of positive discharges. (Total cost $ 1,396, of which $ 500, is a Florida Forever Grant), written by HSAI. Gulf Breeze Safeway to School Sidewalk Program Phase I through Phase III- Design, bid, and inspection of a new sidewalk, including landscaping. ( Total Construction Cost $ 468,785.00) City of Gulf Breeze, Florida - FY-2002 Roadway Maintenance project. ($295, engineer's estimate) ($280, Contact Bid) ($254, Final Payment) City of Gulf Breeze, Florida - FY-2001 Roadway Maintenance project; Sidewalk Improvement at Shoreline Park North; Sidewalk Improvement on Nightingale Lane, Ross Drive and Center Street; and Drainage Improvements on Nightingale Lane near Kevin Drive. ($340, engineer's estimate) ($322, Contact Bid) ($282, Final Payment Estimate) City of Gulf Breeze, Florida - FY-2000 Roadway Maintenance project; Pavement Over-Lay for Shoreline Park South; Sidewalk Improvement on Andrew Jackson Trail & McClure Drive; and Drainage Improvements on Andrew Jackson Trail. ($404, engineer's estimate) ($389, Contract Bid) ($350, Final Payment) Past Project Management and Engineering Design Experience of John L. Swift, PE Design and inspection for $18 million of City of New Orleans Capital Improvement Program including Streetscape design, sidewalks, curb and gutter, landscaping, street lighting and furniture placement work as well as construction inspection. Design and inspection for $2.4 million Vieux Carre Area II Bourbon Street Sidewalk Capital Improvement World s Fair project for the City of New Orleans, Louisiana including the relocation of all underground utilities (cable, phone, power, gas, water, sewer and Stormwater). Also streetscape design, sidewalks, curb and gutter, and street lighting as well as construction inspection. Design and Inspection for $18 million City of New Orleans Capital Improvement Programs including: Roadways, storm drainage, sidewalks, intersection traffic operations reconstruction (through a TOPICS type program), sewer and water, and repair work as well as construction administration and inspection. $12.5 million Sandy Plains Road, Cobb County Public Works Department, Marietta, Georgia Corridor traffic study, preliminary design, final construction documents, surveying, and relocation of water/sewer facilities. This 3.2-mile project required widening from two lanes to four lanes using a divided cross section. Construction plans and specification were prepared including signalization, channelization, drainage, pavement marking, signing, and right-of-way maps, including legal description of taking.

4 Johnson s Ferry Road, Cobb County Public Works Department, Marietta, Georgia Traffic studies, preliminary design, final construction documents, surveying and relocation of water/sewer facilities were prepared on this 2.0-mile project. Widening from two lanes to six lanes with median using an urban cross section was required. Construction plans and specifications were prepared including signalization, channelization, drainage, pavement markings, signing and right-ofway maps, including legal descriptions of takings.($10.3 million) Jones Shaw Road, Cobb County Public Works Department, Marietta, Georgia Final construction documents, surveying and relocation of water/sewer facilities along 2.5 miles of road. This section of four-lane divided road from the southern section of a full loop by-pass road. It connects to a four-lane roadway on Ernest Barrett Parkway on the west and another four-lane improved City Street on the east. Construction plans and specifications were prepared including signalization, channelization, pavement markings, lighting and signing.($7.5 million) Prepared final roadway and traffic signal design at three intersections and a complex guardrail design in Orange County, Florida. Final signal, sign, and pavement marking plans as well as roadway construction plans numbering 104 sheets were prepared using CADD equipment. FDOT and Orange County standards were used as well as the firm s traffic engineering expertise for concept design, capacity analysis, signal timing, and final design. State Road 436 Corridor Study. This project was conducted to determine ultimate roadway needs of this facility, which was carrying some 50,000 vehicles per day. The corridor is approximately 10 miles long and includes a regional shopping center, a major interchange (SR 436 and I-4), a high volume intersection (US and SR 436), and a wide range of retail, office, and institutional activities. Future traffic demands were determined from a microanalysis of 150 traffic zones using models from the Orlando Urban Area Transportation Study. Mainframe computer programs were used in accord with procedures of the FHWA and FDOT. Through citizen and technical committees, a full range of multi-modal improvements were adopted, i.e., interchange improvements, mass transit, bikeways, fly-over (bridge), a park-and-ride facility, and traffic operational improvements (functional plans). The full program was presented and adopted at a final public hearing. Lake Mary Boulevard Corridor Study. Study to determine right-of-way for future widening of this facility to accommodate extremely heavy growth anticipated within the near future. In order to quantify traffic demands and intersections needs, a micro-zoned corridor was used with the Orlando Urban Area Transportation Models. Models were validated to existing conditions prior to use then applied to updated socioeconomic data for 150 micro-zones supplied by Seminole County. Both citizen and technical committees were used to insure public and government agency concurrence as the project progressed. The seven-mile long corridor study resulted in adoption of a specific cross-section (six lanes) and functional treatment of all intersections. Right-of-way plans were prepared at a scale of 1 =50 and reserved as development occurred. KNOWLEDGE AND EXPERIENCE WITH PANHANDLE FLORIDA REGULATIONS AND AGENCIES Hernandez & Swift Associates, Inc. has provided comprehensive planning, topographic and site-specific surveys necessary to design the projects, public consensus meetings, developing master conceptual regional recreation park plan, sports complex conceptual plan, hold additional public meetings for final approval, sports complex design, development of construction drawings/documents, obtaining all required local, state and federal permits, civil, environmental, infrastructure, and transportation engineering, as well as infrastructure, provide bid evaluation services and construction management, provide post construction certifications and as-build drawings to Panhandle Florida clients since Our Panhandle Florida based staff has extensive knowledge and experience with Panhandle Florida regulations and agencies. This with the limited land available, limited drainage outfalls capacity, and high intensity of development, will impose challenges to officials trying to satisfy the community demands for the services, while responsibly meeting ever-increasing environmental concerns and regulations. These challenges need to be met with enthusiasm and with the development of innovative approaches for their solution. It includes existing land use and planning including zoning, future land use, area school capacities, public safety facilities and existing infrastructure concurrency components, parking space, roadway improvements, and many other miscellaneous projects.
