Methods: Randomized ontrolled trial of 761 households. An ionization or photoeletri smoke alarm was installed between June 1, 2000 and July 31, Main outome measures were: perentage of study alarms that were working, observed reasons for non-funtional status, and self-reported frequeny of nuisane alarms at 9 and 15 months of follow-up. Results: At 9 months after installation, 20% of ionization, vs 5% of photoeletri alarms were non-funtional, a differene that persisted at 15 months, with the most ommon reasons for both types being a disonneted or absent battery. The risk ratio for ionization, relative to photoeletri alarms, being non-funtional or removed was 2.7 (95% CI 1.8 to 4.1) at 15 months of follow-up. These findings were not altered by eduational level, or the presene of smokers, hildren,5 years, or adults >65 years. Conlusions: Burn prevention efforts are geared towards inreasing smoke alarm ownership and improving maintenane of funtional status. Results suggest that the seletive use of photoeletri alarms by fire injury prevention programs or onsumers may provide longerterm protetion in similar populations. Designing smoke alarms that minimize nuisane alarming may also result in longer term funtionality. Smoke alarms are effetive interventions for injuries from residential fires. 1 2 Reent surveys indiate that 95 97% of US households have at least one smoke alarm, 3 4 with lower prevalene in rural areas. 5 However, alarms disonneted from batteries, ontaining dead batteries, or removed due to nuisane alarms offer no protetion. Inoperability levels range from 20% 6 7 to 36%. 8 The most ommon types of alarms marketed for home use are ionization, photoeletri, and ombination units. and ionization alarms operate via different mehanisms, deteting visible, and invisible/fine, byproduts of ombustion, respetively. 9 alarms use optial sensors and are more likely to sound due to slow, smoldering onditions. units are more responsive to flames by deteting harged partiles from rapid ombustion. Nuisane alarms, often set off during ooking, are often ited as a reason why oupants disonnet alarms A study onduted in Alaskan villages reported that ionization alarms were more likely to nuisane alarm than were photoeletri alarms. 11 However, a randomized trial of a smoke alarm distribution program in a poor neighborhood of London reported that photoeletri alarms were more likely to sound nuisane alarms. 12 We onduted a randomized ontrolled trial to ompare the funtionality of ionization and photoeletri smoke alarms by installing them in middleto low-inome households in King County, Washington, and omparing their funtional status, reasons for non-funtion, and frequeny of nuisane alarms at 9 and 15 months of follow-up. METHODS Partiipant identifiation A proprietary database of all private property in King County in 2000 (MetroSan) identified owner-oupied dwellings within the lower quartile of assessed values (,$ ) in ensus trats with above-median density (755+ homes). This was to inlude homes at greatest risk of injury from residential fires, given the inverse assoiation observed between soioeonomi status or inome level and fire injury risk Condominiums were exluded, as the study s smoke alarm installation and plaement protools may onflit with those speified in ondominium ovenants. Seleted residenes were mailed a letter inviting partiipation in a fire safety study in ooperation with the King County Fire Chiefs Assoiation, involving installation of smoke alarms and provision of fire extinguishers. Homeowners were not informed of underlying hypotheses. Up to 10 phone alls and 2 messages were attempted to invite partiipation. Only 58 partiipants enrolled after 4 months, an arual rate that would have yielded insuffiient partiipants. Thus, a blokwalk approah was initiated in whih study staff visited households. From the pool of eligible dwellings, we identified lusters of potential partiipants using Thomas Guide map grids 15 to maximize effiieny of identifiation. Twenty multi-day blokwalks to these areas were onduted, eah targeting potential residenes. Within eah blokwalk, households were randomly seleted to reeive either an ionization or photoeletri alarm. Ultimately, 6236 homes were identified for approah using this method (fig 1). Staff worked in pairs during home visits during June 1, 2000 to July 31, If potential partiipants were at home, the team invited partiipation, either then or later. If nobody was at home, they left a letter indiating that a smoke alarm installation team would return on a future date. Residents ould all to refuse, shedule an appointment, or be visited on the return date. 80 Injury Prevention 2008;14: doi: /ip

Proedures were approved by the Human Subjets Division at the University of Washington prior to study ondut. Baseline visits A standardized survey was onduted, followed by installation of one study alarm per household. The survey queried demographis, fire safety praties and behaviors, history of fire or related injuries, building harateristis, heating and ooking fuel soures, soures of fires, flames, or smoke, and presene and loations of existing smoke alarms. A target date was set for the first 9-month follow-up. No appointment time was arranged, to keep subsequent inspetions unannouned and derease the likelihood of observing behavior altered by antiipation of a repeat visit. Study personnel were trained in smoke alarm installation as desribed by the US Fire Administration 16 using the National Fire Alarm Code Handbook. 17 Partiipants were not informed of hypotheses or type of alarm installed, although they were informed they would reeive an alarm endorsed by the National Fire Protetion Ageny (NFPA) for residential use. Beause of the different appearane of the two alarms, it was not possible to blind study staff to alarm type; however, staff were not informed of underlying hypotheses. Teams were asked to install study alarms on main levels, in rooms adjaent to kithens. Adjaent rooms without alarms were the highest priority in seleting the loation. Alarms were mounted on the wall 3 feet from the kithen entrane and 4 12 inhes from the eiling, a loation with potentially greater hane of having nuisane alarms. If alarms were already installed in that room, the study alarm would be installed in the same room; however, the installer would re-install the existing alarms elsewhere if requested. Both alarm units had hush buttons and zin batteries, whih were tested and installed with the alarm. Original batteries were marked with indelible ink to later determine whether new batteries had been installed. The alarm s baseline funtion was tested with anned smoke, and partiipants instruted in alarm maintenane. The operation manual and a fire safety brohure were provided. Residents were also given a fire extinguisher, instrution in its use, and information regarding optimal loation. At baseline, 761 homeowners were interviewed. Three were later found ineligible beause they rented; 1 eleted to have no alarm installed. The 757 remaining households inluded 371 randomized to reeive ionization and 386 randomized to reeive photoeletri study alarms. Follow-up visits At both follow-ups, we queried the status of all smoke alarms, ourrene of any fires and related alarms, and ourrene of nuisane alarms (asertained separately for study and existing alarms) and their pereived auses. If alarms were reported nonfuntional, the reasons were asked. Teams inspeted and tested study alarms to determine funtionality, noted reasons for nonfuntion, and observed battery status. "Based on the single most frequently used fireplae or stove per household. inoperable, new alkaline batteries were installed. A $10 reimbursement was provided per visit. First follow-ups were ompleted with 336 (91%) ionization and 342 (89%) photoeletri study group homes. Mean (SD) elapsed times between baseline and first follow-ups were 9.0 (0.6) months (range ) and 9.0 (0.6) months (range ) for ionization and photoeletri alarms, respetively. Seond visits were ompleted for 315 (85% of baseline) homes randomized to the ionization and 316 (82%) to the photoeletri group. Mean (SD) elapsed times between baseline and seond follow-ups were 14.9 (0.7) months (range ) for the ionization group, and 14.9 (0.8) months (range ) for the photoeletri group. Reasons for exlusion were similar aross study groups at both follow-ups. Data analysis Five households randomized to reeive ionization alarms and 17 randomized to reeive photoeletri alarms instead reeived the opposite type. Data were analyzed using an intention-to-treat approah in whih partiipating households were ategorized by alarm type to whih they were randomized. For 5 ionization and 2 photoeletri study households, original batteries were inoperable, and new alkaline batteries installed. These were exluded from analyses. Although main analyses inluded all eligible households reruited, a sensitivity analysis was onduted using only the 700 eligible households reruited using the blokwalk method, yielding omparable results. Baseline respondent and household demographis, strutural and heating harateristis, safety praties, fire histories, and alarm installation loations were ompared between study groups. Pearson s x 2 and Fisher s exat tests were used to ompare groups with respet to frequeny of household nuisane alarming and fire inidene between baseline and first follow-up, and between first and seond follow-up (overall and after exluding households with inoperable alarms due to battery or alarm removal, or with otherwise malfuntioning 82 Injury Prevention 2008;14: doi: /ip

4 alarms at first follow-up). The funtional status of study alarms at eah visit was ompared. Assoiations between alarm type and non-funtional or removed status at follow-up were estimated with umulative inidene risk ratios (RR) and 95% CIs. Effet-modifiation by respondent and household harateristis was explored using stratified analyses and the Mantel Haenszel test for homogeneity. Generalized linear models speifying the binomial family and log link, appropriate for ohort studies of ommon binary outomes, were used to adjust simultaneously for ombinations of respondent and household harateristis. 18 The presene of onfounding was evaluated by the magnitude of hange in RRs after adjustment for relevant fators, alone and in ombination. No appreiable hanges were noted after adjustment for potential onfounders; unadjusted RRs are presented. The majority of study variables were missing few values (0 2%). Exeptions inluded the years homes were built or remodeled, seondary heating soures, household testing of fire safety devies, and fire history. Partiipating households with missing values were dropped from analyses requiring the speifi missing variable(s). RESULTS Nearly all alarms (99%) were installed on main floors, with 65% in both ionization and photoeletri groups installed in hallways, dining rooms, or living rooms adjaent to kithens. Study alarms were installed in kithens for 20% of residenes randomized to reeive ionization alarms and 16% of those with photoeletri alarms; 16% and 18%, respetively, were installed elsewhere. Similar proportions of ionization (88%) and photoeletri (90%) alarms were installed on walls, the remainder on eilings. The mean (SD) distanes between alarm loations and kithen stoves were similar, at (44.1) and (47.2) inhes. Respondent, household, and house harateristis were similar for both groups, inluding the proportions of smokers and use of kithen stoves and fireplaes, with a few exeptions (table 1). A slightly greater proportion of respondents with ionization alarms had ollege degrees (44%) than those with photoeletri alarms (36%). Homes reeiving ionization alarms were slightly less likely to have been built or remodeled after 1980 (35% vs 43% of homes with photoeletri alarms), and have eletrial heating as their main heat soure (25% vs 32%), or wood-burning fireplaes (38% vs 47%). Homes with ionization alarms were slightly more likely to report natural gas as their main heating soure (54% vs 49%), or have gas fireplae inserts (5% vs 2%). The self-reported number of smoke alarms already present, presene of fire safety equipment, and residential fire history were similar aross groups (table 2). study group households reporting a prior residential fire were slightly more likely to have had fire-related injuries among residents (9%) than were those within the ionization group (6%). Nuisane alarms by non-study alarms between baseline and first follow-up visits were similarly ommon among homes with ionization (43%) and photoeletri (45%) study alarms (table 3). units, however, reportedly were more likely than photoeletri units to have alarmed (78% vs 39%, p,0.001), and alarmed more often (56% vs 17% had.3 alarm episodes, p,0.001). Similar differenes were observed for the interval between first and seond follow-ups. These differenes persisted at seond follow-up. alarms were not, however, more likely to have been removed than were photoeletri alarms. The RR assoiated with having an ionization study alarm, relative to a photoeletri alarm, be non-funtional or removed at first follow-up was 3.8 (95% CI 2.3 to 6.3), and 2.7 (95% CI 1.8 to 4.1) at seond follow-up (data not shown). When analyses for seond follow-up were restrited to homes with alarms observed to be funtional at first follow-up, the RR was 2.1 (95% CI 1.2 to 3.5). The RR assoiated with having a disonneted or absent battery or removed alarm observed among ionization study alarms, relative to photoeletri alarms, was 5.3 (95% CI 2.9 to 9.6) at first follow-up and 3.2 (95% CI 2.0 to 5.1) at seond follow-up. Restrition of seond follow-up analyses to homes with funtional alarms at first follow-up resulted in an RR of 2.6 (95% CI 1.4 to 4.7). Stratifiation by respondent gender, rae/ethniity, eduation, number of oupants, presene of hildren, the elderly, smokers, or alohol onsumers, did not alter these results. Smoking was reported as a soure of nuisane alarms only among 1% in the photoeletri group. Other reasons for nuisane alarming of study detetors were inense (,1% ionization group), andles (1% both groups), onstrution (,1% ionization, 2% photoeletri), and heat from lights (2% ionization group). DISCUSSION Compared to ionization alarms, photoeletri alarms were more likely to be funtional both 9 and 15 months after installation, and had fewer absent or disonneted batteries. The greater proportion of funtioning photoeletri alarms may be attributable to fewer nuisane alarms. Thus, photoeletri alarms may be preferred when a single unit is seleted by onsumers or safety ampaigns targeting similar populations, partiularly when the alarm is installed adjaent to a ooking area as in our study. These findings are onsistent with previous work onduted by this enter. The differene in proportions of homes with alarms present at baseline (11% London vs 90% our study) suggests wide differenes in population harateristis, and emphasizes the importane of appropriately designed injury prevention strategies. Smoke alarms are less ommon in rented than owner-oupied dwellings, 19 whih may aount for some of the differene. Although the London and Seattle-area study households ontained similar proportions of young and elderly oupants, smokers resided in nearly 50% of the London homes versus 24% in our study. If igarette smoking is more likely to ause nuisane alarming by photoeletri units (whih respond more quikly to smoldering fires), this differene in households with smokers may be partly responsible. Differenes in ooking styles, heating soures, ventilation, or use of wood-burning fireplaes may also have aounted for variation in air partiulates emitted, and resulted in differing false alarm patterns. Homes in our study, all single family residenes, were likely larger than in the UK study, potentially affeting the onentration and irulation of air partiulates, and relative potential for nuisane alarms by detetor type. Finally, although both studies targeted lower inome groups, our partiipants were likely of relatively greater soioeonomi status, given that home ownership was required for eligibility. Beause of the poor long-term funtional prognosis for either alarm type in the London study, the authors onluded that 84 Injury Prevention 2008;14: doi: /ip

6 sprinkler systems may provide the most effiient use of limited government resoures in that environment. This ontrasts with some studies in other environments, where smoke alarm installation programs appear effetive. 20 Limitations Although study staff diretly observed funtional status of the smoke alarms and reasons for non-funtion, the ourrene and reasons for nuisane alarms were partiipant-reported. Another potential limitation involved the method used to reruit households. Beause of low suess using targeted mail invitation, we swithed to blokwalk reruitment targeting high density areas and reruited people at home, given that mail response is generally lower than in-person ontat. 21 We attempted to redue potential bias assoiated with nonresponse by leaving letters if oupants were absent, and attempting re-ontat. Nonetheless, some potentially eligible homes ould not be reahed. The impat of this is unlear, but we doubt differential bias beause proportions onsenting and reasons for non-partiipation were similar between study groups. We are also reassured that the proportion of enrolled homes with existing smoke alarms is similar to that in surveys from the same time Potential biases may have been introdued in using two different reruitment methods, although there is evidene of no signifiant differenes in subjets identified by mailing and home visit. 23 We also obtained no information about presene or use of fans over ooking surfaes. Our results are based on single family, owner-oupied households, and it is possible that household harateristis may vary between those in the study and those of rental units or ondominiums. Finally, it should be noted that although we installed alarms in the room adjaent to the kithen, urrent reommendations are that alarms be installed on every level of a home, in partiular, outside every sleeping area. 24 Impliations for prevention Improving funtional status of smoke alarms is important. Even after installing alarms at no ost to homeowners, 10 25% were non-funtional at follow-up. Battery removal by residents was the most ommon reason for non-funtional alarms, with ooking most ommonly identified as the reason for nuisane alarms. Although lithium batteries may redue the prevalene of non-funtional alarms from dead batteries, they do not address the issue of frequent nuisane alarms, whih homeowners appear unwilling to tolerate. Design improvements to redue false alarms may help to redue disarming by residents. A reent survey of US department and home improvement stores shows that battery operated ionization devies are available for $10 (US), photoeletri devies for $15, and dual sensor units for approximately $25. Some manufaturers reommend that onsumers install both ionization and photoeletri alarms, or use hybrid units. The US Consumer Produt Safety Commission and NFPA guidelines desribe both tehnologies in their instrutional materials, but urrently do not reommend use of one over the other. The NFPA guidelines state that sine you an not predit the type of fire that will our, the slight differene is irrelevant. Either type of alarm will detet nearly every type of fire quikly. 24 Consumers or home safety/alarm distribution programs may, however, fous on ost. An alarm ontaining both tehnologies is more expensive; it may also be more likely rendered non-funtional if either tehnology auses frequent nuisane alarms. Our results suggest that installing photoeletri smoke alarms on main floors of Key points Among urban households in Washington state, ionization smoke alarms were more than twie as likely as photoeletri alarms to be non-funtional 15 months after installation. The Committee on Publiation Ethis (COPE) has established a Grant Sheme to fund researh in the field of publiation ethis. The Sheme is designed to provide finanial support to any member of COPE for a defined researh projet that is in the broad area of the organisation s interests, and speifially in the area of ethial standards and pratie in biomedial publishing. The projet should have a speifi goal and be intended to form the kernel of a future publiation. A maximum sum of 5000 will be alloated to any one projet, but appliations for smaller sums are welomed. The terms and onditions of the Grant are as follows: At least one of the appliants must be a member of COPE. Calls for appliations will be made twie a year with losing dates of 1 Deember and 1 June. An eletroni version of the appliation form must be sent to the Administrator no later than 12 pm (noon GMT) on the losing date for onsideration by COPE Counil. The appliation must ontain a lay summary of the projet, a definition of the question to be posed, suffiient methodologial detail to allow assessment of the viability of the projet, a lear timeline and a definition of the likely deliverables. A full justifiation for the sum requested must aompany the appliation. A report on the progress of the researh should be presented within one year of the award and at the end of the projet. The grant must be used within two years from the date of award, and balane sheets must be forwarded annually. These should be sent to the Administrator. Any remaining funds after two years must be returned. It is antiipated that the work stemming from the projet will be presented at one of COPE s annual seminar meetings within 2 3 years of the award. Suh data may also be published in peer-reviewed journals. Any publiations or related presentations at meetings by the reipient emanating in part or whole from COPE s support should be duly aknowledged and opies sent to the Administrator. Appliations are reviewed by a COPE sub-ommittee. Appliants will be advised of a deision as soon as pratiable after the deadline date.
